[Study on the maintenance of the accuracy of cytological diagnosis in the mass screening of uterine cervical cancer].

We studied the effect of the centralized cervical cytological screening program in Shimane prefecture and obtained the following results. After the centralized screening system was introduced, the rate detection of dysplasia almost doubled. The detection rates for carcinoma in situ and invasive carcinoma of the uterine cervix increased from 0.042% to 0.047% and from 0.042% to 0.065%, respectively. The cases with Pap. class III, IV, and V decreased to a third on the centralized cytologic screening system. The false negative rates for carcinoma in situ and invasive carcinoma decreased from 20.0% to 17.9% and from 16.0% to 7.4%, respectively. We concluded that the accuracy of the cytological diagnosis was improved by the establishment of the centralized system.

[Neonatal subdural hematoma secondary to tentorial tearing--clinical analysis on 48 survived cases].

In order to evaluate the treatments of subdural hematoma in neonates and to observe their prognosis, we carried out clinical analyses on 48 survived cases in the past three years from Jan. 1979 to Dec. 1981. Based on CT observations, hematomas were grouped into four types according to their locations as follows: Type I, localized around the posterior interhemispheric fissure in 25 cases (52%); Type II, extending from the posterior interhemispheric fissure to the hemispheric convexity in five cases (10%); type III, extending from incisura to the posterior fossa in 15 cases (31%); and type IV, having subdural hematoma accompanied by intracerebral hemorrhage in three cases (7%). Intracranial pressure was measured via the anterior fontanelle in 13 cases. Ten patients who had extensive hemorrhage were found to exceed above 200 mmH2O in the pressure. Ages of the patients studied were 0 to 7 days old. There were 36 mature infants (75%) and 12 cases immature (25%). The maternal history of delivery was primiparous in 27 cases (56%). The fetal presentations were 38 cephalic (79%) and 10 suction deliveries (21%). The fundus oculi was examined on 32 patients. Retinal hemorrhage was noted in 12 cases. Operations were performed on 13 patients in total; one case in type I, four cases in type II, five cases in type III and three cases in type IV. Functional prognoses were found to be as follows; Type I; normal, 15 cases, abnormal, four and undetermined, six: type II; normal, four and abnormal, one: type III; normal, 13, abnormal, one, and undetermined, one: and type IV; normal, two and abnormal, one case.

Gastric anti-ulcer and cytoprotective properties of 2-(3,4-dimethoxyphenyl)-5-methyl-thiazolidine-4-one in rat experimental ulcers.

Anti-ulcer properties of 2-(3,4-dimethoxyphenyl)-5-methylthiazolidine-4-one (KM-1146) were investigated on various experimental ulcers in comparison with cimetidine. While both KM-1146 and cimetidine at the same dose were comparative on the preventive effects of Shay- and stress-ulcers, KM-1146 was more effective against different necrotizing agents such as 0.2N NaOH and 99.5% ethanol at non-antisecretory dose. KM-1146 50 mg significantly accelerated the healing of acetic acid-induced ulcers while cimetidine 200 mg had no effect. KM-1146 increased rat gastric mucosal blood flow as measured by hydrogen gas clearance and prevented a decrease of gastric mucus gel thickness induced by water immersion restraint stress. These studies indicate that KM-1146 exerts cytoprotective action even in anti-secretory dose in addition to a significant anti-ulcer action which is comparable to cimetidine.

[Examination, prevention and treatment of congenital dislocation of the hip in the newborn infant--experience over an eighteen-year period].

On the basis of the authors' experience during the past 18 years, clinical data on CDH in the newborn were studied and summerized as follows. The use of diaper with hip-flexed position decreased not only the incidence of click, but also the frequency of CDH in infancy, indicating their effectiveness for the prevention of CDH. It is important to know whether the hips are dislocated or dislocatable at the original examination, when one decides on indication and initiation of treatment. The affected hips should be treated when "dislocated state" is found and the instability persists for 1 or 2 weeks or when "dislocatable state" is found and the instability persists for 2 or 3 weeks. Treatment with harness is useful for the newborn with CDH. The use will not cause any complication. Development of infants with CDH following after newborns with CDH is influenced by postnatal environmental factors as well as late diagnosis.

Pseudomonas labyrinthitis.

Pseudomonas aeruginosa is the gram-negative bacterial rod which is often isolated from chronic aural discharge. This microorganism may also cause necrotizing infection of the external auditory canal in certain patients with impaired host-defense mechanisms. Involvement of the inner ear by this microbe is extremely rare. In this communication, we report a case of pseudomonas labyrinthitis which resulted from traumatic middle ear injury. Infection produced massive granulations and extensive bone destruction of the otic capsule. This case shows that while P.aeruginosa is usually an avirulent opportunistic pathogen, it may also cause a highly destructive labyrinthitis if the inner ear is entered.

Bilateral cystic nephroblastomas and multiple malformations with trisomy 8 mosaicism.

The case of a 16-month-old female infant with bilateral cystic nephroblastomas, Dandy-Walker syndrome, microcephaly, bilateral cataracts, and cerebellar heterotopia is reported. The patient's older sister, who had had bilateral cystic nephroblastomas, botryoid sarcoma involving the vagina and urinary bladder, microcephaly, arhinencephaly, and bilateral cataracts, was described in a previous report. Chromosomal study in the present case confirmed trisomy 8 mosaicism (rate of mosaicism, 16 per cent). The familial occurrence and the chromosomal disorder suggest a syndrome involving genetic abnormalities.

Herpes simplex viral infection in human neonates: an immunohistochemical and electron microscopic study.

Specimens obtained at autopsy from six neonates with herpes simplex virus (HSV) infections were examined microscopically, electron microscopically, and immunohistochemically. Coagulative necrosis with inclusions was found in the livers and adrenal glands in all cases, as well as in various other organs, including the spleen, bone marrow, lungs, esophagus, tongue, and thymus, in some cases. Distinct hemorrhagic diathesis was found in three cases. No characteristic clinical findings, such as skin rashes or elevated titers of the antibody to HSV, were found, and clinical diagnosis was therefore difficult. In three cases isolation and typing of the causative virus were performed virologically, and type 1 HSV (HSV-1) was identified as the causative virus. Immunohistochemically, the type and distribution of the virus were evaluated in all cases with type-specific antisera to types 1 and 2 (HSV-2) antigens by the peroxidase-antiperoxidase method. In five cases the infections were found to be due to HSV-1 and in only one case to HSV-2. In the placenta in one case of HSV-2 infection, HSV antigen was demonstrated in the chorionic villi. Electron microscopic study confirmed the existence of viral particles in the placenta in that case and, thus, the possibility of a transplacental route of infection.

Gastroduodenal cytomegalovirus infection after renal transplantation. Fiberscopic observations.

Recently we experienced 2 adult post-renal transplantation cases with gastroduodenal cytomegalic inclusion body. In one case, it was possible to carry out close endoscopic observation from onset to resolution a - very rare opportunity. A 39-year-old man developed continuous occult blood in the stool for 1 month after renal transplantation. Multiple erosive lesions from the upper corpus of the stomach to the bulb of the duodenum were revealed by the endoscopic study. A biopsy specimen from the erosion showed the cytomegalic inclusion body. One month later, the erosive lesions had disappeared. The second case is a 40-year-old man who developed tarry stools 5 days after renal transplantation. Multiple erosions partially covered with blood coagula from the upper portion of the gastric corpus to the antrum were revealed by the endoscopic examination. A biopsy specimen from the erosion in the antrum contained the cytomegalic inclusion body.

Net release or uptake of histidine and carnosine in kidney of dogs.

Previous studies are equivocal as to whether the dog kidney produces histidine. Because one possible source of renal histidine is carnosine (beta-alanyl-L-histidine), we investigated net renal production (release) or utilization (uptake) (Qmet) of histidine and carnosine in 19 female dogs after they were fed histidine-free (9 dogs) or histidine-containing diets (10 dogs). Diets were fed in short-(2-11 days) or long-term (52-57 days) studies. Dogs were infused with half-normal saline for 120 min followed by an infusion of half-normal saline containing carnosine, 50 mumol/min. Renal Qmet histidine, calculated from either plasma or whole blood values, was positive during infusion of half-normal saline. During carnosine infusion, Qmet histidine increased markedly, and there was net renal uptake of carnosine. The Qmet histidine and carnosine were not different in the dogs fed histidine-free vs. histidine-containing diets. Thus there is net renal release of histidine in female dogs that increases when carnosine is administered. Qmet histidine or carnosine do not change adaptively when dogs are fed histidine-free diets.

Direct estimation of cardiac reserve through analysis of relation between oxygen consumption and heart rate during exercise testing.

In order to elucidate fundamental relationship between exercise and cardiac reserve, we investigated the parameters of regression equation (VO2 = A X HR + B) between oxygen consumption (VO2 ml/kg/min) and heart rate (HR) during exercise test (Bruce). 62 male children were divided into 3 groups based on history and clinical evaluation. HR related linearly with VO2 (average r = 0.967, range 0.90-0.993). Both of the parameters A and [B] were significantly high in Athlete (A = 0.465 +/- 0.043, [B] = 36 +/- 6.07), low in Failed (0.297 +/- 0.055, 17.4 +/- 6.1) and middle in Ordinary group (0.366 +/- 0.078, 23.5 +/- 8.6) (p less than 0.005). Based on our result of linear VO2-HR relation, and on the previously reported studies that cardiac output linearly and consistently relates with VO2, we considered the meaning of the parameters as follows; A determines the possible maximum stroke volume (SV), and B determines the HR where SV reaches plateau. In conclusion, since the SV is considered as a measure of cardiac function, measuring the parameters of VO2-HR relation allows us to isolate cardiac functional reserve through the exercise tolerance test.

Left ventricular diastolic pressure-volume and stress-strain relationship in children.

Diastolic pressure and volume (P-V) curves were approximately exponential and fitted the equation, dP/dV = aP + b, where a was left ventricular volume elastic constant. Stress and strain (sigma-epsilon) curves were expressed by the equation, d sigma/d epsilon = k sigma + c, where k was wall stiffness constant. These exponential curves have been fitted over the whole diastole, but theoretically, the mid-diastole should reflect diastolic elastic properties best. In the present study, therefore, special attention was paid to the mid-diastole in each patient, and both P-V and sigma-epsilon relationships were analyzed by fitting the data to the above mentioned curves during this period of time. This analysis was made in two separate groups of patients. One was the control group consisting of 2 patients with normal hearts, 2 patients with mild pulmonary stenosis, and 25 patients with post mucocutaneous lymph node syndrome. The other group consisted of patients with postoperative congenital heart disease, that is, 8 patients with atrial septal defect and 5 patients with tetralogy of Fallot. The elastic constant (a) could not be compared if the size of the hearts differed. The results of the present study, however, demonstrated that the size of the heart was closely related to the constant, a. In the control group, the constant (a) was exponentially related to the size of the heart, and expressed as follows: a = 0.30e-0.037EDV + 0.045 (r = 0.94, p less than 0.01) In contrast, the wall stiffness constant (k) was not related to the size of the heart. After surgical repair of congenital heart disease, the stiffness constant in the left ventricle was normal in patients with postoperative atrial septal defect, while it was significantly increased in patients with postoperative tetralogy of Fallot.

Clinical disorders observed in a beagle breeding colony.

Disorders in a beagle breeding colony were discussed, based on 472 clinical charts made in 1974-1983. In 201 neonates less than a week old, hypothermia associated with pneumonia was mostly seen. In 31 puppies from one week to two months old and in 46 young dogs from two months to a year old, pneumonia, canine parvovirus infection, dermal abscess and dermatosis were mostly found. In 91 dogs aged from one to five, trauma, intervertebral disc protrusion, dermal abscess, dystocia, claudication and otohematoma were frequently noted. In 103 animals over five years old, intervertebral disc protrusion, tumors, abscess, trauma and otohematoma were observed most often.
