Clinical reevaluation of the effect of oxybutynin chloride on uninhibited neurogenic and reflex neurogenic bladder.

Oxybutynin chloride has been clinically used for the relief of symptoms associated with voiding in patients with uninhibited neurogenic and reflex neurogenic bladder in the USA. The present clinical and urodynamic studies were the first meticulously in patients with uninhibited neurogenic and reflex neurogenic bladder in Japan. A single oral dose of oxybutynin chloride (3 or 6 mg) did not induce any change either in subjective symptoms or in urodynamic studies; however, in the continual administration study, 7 of 9 patients showed an improvement in the symptoms associated with voiding. The drug reduced urinary frequency, urgency and incontinence, and significantly increased the volumes at the first desire to void (FDV) and maximum desire to void (MDV) in cystometry. Furthermore, no serious adverse reactions were observed, and hence the drug seems to be of clinical significance.

Priming effect of interferons and interleukin 2 on endogenous production of tumor necrosis factor in mice.

The effects of interferons (IFNs) and interleukin 2 (IL 2) on endogenous production of tumor necrosis factor (TNF) were investigated in mice. Production of serum TNF was triggered by iv injection of OK-432 and tested by in vitro cytotoxicity assay. Injection of recombinant IFN-gamma with OK-432 and tested by in vitro cytotoxicity assay. Injection of recombinant IFN-gamma with OK-432 or of IFN-alpha/beta, recombinant IFN-beta, recombinant IFN-alpha A/D or recombinant IL 2 six hours before OK-432 enhanced TNF production about 10-fold, which indicated priming actions of these compounds in TNF production. These findings suggest that these compounds could also be used as priming agents for endogenous production of TNF in cancer patients.

[A randomized trial with various combinations of cis-platinum (CDDP), mitomycin C (MMC), peplomycin (PEP) and ftorafur (FT) in the treatment of non-small cell lung cancer].

From March 1983 to February 1985, we treated 74 patients (pts) with inoperable non-small cell lung cancer. Twenty-seven pts with squamous cell carcinoma were randomized between regimen PMP (CDDP 60 mg/m2, d 1, MMC 6 mg/m2, d 3, d 10 and PEP 5 mg/m2, d 3-7) and regimen PM (CDDP 60 mg/m2, d 1 and MMC 6 mg/m2, d 3, d 10). Forty-seven pts with adenocarcinoma and large cell carcinoma were randomized between regimen PMF (CDDP 60 mg/m2, d 1, MMC 6 mg/m2, d 3, d 10 and FT 800 mg/body, d 3-21) and regimen PM. The response rates of evaluable cases (EC) were as follows: Squamous cell carcinoma; Regimen PMP 50% (1 CR + 4 PR/10 EC). Regimen PM 40% (4 PR/10 EC). Adenocarcinoma plus large cell carcinoma; Regimen PMF 13.6% (3 PR/22 EC). Regimen PM 11.1% (2 PR/18 EC). The median survival time (MST) was increased from 23 weeks in non-responders to 32 weeks in responders. However, the difference between the survival curves for responders and non-responders was not statistically significant. Of the toxic effects shown in all 74 registered pts, hematological (64.9%), gastrointestinal (60.8%) and renal (31.1%) toxicities were the common complications. We concluded that regimen PMP was more useful than regimen PM for pts with squamous cell carcinoma but that regimen PMF demonstrated no appreciable difference, compared with regimen PM for pts with adenocarcinoma and large cell carcinoma.

Computed tomography and magnetic resonance imaging of intramedullary spinal cord tumors.

Magnetic resonance imaging (MRI) was found useful in the evaluation and management of patients with intramedullary tumors. Both T1 and T2 weighted images are mandatory for depicting intramedullary pathology and sizable change of the spinal cord. Intratumoral cysts and syrinxes were often associated with tumors and shown as a low signal intensity on the T1 weighted image. Computed tomography (CT) with intravenous iodine showed enhancement of tumor nodules, in all mixed gliomas, ependymomas and hemangioblastomas. Enhanced MRI using gadolinium-DTPA is equal or superior to enhanced CT in localizing intramedullary tumors.

[Nuclear DNA content and focal relapse of patients with small-cell carcinoma of the lung].

The relationship between the nuclear DNA content and the focal relapse of 17 patients with small-cell carcinoma completely responsive to treatment was studied. The nuclear DNA content was measured at 550 nm using a microspectrophotometer. The nuclear DNA histogram pattern was classified as either "Type A" (near-diploid) or "Type B" (hyperdiploid). Of the nine patients with type A, only one was found to have focal relapse of the intrathoracic lesion. However, of the eight patients with type B, focal relapse was observed in six (75%). No significant difference was discerned between the metastasis to other organs and the nuclear DNA histogram patterns. The nuclear DNA histogram patterns in small-cell carcinoma of the lung are correlated to the outcome of the patients.

Endogenous production of TNF in mice long after BCG sensitization.

The effect of tuberculin-antigen protein (PPD: purified protein derivatives) on the priming of tumor necrosis factor (TNF) a long time after sensitization with bacillus Calmette-Guérin (BCG) was investigated in mice. Mice were infected intravenously (i.v.) with 4 X 10(7) colony-forming units (CFU) of BCG per mouse, and the TNF activity triggered by 15 micrograms lipopolysaccharide (LPS) was assayed with time using L-929 cells. The activity was maximal 4 weeks after sensitization and decreased to the control level after 10 weeks. The i.v. injection of 10 micrograms of PPD per mouse enhanced the TNF production triggered by LPS in mice even 10-20 weeks after sensitization. This enhancement was unexpectedly quite transient, being maximal 3 h after PPD treatment. The activity was comparable to that in mice 4 weeks after sensitization of BCG (without PPD). The enhancing effect of PPD was observed in the dose range of 0.1-10 micrograms/mouse, and 10 micrograms of PPD significantly enhanced the TNF activity triggered by even 0.6 microgram LPS. These findings indicate that priming of sensitized animals with some antigens followed by triggering with LPS induces a significant endogenous production of TNF even a long time after antigen sensitization. The possible application of these findings for inducing endogenous production of TNF in human patients by use of tuberculin allergy is discussed.
