[Hematological aspects of dengue fever].

Fifty patients with dengue fever from Sept. 1987 to Jan. 1988 were studied for hematological features. The lowest blood counts values throughout the course of illness were Hb: 13.2 +/- 1.9 gm/dl, WBC: (2.77 +/- 1.63) x 10(3)/mm3, Platelet: (8.7 +/- 5.5) x 10(4)/mm3. Leukopenia (WBC less than 4000/mm3) was present in 38 (76%) of the cases and thrombocytopenia (platelet less than 10 x 10(4)/mm3) in 27 (54%) of the cases. Leukocytes reached nadir (1000-2000/mm3) at the 5th-6th day after fever onset, thrombocytes reached nadir (2.0-5.0 x 10(4)/mm3) at the 5th-7th day after fever onset. Bone marrow studies showed mild hypocellularity in the acute stage (less than 1 week) and normal cellularity in the convalescent stage (greater than 1 week). Megakaryocytes increased with various stages of maturation of megakaryocytes appearing in a majority of patients. Nuclear vacuolization of megakaryocytes could also be found. Bone marrow CFU-GM when performed within one week of illness showed no growth or low colony count, and was nearly normal after one week of fever onset. This study may suggest that leukopenia in dengue fever may be caused by virus-induced destruction or inhibition of myeloid progenitor cells. Thrombocytopenia may result from by destruction of peripheral platelet or bone marrow megakaryocytes by viruses which consequently reduce the platelet production.

Erythrocyte complement receptor type 1 in patients with systemic lupus erythematosus.

Erythrocyte complement receptor type 1 (CR1) was measured in 71 patients with systemic lupus erythematosus (SLE) and 43 healthy controls. The level of erythrocyte CR1 in patients with SLE was significantly lower than that of controls and correlated with the disease activity of SLE. The more active the disease, the greater the decrease in erythrocyte CR1. The level of erythrocyte CR1 was inversely correlated with the level of circulating immune complexes (CIC) and was positively correlated with C3, CH50, factor B, and factor H. We also found lupus nephritis more frequently in patients with decreased erythrocyte CR1 than in those with normal levels of erythrocyte CR1. Our results suggest that the deficiency of erythrocyte CR1 in patients with SLE is acquired and may serve as a variable of clinical disease activity and may play a role in the pathogenesis of SLE.

Production and further characterization of an alkaline elastase produced by alkalophilic bacillus strain ya-B.

The characteristics of the obligate alkalophilic Bacillus sp. strain Ya-B, which produces alkaline elastase extracellularly, were examined. This strain grew at pH 7.0 only in the presence of 1% or more NaCl. Its fatty acid distribution pattern was similar to that of other Bacillus species in which iso-C(15) and anteiso-C(15) were the most abundant fatty acids. About 120 mg of enzyme was recovered from 1 liter of culture broth in a medium (pH 10.1) containing mainly glucose, soymeal, and glycerol. The antiserum against this enzyme did not recognize microbial proteinases, such as subtilisins, but reacted with proteinase C, which was purified from commercial pronase. Chemical modification studies revealed that certain histidine and tyrosine residues might be involved in the enzyme activity. This enzyme underwent a partial unfolding at pHs higher than 12.0, as indicated by the circular dichroism study.

Influence of balloon size on initial success, acute complications, and restenosis after percutaneous transluminal coronary angioplasty. A prospective randomized study.

Restenosis after percutaneous transluminal coronary angioplasty (PTCA) is strongly associated with incomplete initial dilatation. To determine if oversized PTCA balloons would reduce the restenosis rate without increasing the risk of arterial dissection and acute complications, we prospectively randomized 336 patients to receive either smaller or larger balloons. Thirty-four percent of patients had multivessel disease and 18% had multisite dilatation. One hundred sixty-nine patients were randomized to PTCA with a larger balloon and 167 to PTCA with a smaller balloon. Balloon:artery diameter ratios were 1.13 +/- 0.14 in the larger group and 0.93 +/- 0.12 in the smaller group (p less than 0.001). The trial was halted as clinically important differences in acute complications emerged. Emergency bypass graft surgery, usually for the treatment of arterial dissection, was required in 7.1% of patients in the larger balloon group and 3.6% of patients in the smaller balloon group (p = 0.15). Myocardial infarction (Q wave and non-Q wave) complicated 7.7% of procedures in which large balloons were assigned and 3.0% of procedures in which small balloons were assigned (p = 0.056). There were no deaths in either group. The incidence of bypass surgery was 1.7% when the balloon:artery ratio was less than 0.9, 3.1% when the ratio was 0.9-1.1, and 7.8% when it was greater than 1.1. Stepwise logistic regression analysis demonstrated that larger balloon assignment, multiple lesion dilatation, and multivessel coronary artery disease were independent predictors of emergency surgery. Angiographic restudy rates were 50% in the larger group and 60% in the smaller group (p = NS).(ABSTRACT TRUNCATED AT 250 WORDS)

Soluble interleukin-2 receptor levels in patients with non-Hodgkin's lymphoma or with acute leukemia.

Enzyme-linked immunoabsorbent assay was used to study the serum levels of soluble interleukin-2 receptor (sIL-2R) in 30 patients with non-Hodgkin's lymphoma (NHL) (19, active disease; 11, complete remission) and 26 patients with acute leukemia (AL) (14, active disease; 12, complete remission). Thirty healthy age-matched volunteers were used as normal controls. The mean values of serum sIL-2R in NHL with active disease and complete remission (CR) were 1043 +/- 108 units/ml and 635 +/- 118 units/ml, respectively; these values are significantly higher than those in normal controls; 220 +/- 30 units/ml (p less than 0.001 and p = 0.001, respectively). The mean values of serum sIL-2R in AL with active disease and CR were 726 +/- 119 units/ml and 363 +/- 61 units/ml, respectively which were also significantly higher than those in the normal group (p less than 0.001 and p = 0.023 respectively). Comparing the active state with CR state in both diseases, sIL-2Rs in the latter state were significantly lower than those in active disease. Serum sIL-2R level might be a parameter of disease activity in NHL and AL.

Stability constants for complex formation between alpha-cyclodextrin and some amines.

Complex formation of alpha-cyclodextrin with 15 amines (including seven 4-substituted anilines) was studied by the potentiometric method, supplemented by direct UV spectrophotometry and a competitive indicator spectrophotometric method. The data were analyzed in terms of 1:1 and 1:2 complexes (amine-cyclodextrin ratios) and the stability constants K11a, K12a, K11b, and K12b were evaluated; the subscripts indicate the stoichiometry and conjugate acid-base form. For all amines K11b was greater than K11a and K12a was 0. On the basis of the relationship of complex stability to amine structure, it was concluded that the primary binding site in anilines is the 4-substituent.

Complex formation between alpha-cyclodextrin and 4-substituted phenols studied by potentiometric and competitive spectrophotometric methods.

Stability constants for complex formation between alpha-cyclodextrin and the conjugate acid and base forms of nine phenols were measured in aqueous solution at 25 degrees. The potentiometric method, in which the apparent acid dissociation constant of the phenol is measured as a function of cyclodextrin concentration, was supplemented by a modified version of a competitive spectrophotometric methyl orange method. For all phenols, the 1:1 stability constant for the conjugate base form (K11b) was larger than K11a for the conjugate acid form. Finite K12b values were found for phenols whose 4-substituents could tolerate a positive charge by electron delocalization. Complex stability, as measured by K11a and K11b, increases with electron density and polarizability at the 4-substituent. It is concluded that the 4-substituent is the sole or predominant site of binding for both the conjugate acid and base forms of the phenols. The general result that K11b is greater than K11a for any phenol is accounted for by relative delocalization of charge in the anion and neutral species.

Fractionation of nucleoli from auxin-treated soybean hypocotyl into nucleolar chromatin and preribosomal particles.

Nucleoli from auxin-treated tissues (Glycine max L. var Wayne or Kaoshiung No. 3) were isolated and purified by Percoll density gradient centrifugation. There was a 2.1-fold increase in RNA and a 2.8-fold increase in protein after a 24-h auxin treatment per unit nucleolar DNA. More than 150 acid-soluble protein spots were associated with the auxin-treated nucleoli on two dimensional (2-D) gel electropherograms.Nucleoli from auxin-treated tissue were fractionated by suspension in 20 millimolar dithiothreitol at room temperature for 20 minutes into two distinct fractions referred to as the nucleolar chromatin and preribosomal particle fractions. The DNA:RNA:protein ratio of the chromatin fraction was 1:2.5:14. Most of RNA polymerase 1 activity and nucleolar DNA recovered in this fraction. The acid-soluble proteins in the chromatin were resolved into 32 protein spots on 2-D gel electropherogram. The most abundant spots were identified as histones.The nucleolar preribosomal particle fraction had a DNA:RNA:protein ratio of 1:24:102 and contained only trace amounts of RNA polymerase 1 activity and only 10 per cent of the nucleolar DNA. Acid-soluble proteins associated with these particles were resolved into 78 protein spots; 72 of these (acid-soluble) protein spots corresponded in 2-D gel electrophoresis to 80S cytoplasmic ribosomal proteins. Some 15 protein spots found in 80S ribosomal proteins were absent in the preribosomal particles. It seems reasonable, based on these data, that the enlargement of nucleoli after auxin treatment is primarily due to the large increase in ribosomal proteins and rRNA which accumulate and assemble in the nucleoli in the form of preribosomal particles.

Potentiometric study of molecular complexes of weak acids and bases applied to complexes of alpha-cyclodextrin with para-substituted benzoic acids.

The theory of the potentiometric methods of studying complexes of ionizable substrates was developed, nd graphical techniques are described for obtaining stability constant estimations from the data. The method described is for a system in which the conjugate acid and base forms of the substrate (S), are capable of forming 1:1 (SL) and 1:2 (SL2) complexes with the ligand (L). It was applied to complexes of alpha-cyclodextrin (cyclohexaamylose) with 10 para-substituted benzoic acid derivatives. Letting K11a and K12a be stability constants for the conjugate acid forms of the substrates, and K11b, K12b for the conjugate base forms, it was found that K12b is zero for all substrates, K12a is zero for seven of the substrates, and K11a greater than K11b in every case. Hammett plots yielded p11a and p11b values of -0.31 and 0.77, respectively, which was interpreted to mean that K11a mainly represents binding at the carboxylic acid site, and K11b describes binding at the site of the para-substituent. This model of the complexing suggests that K12a represents binding at the para-substituent, and therefore K12a should vary roughly with substituent as K11b does; this trend was observed.
