We have investigated the role of interleukin-1 in the regulatory mechanisms of a bone remodeling system. Osteoblastic cell line (MC3T3-E1) established from newborn mouse calvaria spontaneously produced interleukin-1-like cytokine. Although the interleukin-1-like activity was not observed in culture supernatants of the cells during their exponential phase of growth, a most remarkable interleukin-1-like activity was detected in the supernatants of cells cultured in serum-free alpha-MEM on day 5 after the cells had formed a confluent monolayer. Gel filtration analysis indicated that the interleukin-1-like cytokine exhibits some heterogeneity in size.
The results of anorectal manometry in 11 children with complete rectal prolapse were evaluated. Abnormalities of sphincter control or bowel movements occurred in 8 out of these 11 patients. Anal canal pressure of the patients was lower than in the controls, but there was no significant difference of resting pressure profiles of the anorectum between the patients and the controls. Rectoanal reflex was present in all the patients and in the controls. Rectal compliance was significantly lower in the patients, but there was no correlation between rectal compliance and sphincter control of the patients.
Free autologous smooth muscle transplants were performed in 59 Wistar rats from 70 to 90 days old. The free smooth-muscle transplants were prestretched by 0 percent, 100 percent, 120 percent, 150 percent, and 200 percent under resting conditions, and the animals were sacrificed on postoperative days five, 10, 20, 40, and 60, respectively. Five animals died postoperatively. Three of them died with ileus and stenosis after 150 percent prestretching of the transplant. Complications occurred in 11 other animals, and involved ileus in nine due to stenosis or increasing defecation difficulty resulting from post-150-percent-transplant-prestretching-obstruction phenomena. No relaxation reflex was observed manometrically in any of the transplants. Histology confirmed that smooth-muscle necrosis and connective-tissue multiplication increase with increasing prestretching of the transplant. With prestretching of 150 percent or more, the transplant underwent complete fibrosis. Subsequently anorectal smooth-muscle pedunculated flaps (tunnel grafts) were performed on 20 goats of Syrian and German pedigree. Manometric, histologic, and histochemical results were much better in smooth-muscle transplantation of this nature. Even in these animals, however, increased fibrosis and disintegration of the transplant was shown histologically in all animals with more than 150 percent graft prestretch. After 200 percent prestretch, additional myenteric plexus disintegration was observed and no relaxation could be achieved. Based on these clinical, manometric, and histologic results, smooth-muscle transposition (tunnel graft) can be recommended to improve continence in infants as a non-time-consuming modification of the well-established Rehbein-Romualdi-Kiesewetter pull-through procedure in the high imperforate anus. However, prestretching of the transplanted pedicle flap should not exceed 120 to 140 percent.
The effects of the lysosomal fraction isolated from polymorphonuclear leukocytes (PLF) on the growth of cultivated aortic medial smooth muscle cells (SMCs) and arterial endothelial cells (ECs) were studied by assaying DNA synthesis and counting the numbers of cells. PLF proved to promote the growth of cultivated SMCs and ECs. There was a positive correlation between an increase in DNA synthesis and the dose of PLF. The growth-promoting effect was observed in sparsely cultivated SMCs and ECs, in densely cultivated SMCs, but not in confluently cultivated ECs. The difference in response between SMCs and ECs seems to depend on their biological characteristics. Because a small amount of PLF showed potent growth-promoting activity in the presence of 10% fetal bovine serum which possesses a high protease blocking activity, the mechanism of this promoting activity is suggested to be independent of the proteases contained in PLF.
Hot phenol-water-extracted lipopolysaccharide (LPS) from Bacteroides gingivalis 381 was purified by Sephadex G-100 chromatography with Tris buffer supplemented with sodium deoxycholate and EDTA (B-LPS). In the present study, B-LPS was examined for its ability to induce interleukin 1 (IL-1) production, a mitogenic response, and macrophage activation in LPS high-responder C3H/HeN and low-responder C3H/HeJ mice. A significant increase in IL-1 production was observed in C3H/HeN and C3H/HeJ peritoneal macrophages treated with various doses (1.0 to 50 micrograms/ml) of B-LPS. IL-1 production by C3H/HeN macrophages treated with B-LPS (10 micrograms/ml) was about seven times greater than that by C3H/HeJ macrophages. However, the IL-1 production induced by B-LPS (10 micrograms/ml) in C3H/HeN macrophages was four times lower compared with that induced by Escherichia coli O111 B4 LPS. Also, a significant increase in IL-1 production was found in human monocytes stimulated with B-LPS. That B-LPS-induced IL-1 exhibits some molecular weight heterogeneity was indicated from Sephadex G-75 gel filtration profiles. A significant, high mitogenic response by whole spleen cells with 1 X 10(5) to 5 X 10(4) cells of either mouse strain per well treated with B-LPS (10 to 50 micrograms/ml) was observed. However, the response of C3H/HeJ mice was less than that of the C3H/HeN strain. Also, glucose consumption assays indicated that enhanced macrophage activation occurred in C3H/HeN but not in C3H/HeJ mice treated with B-LPS. In light of recent studies showing that IL-1 stimulates bone resorption in a mouse calvaria system and collagenase production in fibroblasts, we suggest that B-LPS-induced IL-1 may play a significant role in the pathogenesis of adult periodontal disease.
Specific 5'-nucleotidase (5'-N) activity in 17 human fetuses was systemically studied in order to get basic values of the enzyme activity. A wide spectrum of 5'-N activity among the organs was revealed. Very strong activity was found in placenta (42.62 +/- 19.24 nmol adenosine/mg protein/min), skeletal muscle (31.80 +/- 11.10) and skin (27.31 +/- 13.00); strong activity in liver (18.96 +/- 7.60), pituitary (15.06 +/- 6.13) and thyroid (12.69 +/- 4.47); moderate activity in lung (8.43 +/- 3.68), pancreas (8.20 +/- 2.26), small intestine (8.07 +/- 2.28), lymph node (7.26 +/- 2.22), thymus (7.06 +/- 2.52), spinal cord (5.86 +/- 1.96) and spleen (5.65 +/- 2.52), and weak activity in testis (4.02 +/- 0.79), heart (3.95 +/- 1.22), adrenal (3.46 +/- 1.68), kidney (3.16 +/- 0.83), ovary (3.16 +/- 0.72), aorta (2.98 +/- 1.20), cerebellar hemisphere (2.28 +/- 1.09) and frontal lobe of the cerebrum (1.49 +/- 0.54). These data are the first to be reported on specific 5'-N activity in the human fetus. The significance of the wide range of 5'-N activity among the organs, and future aspects on the study of 5'-N activity with respect to cell differentiation, maturation, development, aging or malignant transformation were discussed.
Hepatocellular carcinoma (HCC) showing marked elevation of serum alpha fetoprotein (AFP) (maximum; 70942.0 ng/ml at the end stage) and serum carcinoembryonic antigen (CEA)(maximum; 7368.4 ng/ml at the end stage) was surgically resected. In the resected liver, there were two different tumor nodules which were adjacent to each other but clearly separated by a thin connective tissue. One of the nodules was a well differentiated and the other was poorly differentiated HCC. Immunoperoxidase study revealed that both CEA and AFP were localized in the tumor cells of the poorly differentiated HCC. This is the first report which clearly proved CEA synthesis in the cells of HCC. Serial staining showed that there was simultaneous synthesis of CEA and AFP in some of the tumor cells.
Anorectal manometry was carried out in 65 children with idiopathic chronic constipation. Forty-five had no megarectum, and 20 had megarectum. Patients without megarectum showed normal anorectal pressures, recto-anal reflex, rectal sensation and rectal compliance, but motility of the colon was disturbed in these patients. Patients with megarectum also had normal anorectal pressures, recto-anal reflex and rectal sensation, but they showed remarkably high rectal compliance, or excessive rectal reservoir. Patients without megarectum were treated with laxatives and all of them were cured within 6 months after institution of therapy. Patients with megarectum were treated with rectal washout and suppository and 14 out of 20 were cured within 10 months. Six out of 20 patients with megarectum, however, were resistant to conservative therapy and necessity of surgical treatment to reduce the excessive rectal reservoir was proposed for these patients.
Clinical assessment of fecal continence and anorectal manometry were carried out in 22 patients with Hirschsprung's disease who had undergone "Rectoplasty with posterior triangular colonic flap". Satisfactory postoperative continence was achieved in 18 out of these 22 patients. The result of anorectal manometry disclosed that normal rectal compliance (reservoir function of the rectum), together with normal resting pressure of the anorectum and rectoanal reflex, is indispensable for an adequate postoperative continence.
Free autologous muscle transplants were performed on 59 Wistar rats of 70 to 90 days of age according to the method described by Schmidt (1978). The free smooth muscular transplant was prestretched by 0%, 100%, 120%, 150% and 200% and the animals were sacrificed on the 5th, 10th, 20th, 40th and 60th postoperative day, respectively. Five animals died postoperatively, of these three during ileus and stenosis after more than 150% prestretching of the transplant. Complications occurred in eleven further animals, which included again ileus phenomena nine times due to stenosing or disturbance of defecation, also as a result of obstruction phenomena. Complications always occurred in animals with more than 150% transplant prestretching. No relaxation reflex was observed manometrically in any of the transplants. Budding of nerve fibres also did not occur in any of the cases, neither from the plexus myentericus of the inner intestinal wall nor from outside. Histology revealed already after five days' survival leucocytous infiltration and connective tissue multiplication which increased with increasing prestretching of the transplant and longer survival time of the animals until complete metaplasia of the connective tissue. In preparations with 100 up to 120% prestretch not more than 20% of the original muscular mass was preserved. With prestretching of 150% and more the transplant underwent complete fibrosis. On the basis of these studies we must discourage performance of free transplants of smooth-muscle colon segments in newborn and young infants.
Anorectal smooth muscles pedicle flaps (funnel grafts) according to the dragging method described by Holschneider in 1980, 1981 and Hofmann-von Kap-herr and Koltai 1981 were performed on 20 Syrian goats and German pedigree goats. Postoperatively, three animals died because of pulmonary infection with worms; 17 animals could be examined. Six of these 17 animals died during the postoperative follow-up period from the sequels of the operation. In five animals the cause of death was prestretching of the transplant by 150 and 200%; three of these developed severe enterocolitis, whereas two had stenosis and ileus, resulting in death in each case. In addition, five further animals showed signs of enterocolitis; eight goats had constipation which in every case could be ascribed to prestretching of the transplant by 150% and more. These animals had to be treated by infusion and bouginage, leading to recession of the signs after four to six days. Electromanometrically an increased anorectal resting pressure profile was found postoperatively in all cases, i.e. even if the transplant was prestretched to tension values of only over 100%. Relaxations of the internal anal sphincter could be achieved in all animals; however, in goats with a transplant stretch of 150% and more they were observed only after the resting pressure profile had receded, a few months after the operation. Histologically, there was increasing fibrosis and disintegration of the transplant in all animals with more than 150% prestretch. With 200% prestretch, the plexus myentericus disintegrated also, so that no relaxation could be achieved. Basing on these clinical, manometrical and histological results, smooth muscle transposition (funnel graft) can be recommended to improve continence in infants by employing the dragging method according to Rehbein-Romualdi. However, prestretching of the transplanted pedicle flap should not exceed 120-140%.

Human astrocytomas contain 5'-nucleotidase as shown using histochemical, cytochemical and biochemical techniques. 5'-Nucleotidase activity was revealed by electron microscopy to be associated with the outer surface of the plasma membrane of astrocytoma cells. The results confirm and extend previous reports of the localisation of 5'-nucleotidase in normal astrocytes in situ. Biochemical assay of 5'-nucleotidase activity in 35 tumours showed it to be clearly higher in malignant (anaplastic astrocytomas and glioblastomas) than in benign astrocytomas.
In an attempt to obtain information regarding vascular elastase in arterial hypertension, we examined biochemical changes in elastase-like enzyme activity, and the intravascular localization of elastase by immunohistochemical techniques in the aorta of spontaneously hypertensive rats (SHR). In the biochemical study, aortic elastase-like enzyme activity was significantly higher in SHR than in controls. Using an antibody against rat pancreatic elastase raised in the rabbit, it was demonstrated immunohistochemically that the enzyme was localized in the endothelial cells and subendothelial spaces in the aorta of control animals. In SHR, elastase was also demonstrated in medial smooth muscle cells and particularly in the modified smooth muscle cells in areas of intimal thickening. Some vacuoles in the smooth muscle cells also showed positive enzyme staining. Elastase seems to play an important role in the development of hypertensive vascular changes.
To obtain information about changes in lysosomal enzyme activities in the aortic endothelial cells in arterial hypertension, semi-quantitative histochemical investigations of acid phosphatase (Ac-Pase) and N-acetyl-beta-glucosaminidase (NAGase) activities in the aorta of rats with renal hypertension were performed on "Häutchen" monolayer preparations. The aortic endothelial cells in renal hypertensive animals showed increased Ac-Pase and NAGase activities compared with those in control normotensive rats and tended to increase with advancing age. These results, like our previous data from spontaneously hypertensive rats (SHR), indicated that degeneration of endothelial cells, expressed by increased lysosomal enzyme activity, was accelerated by hypertension, and the possible participation of genetic factors in the activation of these enzymes in SHR was ruled out. Increased lysosomal enzyme activity may be involved in the development of other hypertensive vascular changes.
In order to obtain information about changes in lysosomal enzyme activities in the aortic endothelial cells under hypertensive conditions, semiquantitative histochemical investigations of acid phosphatase (Ac-Pase) and N-acetyl-beta-glucosaminidase (NAGase) activities in the aorta of spontaneously hypertensive rats (SHR) were performed on 'Häutchen' monolayer preparations. The aortic endothelial cells in SHR showed increased Ac-Pase and NAGase activities as compared with those in control normotensive rats, and the activities tended to increase with advancing age. The degenerating process of endothelial cells expressed by lysosomal enzyme activity seems to be accelerated by hypertension. The increased lysosomal enzymes may participate in the further development of hypertensive vascular changes.
