Liver, lung and malignant disease in heterozygous (Pi MZ) alpha1-antitrypsin deficiency.

Theoccurrence of PAS-positive, diastase-resistant, inclusion bodies in hepatocytes has been used as a marker of heterozygous (Pi MZ) alpha1-antitrypsin deficiency in order to study the frequency of lung, liver and malignant disease in a consecutive autopsy material from a well defined population. Typical PAS-positive inclusion bodies were found in the liver in 26 (3.7%) of the 700 cases studied. In these 26 subjects both liver (cirrhosis or fibrosis) and lung disease (emphysema) were significantly more prevalent (p less than 0.001) than among PAS-negative controls. The prevalence of malignant disease did not differ between the groups. The results are consistent with the concept that heterozygous (Pi MZ) alpha1-antitrypsin deficiency predisposes to both liver and lung disease but does not influence the survival rate.

Dysplasia of the testis and epididymis.

Various congenital malformations of the testis and the seminal pathways are illustrated by 10 cases. Though these malformations are often free from symptoms, an increasing number of them are now being discovered in connection with investigations of male infertility. In cases of azoospermia where testicular biopsies show complete spermatogenesis, surgical exploration is indicated. Some of these malformations may then be treated by surgery, with fairly good prospects of restored fertility.

Late occurrence of precancerous changes and carcinoma of the gastric stump after Billroth II resection.

In a prospective study, patients resected for benign ulcer disease 20 years previously were followed up. Of 100 patients operated with anterior gastro-jejunal anastomosis and entero-anastomosis 51 were examined with gastroscopy and multiple biopsies taken from the stoma and gastric fundus. Acute and chronic inflammation of varying degrees were general findings. Single or multiple polyp-like changes at the stomal ring were found in 4 cases and in one patient an asymptomatic low differentiated, adenocarcinoma was found. Intestinal metaplasia and cystic dilatation of the gastric glands were most frequently found in the stomal biopsies and less frequent in biopsies taken from the gastric fundus. In order to evaluate these possibly precancerous changes all patients will be continuously controlled. Nineteen patients were dead at the time of the follow-up. One of these had died of carcinoma of the gastric remnant.
