[Acute epiglottitis. 27 years of experience with and future development of nasotracheal intubation].

During a 27-year period 295 patients in Copenhagen County were treated for acute epiglottitis. One hundred and eight-two were adults and 113 were children. Most children were treated by nasotracheal intubation while only a few adults required nasotracheal intubation in order to secure the airway. The incidence of acute epiglottitis in children less than five years old was calculated to be 8.7/100,000 with minor annual variation. Since vaccination against Haemophilus influenzae type b has been implemented in Denmark from May 1993, this figure will probably be markedly reduced, and the disease even may be even eradicated in children, but in adults the same reduction cannot be expected as the causative agent in this group is less frequently Haemophilus influenzae type b. It is emphasized that this reduced incidence will make it even more important that each department engaged in the treatment of suspected acute epiglottitis have a departmental protocol for management of the condition, so that the future rarity of acute epiglottitis does not cause the mortality rate to rise.

Tympanic membrane ossification.

Ossification of the tympanic membrane after myringoplasty is recorded for the first time. Myringoplasty was performed for closure of a perforation which followed the surgical treatment of otitis media with effusion and had included the insertion of a long-term T-tube.

Assessment of potential ototoxicity following high-dose nebulized tobramycin in patients with cystic fibrosis.

A deterioration in pulmonary function is a common problem in patients with cystic fibrosis. One strategy which might help to arrest this process is the administration of aminoglycosides by nebulizer in dosages which greatly exceed those which are normally given parenterally. Ten patients (4-19 years of age) with cystic fibrosis underwent evoked response audiometry at regular intervals following a 400 mg dose of nebulized tobramycin; the concentrations of tobramycin in samples of blood taken at the same time intervals were also determined. No significant abnormalities in the audiometric investigations were detected. The relationship between shifts in vestibulocochlear potentials and long-term ototoxic sequelae in patients receiving aminoglycosides via a nebulizer requires further evaluation.

Acute epiglottitis--25 years experience with nasotracheal intubation, current management policy and future trends.

During a 25-year period 168 adults and 111 children in Copenhagen County were treated for acute epiglottitis. Four patients, two children and two adults died, of these the two children and one adult had a cardiac arrest on arriving at the hospital. Most children were treated by nasotracheal intubation while only some adults required nasotracheal intubation in order to secure the airway. Our data indicate that intubation of adults with epiglottitis is technically more difficult than in children. The fibrelaryngoscope, a new diagnostic tool, is advocated, and was in this study used to establish the diagnosis in 12 unclear cases of acute epiglottitis. The incidence of acute epiglottitis in children was calculated at 3.2/100,000 with a minor annual variation. As vaccination against Haemophilus influenzae type b becomes more common, the incidence will probably be markedly reduced, maybe even eradicated in children, but in adults the same reduction cannot be expected as the causative agent in this group is less frequently Haemophilus influenzae type b.

[Fever, somnolence, vomiting, diarrhea].

A 69-year-old female was admitted because of a febrile state, somnolence, nausea and diarrhea. Addison's disease known to have developed several years earlier after adrenal tuberculosis suggested Addisonian crisis triggered by an acute infection. Therapy with steroids and substitution of electrolytes and fluid led to rapid improvement. Streptococcus bovis was identified as causative agent. The known association of this germ with cancer of the colon led to further investigation which revealed cancer of the sigmoid and incidental cancer of a salpinx. After surgical removal of both cancers, the patient has remained free of complaints under substitution of steroids.

Cervical osteophytes: their significance in ENT practice (ankylosing vertebral hyperostosis--Forestier's disease).

The condition known as ankylosing vertebral hyperostosis or Forestier's disease is described. Eight cases are reviewed where the radiological findings warranted this diagnosis. Their presenting symptoms are summarized as are their responses to treatment. The relevance of this diagnosis is discussed with emphasis being placed on the importance of thorough investigation of these patients. In those for whom no other cause has been found for their symptoms, the various therapeutic options are outlined with the possible complications.

Cystic hygroma presenting in adulthood.

We report a typical case of cystic hygroma presenting in an elderly female patient. A review of the literature on this condition reveals that although rare in adults, it should form part of the differential diagnosis of a cystic swelling in the neck in this group of patients as well as in the more commonly observed juvenile population. The embryology and pathogenesis of lymphangiomata are discussed as well as the recommended management of a cystic hygroma in an older patient.

Sub partu monitoring of the fetus by cardiotocography and fetal blood analysis.

A retrospective study to determine how well cardiotocogram (CTG) and micro blood analysis (FBA) results correspond to each other was made on 200 deliveries which had been monitored sub partu by these techniques. FBA was performed 306 times. CTG results were analysed according to Hammacher's method and the scores compared with the FBA results. In 40 cases with a normal Hammacher score, a normal FBA-pH value, as expected, was also found. In a total of 266 observations with a CTG score greater than two, there were simultaneously 10 times (scarcely 4%) found when there was a FBA value under 7.20. Of the 200 infants involved, 25 had a cord arterial pH value under 7.20. Ten minutes after birth, all of these infants were doing fine, with an Apgar score of at least seven. Statistically, sub partu CTG analysis according to Hammacher's method is highly sensitive, yet not very specific, when compared to the corresponding FBA-pH value.

[Multicenter experiences with the intracervical administration of a new PGE2 gel in labor induction].

With the participation of four Swiss obstetric clinics, medically indicated inductions of birth (with living fetuses) were performed using a new, stable PGE2 gel, and documented according to a uniform protocol. The study was conducted to investigate the efficacy of 0.5 mg of PGE2, in 2.5 ml of a vehicle (Triacetin) not yet commercially available, for local cervical maturation (n = 41). Thirty-nine patients selected by prospective randomization, in whom birth was induced conventionally, served as a control group. The efficiency of the prostaglandin gel alone or respectively with additional administration of oxytocin was evaluated on the basis of the clear changes in the cervical findings observed within 12 or respectively 24 hours, the spontaneous births, or, in the case of cesarean deliveries, according to the pelvic score. Application of PGE2 alone led to impressive changes of the cervix score and, in 34 of the 41 cases, to regular contractions after an average time of 87 minutes. After 12 hours, prior to administration of oxytocin, 43% of the patients were already delivered. The combination of locally applied PGE2 gel with conventional oxytocin induction significantly increases the number of successful inductions. The percentage of unsuccessfully attempted inductions was reduced to 24% in the PGE2 gel group as compared to 44% in the control group.
