Experimental ventricular preexcitation--type WPW syndrome.

Experimental ventricular preexcitation type WPW was performed in five dogs, through an artificial bypass, an electric conductor, made of copper, with a diameter of 8 mm and 12 cm in length. Whenever the conductor was put in contact with both the atrial and the ventricular wall, a WPW electrocardiogram pattern was obtained. This work has been done to better localize with epicardial mapping the site of preexcitation for surgical purposes.

Parasystolic ventricular tachycardias in young healthy patients.

Seven patients with short episodes of parasystolic ventricular tachycardia, in the absence of organic heart disease, and in good healthy state, are presented. The tachycardic episodes were analyzed through simultaneous esophageal and standard electrocardiographic recordings. The persistent fetal dispersion of nodal and Hiss bundle fragments within the ventricular septum is proposed as a possible explanation.

Emergency situations in sinus node dysfunction.

Sinus node dysfunction may produce in some cases life threatening medical emergencies. From 138 patients with sinus node dysfunction followed up in our department, we present 8 cases in which emergency situations required special therapeutical approaches (precordial thump, atrial or ventricular pacing, permanent pacemaker implantation). These situations appeared spontaneously, during the investigation procedures or after conversion of atrial fibrillation, performed for therapeutic purposes, when this arrhythmia concealed a sinus node dysfunction. The tachy-brady-arrhythmia could be influenced only by the implantation of a permanent pacemaker.

Precordial electrocardiographic mapping technique.

Using an original device, unipolar electrograms in 36 points on the anterior thoracic wall were recorded. The equal intensity contour lines of the amplitudes of electrical events were drawn as precordial maps, by mathematical procedures.

Ventricular pacing as a bedside method in the treatment of ventricular tachycardia.

In five patients with drug resistant ventricular tachycardia (VT) seven successful ventricular pacings were performed, using a bedside technique. The ventricular stimulation was always started with underdrive pacing, and in case of no result, continued with overdrive pacing. In two cases the underdrive pacing was successful, and in four cases only the overdrive pacing achieved suppression of tachycardia.

Diaphragmatic action potentials recorded on standard electrocardiogram. A clinical and experimental study.

A transient pseudoatrial dissociation (Deitz-Marques phenomenon) occurring immediately after right atrial pacing in a 44-year-old woman, and spontaneously together with periods of hiccups in a 50-year-old man are presented. The same phenomenon was produced experimentally in three dogs, after rapid diaphragmatic stimulation. The diaphragmatic origin of fibrillary-like waves on standard ECG is argued because of dependence on respiratory phases and because it does not influence the basic sinus rhythm.

Hypothesis of transition in two ways from atrial fibrillation to sinus rhythm.

A hypothesis of transition from atrial fibrillation to sinus rhythm in close relation with monophasic action potential duration is proposed. The first way: the prolongation of the right atrial refractoriness reduces the wave fronts below a critical number and their collision terminates the arrhythmia. The second way: progressive shortening of refractoriness at a critical level with block and collision of wave fronts.

Postextrasystolic changes in ventricular depolarization (electrocardiographic observations).

Postextrasystolic changes in ventricular depolarization were found in patients with coronary artery disease in two situations: a) after a compensatory postextrasystolic pause and, b) after an interpolated ventricular beat. The explanation and the underlying conditions of this phenomenon are discussed. In our opinion the postextrasystolic change may be a silent myocardial ischemia.

Role of ketanserin in the treatment of the essential arterial hypertension associated with ischemic heart disease.

This clinical study was set-up to evaluate the antihypertensive effect of ketanserin in essential hypertension (EH) associated with asymptomatic ischemic heart disease (IHD) in twenty-seven patients. Every patient received 20-40 mg ketanserin, a selective S2 serotoninergic antagonist, twice daily, for a three-month period. The blood pressure (BP) was significantly lowered during the treatment with ketanserin, much more in female patients and elderly group.

Electrophysiologic evaluation of extrinsic and intrinsic sinus node dysfunction.

Electrophysiologic investigation was done for ten patients with clinical and ECG criteria for sick sinus syndrome (SSS). Sinus node function was assessed through corrected sinus node recovery time (CSNRT) and sinoatrial conduction time (SACT), before and after autonomic blockade (AB) with propranolol 0.2 mg/kg and atropine 0.04 mg/kg, given intravenously. Four patients having electrophysiologic parameter in normal range before AB were not considered as SSS. In three patients extrinsic sinus node dysfunction (SND) was found, the pathologic values of CSNRT and SACT being normal after AB, and in one patient with sinus bradycardia, the sinus node function was difficult to be appreciated. In two patients only intrinsic SND was found and, in consequence, pacemaker device was implanted.
