Evaluation of a class III biological safety cabinet for enclosure of an ultracentrifuge.

An evaluation of a special safety cabinet housing a high-speed centrifuge was made. The cabinet enclosed both the top access port and the drive and pumping machinery of the centrifuge. A titanium rotor was loaded with tubes containing a bacterial culture, weakened, and driven until rotor rupture occurred. There were several bent and broken components in the centrifuge, and bacteria leaked from the vacuum chamber. Although the forces were sufficient to displace the cabinet, none of the test bacteria were found outside the cabinet.

Extracellular fluid of the kidney preserved by the Collins technique.

Hilar drainage fluid of dog kidneys was analyzed as an approximation to renal extracellular fluid after preservation by flushing with chilled high K-low Na solution (Collins C4) followed by ice-cold storage for 24 and 48 hr in a bath of flushing medium. Compared with the medium, Na and Cl were increased to 30 mM/liter and K decreased slightly to 93 mM/liter. Glucose decreased, whereas lactate, lactic dehydrogenase, and creatine phosphokinase increased by significant amounts in both the drainage fluid and bath. The inulin space of the undrained kidney average 37% of wet weight. Calculated intracellular Na and Cl concentrations averaged 50 and 37 mM/kg cell water while K remained within normal limits. A significant fraction of red blood cells retained during initial flushing entered the effluent during storage. Bath and effluent composition of a human cadaver kidney approximated those of a dog.

Cellular proliferation in the urinary tract epithelium of the rat following temporary unilateral renal ischaemia.

A study of the effect of temporary unilateral renal ischaemia on the epithelium lining the renal pelvis and the bladder of the rat is described. There is a significant increase in the mitotic index in the pelvic epithelium of the affected kidneys, presenting a well-defined peak approximately 36 h after reflow. A similar but less pronounced cellular proliferation occurs in the bladder epithelium. The increased mitotic index in the renal pelvis and the bladder is not preceded by any detectable cell destruction at either site. It also has a different time course from the tubular regeneration which follows renal ischaemia.

'Someone who cares:' a qualitative investigation of cancer patients' experiences of psychotherapy.

Although psychotherapy for cancer patients is known to be effective, there is little in the research to indicate what elements of their therapy patients find most helpful. To explore this question, we interviewed cancer patients diagnosed with metastatic disease who had been offered two different forms of individual psychotherapy as part of a larger funded study. These interviews were then transcribed and analysed using grounded theory. Our aim was to explore patients' psychotherapy experience from their perspective and to determine what common elements in the two approaches they felt were of greatest benefit. Results indicated that patients offered cognitive behavioural therapy had similar experiences to those who received a type of relaxation therapy that included time for non-specific, patient-centred 'chat'. Central to participants' experiences was the opportunity both therapies gave them to enter a relationship in which they could safely share their thoughts and feelings with someone who seemed genuinely interested in understanding their cancer experience and 'truly cared'. These findings suggest that the unique perspectives of cancer patients can add considerably to our understanding of individual psychotherapy in cancer care settings and how this might be improved.

Measuring registration potential in planar transmission images.

One method of detecting change in an anatomical structure is by comparing elements of a longitudinal series of planar transmission images (i.e. radiographs). Unfortunately, changes in image formation parameters, such as patient placement, prevent successful image registration from occurring. In this paper, an analytic method is described which allows one to quantify the ability to register two planar transmission images. The concepts described in this paper are not dependent on a specific image registration method. In fact, these results hold independent of the registration algorithm used since they are based solely on phenomena occurring during the image formation process.

Instrument for scanning the angular variation of irradiance in ultraviolet phototherapy cabinets.

An instrument is described that measures irradiance and the angular distribution of the intensity of ultraviolet radiation sources inside phototherapy cabinets. Failed lamps and lamps with higher or lower outputs are readily identified. The measurements are controlled from outside the cabinet, thus reducing the risk to staff from exposure to ultraviolet radiation.

Portable biofeedback apparatus for treatment of anal sphincter dystonia in childhood soiling and constipation.

Biofeedback is an effective treatment for anal sphincter dysfunction in many cases of childhood constipation and soiling. Conventional methodologies centre on clinic-based manometric apparatus but are compromised by equipment being thinly available and practice sessions often infrequent. To counter these shortcomings, we have designed a portable anorectal biofeedback apparatus (the Easograph) for domiciliary use. It can be lent to families, enabling carers to mediate what is an intrinsically sensitive procedure. In this paper, we provide a technical description of the device which is based on an inexpensive pressure transducer. Casework experience suggests it is reliable in operation and readily understood by users. In a pilot study of eight patients treated with the apparatus, five achieved bowel continence after lifelong soiling in one to eight weeks of daily practice. We believe this is an innovative approach that has good potential for treating faecal retentive and expulsive disorders in childhood associated with poor anal sphincter control.
