Searching for hidden information with Gabor Transform in generalized tonic-clonic seizures.

The analysis of generalized tonic clonic seizures is usually difficult with scalp EEG due to muscle artifact. We applied Gabor Transform to evaluate 20 seizures from 8 consecutive patients admitted for video-EEG monitoring. We studied the relative intensity ratios of alpha, theta and delta bands over time. In 14/20 events we found a significant decremental activity in the delta band at the onset of the seizure indicating that this is dominated by theta and alpha bands. We conclude that GT is a useful auxiliary tool in the analysis of ictal activity that sheds light on the underlying pathophysiological mechanisms.

Protein separation by electrophoresis in a nonsieving amphoteric medium.

A numerical model has been used to study the separation of protein mixtures by zone electrophoresis in a nonsieving amphoteric medium. An amphoteric buffer fixes the pH of the solution close to its isoelectric point, where the buffer molecules are unchanged: they thus contribute very little to the conductivity of the solution. This means that high field strengths can be used for rapid separation without sacrificing resolution. The numerical study shows that in this process the band spreading that can reduce resolution is essentially due to differences in migration rate between protein-rich and low-protein zones: both differences in solution conductivity and in protein mobility are involved. Rules for judging the buffer capacity of amphoteric molecules are presented and it is shown how, for a given protein, the effectiveness of this technique varies with the range of pH in which it is applied.

[An adenocarcinoid tumor of the vermiform appendix].

Goblet cell carcinoids of the vermiform appendix are uncommon. A comprehensive review of the literature revealed about 100 reported cases. Clinical presentation included: asymptomatic patients, acute appendicitis, and/or chronic intermittent lower abdominal pain with or without a palpable mass. We report the case of a 57 year-old woman with lower abdominal pain, nausea and vomiting. Normal blood tests. X-ray of the abdomen showed dilated small bowel loops with fluid levels. Initially, conservative treatment was started. After three days with no clinical improvement, the patient was operated on. An appendicocecal inflammation was found and a terminal ileum plus cecal resection with an end-to-side anastomosis was performed. The pathological diagnosis was goblet cell carcinoid. The patient shows no evidence of recurrence after 1 year follow-up.

[Gastric adenocarcinoma following the treatment of a gastric non-Hodgkin's lymphoma].

The association of gastric non-Hodgkin's lymphoma (NHL) and the subsequent development of gastric adenocarcinoma is an exceptional finding. In our series of 24 cases of primitive NHL of the stomach diagnosed between 1966 and 1985 only one developed gastric adenocarcinoma. It was a 38-year-old male with a gastric NHL of immunocytoma type and in the stage IE of the Ann Arbor classification. The patient was treated with subtotal gastrectomy and adjuvant radiotherapy. Twenty-seven months later an adenocarcinoma of the gastrojejunal anastomosis mucosa was found. In the review of the literature we have found 17 cases of this association. Gastric adenocarcinoma developed 3.5-34 years after the treatment of gastric NHL. The implicated factors in the development of this association are: gastric resection, chemotherapy, radiotherapy and NHL itself.

[Neonatal Chagas disease: laboratory diagnosis during the first year of life].

This paper describes the parasitologic and serologic studies carried out during the first year of life in 721 pediatric patients born to mothers serologically positive for Chagas disease. The search for circulating trypomastigotes was performed by Strout, blood culture and/or Xenodiagnosis. In some cases, amastigotes were also detected in placenta and umbilical cord. Complement fixation test, indirect hemagglutination and indirect immunofluorescence were used to detect Trypanosoma cruzi antibodies. The dosage of total Ig by single radial immunodiffusion was also carried out. The results obtained showed an absolute correlation between parasite detection and the persistence of antibodies after six months of life. In the first group (GI) formed by 8 children, the diagnosis of congenital infection could not be confirmed because the isolation of T. cruzi was obtained only in later studies. In another 8 children grouped in GIII, it was impossible to detect parasitemia, and the diagnosis was reached by the serological positivity after six months of life. Finally, in 684 patients with anti-T. cruzi antibodies at birth, the serology became negative at the age of 3 months (GIV) or 6 months (GV). The methodology employed in this work is accessible to laboratories of medium complexity, and permits the diagnosis of congenital or neonatal chagasic infection with a high degree of reliability. On the other hand, it avoids unnecessary administration of trypanomicide drugs in a number of newborn and infants who have only received maternal antibodies at birth and were not infected by T. cruzi.

Quality assurance in radiation therapy: systematic evaluation of errors during the treatment execution.

A method to control the quality of the radiotherapy treatment during its execution has been developed. The method is based on a wide use of verification films in the practical routine of a centre together with a statistical analysis of the data obtained from them. The stepwise multiple regression analysis is applied in order to assess the influence, on the quality of the treatment, of a set of chosen factors with a certain clinical or technical significance. A test of the method is presented and the results are discussed.
