Sonographic findings of uterine leiomyosarcoma.

We describe herein various sonographic features of uterine leiomyosarcoma. Transabdominal scanning (TAS) revealed an image indistinguishable from benign leiomyoma, with evidence of degeneration. Transvaginal scanning (TVS) clearly showed the thinness of the myometrium, and the possibility of deep myometrial invasion was suspected at intraoperative open direct ultrasonography (ODU). A very high peak systolic velocity and a slightly increased diastolic component at the periphery of the tumor were evident with pulsed Doppler ultrasound. Thus, TAS, TVS, ODU and Doppler ultrasound will yield useful information for the physician attempting to evaluate the extent and vascularity of uterine leiomyosarcoma.

Efficiency of transvaginal scanning in obstetric and gynecologic fields.

Transvaginal ultrasonography was done on 26 pregnant women ranging from 5 to 27 weeks of gestation and 12 patients with gynecologic diseases. In early gestation, significant correlations were found among gestational age, crown-rump length, diameter of yolk sac and fetal heart rate. The maximal length of the gestational sac did not correlate with any other parameter. Transvaginal image quality was better in 76.2% in obstetric field and in 66.7% in gynecologic field, as compared to findings with conventional abdominal ultrasonography, respectively. Fine structures and individual organs were better evidenced transvaginally. Therefore, transvaginal ultrasound affords pertinent information complementary to that obtained with the transabdominal technique.

Magnetic resonance imaging of the fetus: initial experience.

Magnetic resonance imaging (MRI) examinations were made on 9 high-risk fetuses (poor fetal growth, 3; maternal manic-depressive psychosis, 1; maternal systemic lupus erythematosus, 1; habitual intrauterine fetal death, 1; twin pregnancy with 1 fetus in distress, 1; fetal hydrocephalus, 1), ranging from 29 to 36 weeks of gestation. T1 weighting sequences were better than T2 weighting sequences for in utero fetal imaging. Fetal movement significantly degraded the imaging quality. In all fetuses, cerebrum, cerebellum, heart, lung, liver, stomach, bladder and extremities were identified and in 1 fetus, even the cochlea was seen. In 2, the four-chamber view of the heart was evident. The signal intensity of the lung was lower than that of the liver in T1 weighting imaging. In the case of maternal lupus erythematosus, a cavum veli interpositi was noted. In the fetus with hydrocephalus, clear images of the intracranial structure were obtained. Similarly, location of the placenta was clearly depicted. MRI is currently less than satisfactory to determine the fetal anatomy, however, it does provide unique and characteristic information that complements the superior anatomic imaging by ultrasonography.

[Anticholinergic action of Paeony root and its active constituents].

Anticholinergic action of Paeony root was examined in in vivo experiments with rat in order to substantiate the presence of analgesic, antispasmic and antidiarrheal properties. The 50% methanol extract of Paeony root was found to be effective. Fractionation of 50% methanol extract through column chromatography revealed that paeoniflorin was one of the active constituents in anticholinergic action in vivo, but in vitro, paeoniflorin had no effect on contractile responses of isolated rat proximal colon to the carbachol and KCl.

A cytotoxic antibody produced by allo-skin transplantation in cats.

A high titer of cytotoxic antibody against donor lymphocytes, tentatively designated as anti-F1, was found in one cat who received 3 times of skin allograft from the same donor. The anti-F1 antibody titer was 1:16, and the antibody activity was mostly detected in the IgG fraction. A cross-absorption test using erythrocytes from F1-positive animals revealed that the F1(+) antigen recognized by anti-F1 antibody was expressed on erythrocytes as well as lymphocytes. The frequencies of F1(+) and F1(-) types in 81 healthy cats, involving all the cats used for allo-immunization in the present study, were 30.9 and 69.1%, respectively. Moreover, F1 antigen was considered to be a hereditary antigen from family studies.

Accumulation and metabolism of 2,4,6-trichlorophenyl-4'-aminophenyl ether by carp.

1. Accumulation of 2,4,6-trichlorophenyl-4'-aminophenyl ether (CNP-amino) in carp was investigated for 14 days. CNP-amino in carp could be divided into free CNP-amino [CNP-amino(I)] and bound CNP-amino [CNP-amino(II)]. 2. The bioconcentration factors (BCF) of CNP-amino(I + II) in carp were 90 +/- 38 (mean +/- SD, n = 3) for muscle, 402 for liver, 501 for kidney and 5368 for gallbladder after 14 days exposure. 3. 2,4,6-Trichlorophenyl-4'-acetamide phenyl ether (CNP-acetamide) was detected as metabolites of CNP-amino in muscle and viscera of carp.

Bioconcentration and excretion of diazinon, IBP, malathion and fenitrothion by carp.

1. Bioconcentration and excretion of diazinon, IBP, malathion and fenitrothion were studied for carp (Cyprinus carpio L.). 2. The concentrations of these pesticides in muscle and viscera of the carp reached plateaus in 12-48 hr exposure. 3. The average values of bioconcentration factors (BCF) for diazinon were 20.9 in muscle, 60.0 in liver, 111.1 in kidney and 32.2 in gallbladder over the 168 hr exposure period. Similarly, those values were 4.3-26.7 for IBP, 2.7-17.3 for malathion, and 36.0-157.1 for fenitrothion. 4. The excretion rate constants of malathion (hr-1) were 0.13 for muscle, 0.12 for liver, 0.08 for kidney and 0.06 for gallbladder. Those of diazinon, IBP and fenitrothion (g.ng-1.hr-1) were 0.002-0.024 for muscle, 0.001-0.020 for liver, 0.0004-0.004 for kidney and 0.002-0.023 for gallbladder, respectively.

Evidence for the presence of rat liver glucokinase in the nucleus as well as in the cytoplasm.

Subcellular distribution of glucokinase was studied in rat liver. With an immunohistochemical procedure, glucokinase immunoreactivity was clearly shown in the nucleus of parenchymal cells of rat liver, but faintly in the cytoplasm. Nuclei, cytosol (extranuclear fraction in the strict sense), and homogenate prepared in nonaqueous medium, i.e. glycerol, were analyzed for glucokinase by both immunoblotting and activity measurement. Such analyses demonstrated that glucokinase concentration was far higher in the nuclei than in the cytosol when compared on the basis of total protein content and that total glucokinase activity in the cytosolic fraction was about 1.8 times that in the nuclear fraction. These results indicate that hepatocyte glucokinase is present in the nucleus as well as in the cytoplasm in contradiction to the widespread belief of its exclusive localization in the cytoplasm.

[Clinical results of total knee arthroplasty using Kinematic type in patients with rheumatoid arthritis under the age of 50 years].

We reviewed 24 total knee arthroplasties using Kinematic type (anteriorly joined type or posterior cruciate retention type) in 18 rheumatoid patients who were under the age of 50 years. Thirteen knees with Kinematic type total knee arthroplasty in 10 rheumatoid patients who were over 70 years of age were also reviewed. Using rheumatoid arthritic knee rating score of J.O.A. (Japan Orthopedic Association), the clinical results of young patients were evaluated and were compared with those of elderly patients. The average total score of young patients and that of older patients was almost equally increased after arthroplasty. Postoperatively in older patients, pain score was elevated more remarkably than in young patients. However, increase in score of quadriceps muscle strength, walking ability and climbing stairs ability were less in older patients than that in young patients. Range of motion was not improved in both young and older patients. However, flexion contracture was improved remarkably in both groups.

[A case of pulmonary thromboembolism due to circulating lupus anticoagulant].

A 22-year-old man was admitted because of hemosputum and progressive dyspnea with 3 attacks of chest pain and dyspnea over the previous 4 months. Chest roentgenography showed pulmonary infarction of the left lower lobe, and the diagnosis was confirmed by pulmonary perfusion and inhalation scintigraphy and pulmonary arteriography. Thrombolytic therapy was performed, but no significant effect could be obtained and anticoagulant therapy was performed continuously. No deep-vein thrombosis could be seen. He was considered to have autoimmune hemolytic anemia with lupus anticoagulant on the basis of auto-antibody data. Lupus anticoagulant is an antibody to phospholipid, and it is suggested that a decrease in the production of prostanoid in the endothelium causes thrombosis. In this case, as the patient showed a low level of 6-keto-PGF1 alpha in the blood, it is suggested that one of the etiological factors of pulmonary thromboembolism is a disorder of prostacyclin production in the endothelium, causing thrombosis by lupus anticoagulant.

Histo-pathological study of pulp response to a composite resin restoration with two lining materials.

This histopathological study investigated the pulp reaction to a restoration system employing a posterior composite resin with or without the pulp protection of visible light curing calcium hydroxide composition and alpha-TCP cement lining to dentin. Black's class V cavities were prepared in 120 adult dog teeth. They were then extracted for histological examination. As a result of this study, their lining materials were found to be effective in pulp protection. To understand the pathological finding, the pH values of "Fulfil", "Universal bond", "VLC Dycal" and "Vitacemen Type II" were measured. The pH levels of "Fulfil" and "Universal Bond" were mildly acidic (4.79-5.18) before polymerization, with no subsequent changes. "VLC Dycal" was initially a strongly alkaline (11.75) and remained this condition. "Vitacemen Type II" was initially acidic (3.78), but eventually reached the milder acidity of 5.12 after 24 hours.

[Transition of ovarian arterial compliance during the human menstrual cycle, assessed by Doppler ultrasound--correlation with serum hormone levels].

Ovarian arterial velocimetry was performed using color and pulsed Doppler ultrasound. Seventy-one examinations were done on 9 healthy women with regular menstrual cycles. The change in ovarian arterial compliance was based on the calculation of the pulsatility index (PI). In the active ovary carrying a dominant follicle or corpus luteum, PI in the early proliferative phase was significantly higher than that in the late proliferative phase (p less than 0.001), and PI in the early secretory phase was significantly lower than that in the late proliferative phase (p less than 0.001). PI became significantly higher in the late secretory phase than that in the early secretory phase (p less than 0.001). In the inactive ovary without a follicle of corpus luteum, no changes were seen among the values for PI, in any menstrual phase. There was a significant difference between the values for PI in the active ovary and the inactive ovary in the late proliferative, the early and the late secretory phases (p less than 0.001), respectively. The PI values for the active ovary significantly correlated with the serum progesterone levels (r = -0.53, p less than 0.05) but not with the estradiol levels. These findings provide a useful foundation for assessing ovarian hemodynamics during the menstrual cycle.

[A case of triple carcinomas: stomach carcinoma surgically removed, renal tumor, and carcinoma of the remnant stomach].

Reported is the case of a 57-year-old male patient, who manifested tarry stool and who had undergone a subtotal gastrectomy at our hospital in 1983 for an early carcinoma, type IIc, which proved to be a well differentiated tubular adenocarcinoma. Three years later, he returned complaining of epigastralgia, although no evidence of a recurrence could be seen. An abdominal ultrasonogram and a CT scan, however, revealed a right renal tumor and he subsequently underwent an operation for the removal of a renal cell carcinoma, a clear cell type. In 1988, he again was readmitted to hospital, this time because he easily became fatigued. An upper GI series and gastroscopy located a polypoid lesion in the remnant stomach. Thus, a total gastric resection was performed, and the lesion was diagnosed as being a well differentiated tubular carcinoma, an early gastric cancer, type I. Triple carcinomas are very rare, and the details of this case are discussed.

[Laboratory and clinical studies of cefodizime in pediatric field].

We have carried our laboratory and clinical studies on cefodizime (CDZM, THR-221). The results were summarized as follows. CDZM was given by 30-minute drip infusion to 2 children at a single dose of 10 mg/kg and to 2 children at a single dose of 20 mg/kg and to 3 children at a single dose of 40 mg/kg. After the 30-minute drip infusion, mean serum levels of CDZM obtained for the 3 dose levels were 76.16 +/- 5.52 micrograms/ml, 170.49 +/- 16.70 micrograms/ml, 270.01 +/- 50.44 micrograms/ml at the end of injection, respectively, and serum half-lives were 2.03 +/- 0.78 hours, 2.03 +/- 0.38 hours, 2.28 +/- 0.30 hours, respectively. The mean urinary excretion rate of CDZM were 83.3 +/- 22.3%, 73.1 +/- 13.9%, 51.1 +/- 8.5% in the first 8 hours after the 30-minute drip infusion of 10 mg/kg, 20 mg/kg, 40 mg/kg, 40 mg/kg, respectively. Treatment with CDZM was made in 28 cases of pediatric bacterial infections; 5 cases of tonsillitis, 2 cases of bronchitis, 10 cases of pneumonia, 6 cases of enteritis, 3 cases of urinary tract infection and 1 case each of maxillary sinusitis and laryngitis. Results obtained were excellent in 13 cases, good in 7 cases, fair in 2 cases, poor in 6 cases. No significant side effect due to the drug was observed except one case of thrombocytosis and 2 cases each of elevated GOT and elevated GOT and GPT.

[Bronchial hyperresponsiveness to histamine in guinea pig induced by intravenous administration of platelet activating factor].

Platelet activating factor (PAF) has recently been demonstrated to play an important role in allergic diseases such as bronchial asthma. Changes in airway wall thickness have recently been suggested to cause excess airway narrowing of asthma. In this study, the authors examined the bronchial hyperresponsiveness to histamine in guinea pig induced by intravenous administration of PAF by measuring dynamic compliance and dynamic respiratory resistance. Moreover, a new formula which can exclude the effect of the changes of the airway wall thickness was proposed. With this formula, the administration of PAF was suggested to induce airway wall to be thickened. The ratio of bronchial smooth muscle constriction by histamine was significantly enhanced by the administration of PAF (p less than 0.05). Moreover, antagonists such as CV3988 and CV6209 inhibited the effect of PAF. The above results suggest that PAF may be an important mediator affecting bronchial hyperresponsiveness.
