Helping clients build links with their neighbours.

How does the effort we put into helping clients build links with their immediate neighbours compare with the time spent getting them out of their immediate street and into the local community? This article is aimed at purchasers and providers and examines the problems associated with establishing good relationships between clients and their neighbours. It also discusses the importance of finding the right property for clients' needs.

The provision and evaluation of a neonatal resuscitation program.

The need to train perinatal staff in neonatal resuscitation is widely accepted; however, standardized educational programs have not been available. This study used a randomized control trial to evaluate a one-day neonatal resuscitation education program with 190 nurses. Experimental subjects receiving the program had significantly improved knowledge and skill performance. Knowledge, but not skill performance, was maintained at six months for the experimental group. There was a significant relationship between subjects' self-rating of knowledge and performance, suggesting that this method could be used to prioritize staff for basic or refresher training. An effective format and evaluation instruments for neonatal resuscitation training have been developed. Strategies to maintain skills should be addressed in future research.

Fluorophotometric studies on postmortem changes in porcine corneal endothelial barrier functions.

Using an in vitro fluorophotometric technique, we studied the effect of postmortem storage duration on the endothelial barrier function of porcine corneas. A marked decay in endothelial barrier function was demonstrated that correlated with duration of storage. This decay appears to be nearly complete after 2 weeks of storage. This loss of barrier function correlated grossly with ultrastructural changes demonstrated on scanning electron microscopy.

Noninflammatory bacterial infiltration of a corneal graft.

A multifocal progressive infiltration of a corneal graft, which simulated a mycotic keratitis but was repeatedly negative on culture and unresponsive to antibacterial and antifungal therapy, was found upon regrafting to consist primarily of Gram-positive cocci. This bacterial colonization of the corneal graft was associated with epithelialized suture tracks and minimal inflammatory response. The immunosuppressive and bacteriostatic effects of chronic postsurgery low dose steroid and antimicrobial therapy may have played a role in the development of this atypical bacterial infiltrate.
