Chronic paroxysmal hemicrania (CPH). The clinical manifestations. A review.

Eight definite and 10 possible cases of CPH are known to the authors. Decisive diagnostic features in the differential diagnosis versus ordinary cluster headache (Horton's headache) seem to be: the presence of headache every day, a high maximum daily attack frequency (greater than or equal to attacks/24 hours) and an absolute indomethacin effect. There is increasing evidence for a female preponderance in CPH. It emerges from this study that there frequently (or invariably?) is a pre-CPH stage with atypical attack pattern, usually lasting several years. Pregnancy seems to have a rather clear ameliorating effect on attack frequency and severity. In other patients, the very onset of headache is immediately after delivery. The importance of recognizing this special headache from a clinical point of view is clear since this disabling disorder can be completely abolished by drug therapy.

Cluster headache: relation to and comparison with migraine.

Numerous factors, such as location of pain, sex, frequency and pattern of occurrence, and symptoms, distinguish cluster headache from migraine. Cluster headache is characterized by severe unilateral periorbital pain. Attacks lasting from several minutes to several hours occur many times a day over a period of weeks to months. Opinions differ as to whether cluster headache is a variant of migraine or a completely different disorder. For relatively mild attacks, abortive treatment with ergotamine tartrate is usually successful. Cases which do not respond to abortive measures require prophylaxis.

Statistics on alcoholic marriages: an overview.

This paper is a brief American literature review reporting on (1) the percentage of alcoholics who marry, (2) the percentage who separate or divorce, and (3) the temporal relationship between date of marriage and onset of alcoholism. All studies discussed control for age and compare results to a general population. The authors show that early studies suggested that alcoholics were an unsocialized and homeless group of people who were not likely to marry whereas later research showed that alcoholics are just as likely as persons in the general population to marry but are at least four times more likely to separate or divorce. Multiple variables account for the high rate of marital disruption. The literature is conflicting regarding whether people tend to marry before or after the onset of alcoholism. Future research must delineate relevant psychological and sociocultural variables in addition to the alcoholism itself.

The clinical link between migraine and cluster headaches.

We describe seven patients with vascular headaches. Five of them had cluster headaches, which were preceded by migrainous scotamata (two patients), weakness contralateral to the pain (one), accompanied by ipsilateral photopsias (one), or by contralateral paresthesias (one). The other two patients had "clusters" of daily common migraine headaches separated by long free intervals. The symptoms of these patients suggest a common root for cluster and migraine headaches.
