Visceral leishmaniasis infection in a rheumatoid arthritis patient treated with infliximab.

Anti-TNFalpha strategies can result in significant clinical benefits in rheumatoid arthritis (RA), but with an increased rate of opportunistic infections. Visceral leishmaniasis (VL) is a severe disease that can develop in immunocompromised hosts, principally in HIV patients. VL in RA patients treated with TNFalpha antagonists is an extremely rare event, and only one case has been described. Here we report a case of VL, occurring after 9 infusions of infliximab in association with azathioprine, in a patient who developed blood cytopenia, fluctuant fever, and splenomegaly.

[Re-expansion pulmonary edema after iatrogenic pneumothorax. A propos of 1 case].

Re-expansion pulmonary edema (RPE) due to pneumothorax aspiration can lead to a fatal outcome, as in the case reported, the chronic nutritional deficiency and hypoproteinemia that it provokes probably playing a contributing role. Pathogenesis and factors affecting prognosis of RPE are discussed. These include the duration of the pulmonary collapse, though this is not an essential factor, the alterations in alveolar surfactant activity possibly related to the chronicity of the collapse, and the abruptness of aspiration which is, in contrast, a determining mechanical factor. Finally, the hypoproteinemia present in certain cases could facilitate fluid extravasation towards the alveolus. It is concluded that aspiration should be a gentle procedure in all cases, and should be conducted with extreme caution in the presence of hypoproteinemia.

In vitro vasorelaxant effects of indolocarbazole and bis-indole protein kinase C inhibitors on rabbit renal arteries.

The effects of 12 compounds, structurally related to the indolocarbazole bacterial metabolite staurosporine, on caffeine-induced contractions in rabbit renal arteries were studied. Eight of these compounds are effective protein kinase C inhibitors, the others are inactive towards the enzyme. The results show a link between the protein kinase C inhibitory activity and the inhibition of vascular smooth muscle contraction. However, a strong inhibition of protein kinase C is required to observe the vasorelaxant effect.
