The book of job and the modern view of depression.

The Biblical Book of Job is a wisdom book. Wisdom, in this context, refers to the intellectual discipline taught by the sages of ancient Isreael to provide professionals with a realistic approach to the problems of life. Chapter 30 of the Book of Job is a key-index of ideas that, if followed through the book, discloses a modern scientifically accurate description of a depression that, at times, was life-threatening. There are practical clinical clues to distinguish between normal mourning and depression, as well as aids to the differential diagnosis of somatic symptoms that may arise from depression. A timeless model of the scope and limitations of the professional relation between patient and comforter is also presented. Part of the wisdom of the Book of Job is to use depression as an example of a life-threatening illness to provide an unexcelled standard of clinical observation and medical intervention.

Immunocycte stimulation in vitro by nontoxic bacterial lipopolysaccharide derivatives.

Intact lipopolysaccharides (LPS), considered nonspecific enhancers of B cell responses, as well as nontoxic derivatives from Serratia marcescens LPS, were studied with regard to their ability to stimulate in vitro immune responses to a T-dependent antigen, sheep erythrocytes. Intact LPS, at a dose of 10 to 50 microgram, consistently enhanced the in vitro anti-SRBC immune response by normal splenocytes. The LPS also increased the background PFC response to SRBC in nonimmunized cultures. A chemically detoxified preparation derived from LPS (Mex B) had no stimulatory activity in vitro. A completely nontoxic, relatively small m.w., polysaccharide-rich preparation (PS), free of detectable lipid and protein, was stimulatory in vitro and at a dose of 10 microgram resulted in a 40 to 70% enhancement of the anti-SRBC response. The PS also stimulated an enhanced background response to SRBC as well as several other RBC species in nonimmunized cultures. PS had no mitogenic effect in vitro since addition of this bacterial derivative failed to stimulate thymidine incorporation into mouse splenocytes, as occurred with the intact LPS. The use of nontoxic preparations from gram-negative bacterial LPS for dissecting the stimulatory vs antigenic properties of bacterial products provides a model system for determining the role of a mitogenic stimulus in B cell activation.

[Terminology and chemotherapy of the so-called endodermal sinus tumor].

In addition to a discussion on the histogenetic, terminological and clinical problems concerning the so called endodermal sinus tumor a typical case in a far advanced stage is reported. Chemotherapy with vincristine and adriblastine was not able to influence the deleterious course of desease. Anyhow, since little is known about the effect of chemotherapy on the so called endodermal sinus tumor, this report might be of advantage.

[Needle biopsies of areas suspicious of recurrence in patients with gynaecological tumors (author's transl)].

With the biopsy needle of Franzén, 403 needle biopsies were carried out in 263 patients with gynaecological malignant tumors. In 152 cases the results of the aspiration cytology were compared with microscopic examinations or with the clinical course for at least 6 months. In 88 cases with probable recurrence the needle biospy showed a positive or suspicious result in 73 cases. A positive needle biopsy result was confirmation of recurrent tumor whereas a negative result did not rule out a recurrence. Aspiration cytology is a valuable ancillary method in the diagnosis of the recurrence of gynaecological tumors.

[The purity degree of the vagina containing mycoplasmosis urogenitalis (author's transl)].

In 156 women with mycoplasmosis urogenitalis the degree of purity of the vagina was determined. In none of the patient's a first degree of purity (containing lactobacilli only) was found. This is an additional sign that mycoplasma has an optional pathogenetic importance in the female urogenital system. In order to restore a normal vaginal flora of lactobacilli, the mycoplasma has to be eliminated.

[Urethritis syndrome and atypical germ flora of the exterior female genitalia (author's transl)].

A positive microbiological evidence could be obtained 54 times from the smear of the exterior genitals of 80 women suffering from complaints that were caused by urethritis, criteria of the examinations being sterile catheter specimen, negative cystoscopical findings, and missing indications to anatomical changes in the urethral region. Cytological examinations of these cases with regard to the vaginal epithelium had the following results:Group I:6 times; group II: 37 times; group IIW:8 times; group IIId: once; group IVa:twice. The cytological tests were carried out according to the method of papanicolaou as modified by Soost. Furthermore, we could state the following degrees of purity: Degree I: 8 times; degree II:16 times; degree III: 30 times. The cytological examinations of the urethral epithelium demonstrated, in 52 cases, an increased appearance of "nude" completely exposed epithelial cell nuclei--a fact corresponding to a degenerative autolysis (according to Wied). In the 26 women with missing atypical germ flora within the region of the exterior genitals, exclusively groups I (according to Papanicolaou and Soost) and degrees of purity I were stated. These persons also demonstrated remarkably grave psychical disturbances, especially in the intimate regions. In the cases of positive microbiological evidence, the following measures have proved satisfactory: Vaginal hygienization combined with a directly aimed antibacterial therapy, and the prescription of preparations containing lactic acid. A transitory discontinuation of contraceptives is being discussed. Our examination results are emphasizing the necessity of an analysis of the germ flora in cases of complaints arising from urethritis. Also psychical disturbances must be taken into consideration in cases of missing urological and gynaecological criteria of evidence.

[Mycoplasms in tumors of patients with carcinomata of the collum and its preliminary stages (author's transl)].

Within one year smears were taken from 190 patients who had not been pretreated in our oncological department. The culture media established were examined with regard to Mycoplasms. A Mycoplasma population was stated in 27 women. In the cases of carcinomata of the collum and in its preliminary stages, Mycoplasma infestation was about twice as frequent as in all other cases of genital carcinomata. Discussion is made if Mycoplasms are possible or promoting factors during the development of carcinomata of the collum.

Testing of drug delivery systems for use in the treatment of narcotic addiction.

The evaluation of the drug release characteristic of four naltrexone delivery systems has been carried out together with the development of analytical techniques and an investigation of the metabolic profile of naltrexone. Pharmacologic evaluation of the four delivery systems in the mouse indicated significant analgesic antagonism for a period of from 16-22 days. Further evaluation of one of these systems by measurement of the rate of excretion of radioactivity after administration of radiolabelled naltrexone in the delivery system confirmed that significant release occurs for a time period of about 15 days. Electron capture gas-liquid chromatographic assays for naltrexone and naloxone in plasma or urine have been developed that yield linear calibration curves and are sensitive to one ng/ml. Studies on naltrexone disposition indicate that (a) binding to plasma proteins in several species varies from 20-26%, (b) distribution of drug from blood is extremely rapid and extensive, (c) beta-naltrexol is a major metabolite of naltrexone in man, monkey and guinea pig among six species studies, whereas alpha-naltrexol is a minor metabolite in the monkey and guinea pig only, and (d) metabolic reduction of naltrexone occurs in the 100,000 x g supernatant of guinea pig liver. Pharmacokinetic studies of naltrexone in the dog and monkey indicate that the drug is rapidly distributed and eliminated, has a very large apparent volume of distribution and a total body clearance greater than the rate of liver blood flow.

[1st report on programmed chemotherapy of ovarian carcinoma].

A prospective study with regard to a cytostatic drugs combination treatment, partly synchronized and carried out in team work, of advanced ovarian carcinoma is being accomplished at the II. Women's Clinic of the University of Munich. The team consists of one oncologically orientated internal specialist, one radiotherapist, and one surgeon. The therapeutic procedure should be discussed; a report is given about the experiences gained so far in 41 patients, i.e. until May 1st, 1975.
