Colony-stimulating factors in B-cell colony formation in patients with B-cell chronic lymphocytic leukemia.

A new in vitro colony growth assay system method was found to be reliable in its use for evaluating B-cell proliferation in normal subjects and in 9 patients with B-cell chronic lymphocytic leukemia (B-CLL). The method is based on a phytohemagglutinin (PHA)-stimulated monocyte and T-cell-conditioned medium (PHA-MTCM) composed of PHA, silica, normal monocytes and normal T cells. The number of colonies proliferated was significantly greater in 5 patients who had not undergone treatment than in normal subjects (1,417 +/- 660 vs. 661 +/- 119) (p less than 0.002). Normal cultured B-cell colonies were shown to be 71% surface IgM colonies, and 6% cytoplasmic IgA colonies with the appearance of blastic cells. B-CLL colonies, on the other hand, were demonstrated to be monoclonal with the same CLL circulating cells being retained. We also studied the effect of interleukin-2 (IL-2) on B-cell colony growth assay in 4 patients with B-CLL cells. Only 1 patient with M protein responded to IL-2, proliferated and expressed IL-2 receptors. Although 3 patients without M protein did not respond to IL-2, they did respond to the supernatant, and they proliferated but expressed no IL-2 receptors.

Interleukin 1 production by alveolar macrophages is decreased in smokers.

To evaluate the mechanism by which cigarette smoking suppresses pulmonary immune responses, we determined the capacity of alveolar macrophages (AM) to produce interleukin 1 (IL-1 in 32 normal subjects and in 40 patients with pulmonary sarcoidosis. The amount of IL-1 released from LPS-stimulated AM from smokers was significantly decreased compared with that in nonsmokers in both normal and sarcoid groups. The addition of indomethacin to the cultures in 18 normal subjects and in 22 patients with pulmonary sarcoidosis yielded similar results, thus excluding the possibility that this difference resulted from a difference in the amount of cyclooxygenase metabolites released in the culture supernatants. Similar results were obtained by enzyme-linked immunosolvent assay. Because IL-1 is thought to induce the accumulation of T cells at the site of disease and contribute to local cellular and humoral immunity of the lung, our data suggest that the reduced capacity of AM to release IL-1 in smokers affords partial protection against the initiation of immune responses in the lung and the development of granulomatous lung diseases.

Studies on topical antiinflammatory agents. II. Synthesis and vasoconstrictive activity of 21-substituted corticosteroids with sulfur-containing moieties.

As part of the search for new topical antiinflammatory agents, various 21-substituted corticosteroids having sulfur-containing moieties were prepared and tested for vasoconstrictive activity in humans. A structure-activity relationship study revealed that substitution of the 21-hydroxy group with a lower alkyl-thio group enhanced the activity. The activities of the 21-methylthio (3Ad) and the 21-ethylthio (3Ae) compounds were more potent than that of 9 alpha-fluoro-11 beta,21-dihydroxy-16 beta-methyl-17 alpha-valeroyloxy-1,4- pregnadiene-3,20-dione (betamethasone 17-valerate, BV).

Studies on topical antiinflammatory agents. III. Synthesis of 17 alpha-acyloxy-9 alpha-fluoro-11 beta-hydroxy-16 beta-methyl-1,4-pregnadiene-3,20-dione 21-thio derivatives and related compounds.

A series of 21-thio derivatives of 9 alpha-fluoro-11 beta,17 alpha-dihydroxy-16 beta-methyl-1,4-pregnadiene-3, 20-dione 17-esters and related compounds were synthesized and evaluated as topical antiinflammatory agents. These compounds were prepared by the reaction of 9 alpha-fluoro-11 beta,17 alpha,21-trihydroxy-16 beta-methyl-1,4-pregnadiene-3, 20-dione (betamethasone, I) 17-ester derivatives and various mercapto compounds. A structure-activity relationship study revealed that the structural combination of a thio group at the 21-position and an ester group at the 17-position contributed to vasoconstrictive activity. Among these compounds, the 21-methylthio 17-propanoate compound (6) was found to have the most potent activity, being more potent than betamethasone 17-valerate (BV).

CD8+ cell-dominant alveolitis in pulmonary sarcoidosis.

A 55-year-old woman had bilateral hilar lymphadenopathy and retinouveitis. Microscopic examination of a biopsy specimen from a mediastinal lymph node revealed noncaseating epithelioid cell granulomas. Since granulomatous diseases of infectious origin and sarcoid reaction were reasonably excluded, she was diagnosed as having pulmonary sarcoidosis. Bronchoalveolar lavage findings were atypical in that the ratio of helper T cells to suppressor T cells (CD4/8 ratio) was 0.5 in two separate examinations, despite a moderately increased proportion of lymphocytes. Immunohistochemical examination of the lymph node also showed a predominancy of suppressor T cells. The inversion of the CD4/8 ratio suggests the presence of heterogeneity in the immunoregulatory mechanism in pulmonary sarcoidosis and may have a prognostic significance.

Tumor-infiltrating lymphocytes and histocompatibility antigens in primary intracranial germinomas.

Subpopulations of tumor-infiltrating lymphocytes (TIL's) and the major histocompatibility complex (MHC) antigens of neoplastic cells were examined in three intracranial germinomas by an immunohistochemical method using monoclonal antibodies. About 70% to 80% of TIL's were T lymphocytes which were either infiltrating diffusely or in clusters, whereas 20% to 30% of TIL's were B lymphocytes which tend to cluster in tumor tissues. Examination of T lymphocyte phenotypes revealed both the cytotoxic/suppressor and helper/inducer T lymphocytes, as in other tumors. However, the existence of a considerable number of B lymphocytes in the TIL population was uncommon and seemed to be a characteristic feature of the intracranial germinoma, which might suggest a difference of host immune response to this neoplasm as compared to other tumors. On examination of the MHC antigens, no MHC class I or II antigens in the neoplastic cells were stained, while positive staining for both antigens was seen in the TIL and stroma tissues. From these findings, it was suggested that the degree of TIL infiltration might not be correlated with the expression of MHC antigens in neoplastic cells in cases of primary intracranial germinoma.

Surgical treatment of advanced carcinoma of the esophagus.

This report was done to evaluate the role of surgical treatment of locally advanced carcinoma of the esophagus. All 32 patients had histologically proved epidermoid carcinomas of the esophagus. The post-operative one and two year survival rates and median survival time were 34.6 and 17.3 per cent and 199 days for those in the group undergoing esophagectomy, zero per cent and 55 days for those in the group with bypass and zero per cent and 80 days for those undergoing formation of a stoma. Thirty day postoperative mortality rates were zero per cent for the esophagectomy group, 25.0 per cent for the stoma group and 33.3 per cent for the bypass group. When calculated from the date of diagnosis, the one and two year survival rates and the median survival time were 50.2 and 16.7 per cent and 296 days for the esophagectomy group, 16.7 and zero per cent and 196 days for the bypass group, 25.0 and zero per cent and 142 days for the stoma group and zero per cent and 142 days for the group who did not undergo surgical treatment. Complication related postoperative deaths occurred in 14.8 per cent of the 27 patients operated upon. For those in the esophagectomy group, it was 5.9 per cent, and for the bypass group, 50.0 per cent. Swallowing was improved in 58.8 per cent after esophagectomy, but in none of the patients was swallowing restored after the bypass procedures. When the invasion spared the aorta or major respiratory tract, postoperative survival time after esophagectomy was significantly better. Considering the short postoperative survival time and frequent complications, bypass procedure should not be the final resort after nonsurgical treatment.

[Effects of interleukin-2 and alpha and gamma-interferon on B cell colony formation in patients with hairy cell leukemia].

We studied the effect of interleukin-2, and alpha and gamma-interferon on the growth of B cell colonies in three patients with hairy cell leukemia (HCL). Cells in the colonies from HCL were morphologically similar to the circulating cells, but HCL colonies were morphologically different from B-CLL colonies. HCL colonies had several satellite cells. Spell out (PHA) was essential for the growth of HCL. HCL responded to PHA-MTCM, the supernatant derived from 24 hr incubation of PHA, silica, monocytes and T cells. HCL cells didn't respond to IL-2 or alpha-interferon. However, gamma-interferon significantly enhanced HCL cell proliferation.

[A case of pulmonary sarcoidosis with CD8 positive cell dominant alveolitis].

A 55-year-old woman was admitted for examination of bilateral hilar lymphadenopathy. Serum angiotensin converting enzyme was not elevated, but gallium-scan revealed an increased uptake in both lung hilus. The percentage of lymphocytes in the bronchoalveolar lavage fluid (BALF) was elevated (28.9%), and the proportion of OKIa1 positive cells was also elevated (50.5%). However, the ratio of cluster differentiation (CD) 4/8 was inverted at 0.5. Microscopic examination of the mediastinal lymph nodes revealed epithelioid cell granulomas without ceasation, and a diagnosis of sarcoidosis was made. Immunohistochemical examination of the lymph nodes showed predominancy of CD8 positive cells. Patients with pulmonary sarcoidosis having a low CD4/8 ratio in BALF are extremely rare in the early stage of sarcoidosis. The inverted CD4/8 ratio was of interest in regard to the pathogenesis of sarcoidosis.

[Measurement of coronary flow velocity with Doppler catheter: evaluation of coronary flow reserve in successful angioplasty].

To assess the therapeutic effect of percutaneous transluminal coronary angioplasty (PTCA) on coronary flow reserve, coronary flow velocity (CFV) was measured with a Doppler catheter before and immediately after PTCA in 11 patients, who underwent elective PTCA for critical stenosis in proximal or mid portion of the left anterior descending artery (LAD). A Doppler catheter was positioned at the proximal portion of the LAD and the CFV was measured at rest and after intracoronary injection of 6 ml of contrast material (Iopamidol), 6 ml of saline or 3 mg of Isosorbide Dinitrate (ISDN). Peak to resting velocity ratio (PRVR) was calculated as an estimate of coronary flow reserve. Percent diameter stenosis (%S) was measured from cineangiogram. A translesional pressure gradient was obtained with an angioplasty catheter. These parameters measured in PTCA candidates were compared with those in 11 patients whose LAD had no critical stenosis. After PTCA, %S was decreased (94.2 +/- 1.4 vs 34.1 +/- 5.1%; mean +/- SEM). Pressure gradient was also decreased (59.5 +/- 4.9 vs. 25.1 +/- 3.3 mmHg). There was no difference between mean CFV at rest in patients before PTCA and that in patients without stenosis (4.52 +/- 0.63 vs. 5.46 +/- 0.61 cm/sec). By successful PTCA, CFV at rest was increased (7.39 +/- 1.32, p less than 0.05 vs. before PTCA). PRVRs in patients before PTCA were smaller than those in patients without stenosis (1.5 +/- 0.1, 1.4 +/- 0.1, 1.6 +/- 0.2 vs. 2.8 +/- 0.1, 2.5 +/- 0.2, 2.8 +/- 0.2, p less than 0.01; by contrast material, saline, ISDN, respectively).(ABSTRACT TRUNCATED AT 250 WORDS)

Studies on T-cell colony formation in chronic renal failure (CRF) patients.

In order to study the possibility of abnormal differentiation and proliferation of T-cell precursors in chronic renal failure (CRF), we studied T-cell colony formation in CRF patients. The two-step monolayer method, with phytohemagglutinin-P as the inducer, was used for T-cell colony formation. In our results, colony formation was markedly reduced in CRF patients in comparison with normal controls, with about half of the former showing no colony growth. All cases showed a significant increase in colony numbers with in vitro plasmapheresis (the replacement of autologous plasma in the culture system with normal AB plasma). A significant increase in colony numbers was also seen with the addition of exogenous interleukin-2 (IL-2). The addition of IL-2 in the presence of normal plasma, in particular, induced an increase in colony numbers to near the levels in normal subjects. These results suggest that T-cell precursors exist in near normal numbers in CRF patients and that there are uremic inhibitors in the plasma. A reduced production of IL-2 is also indicated. These factors may be involved in the pathogenesis of immunodeficiency in CRF patients.

[A case of pulmonary sarcoidosis with calcification in the lung].

A 35 year-old male farmer presented with complaints of productive cough and sputum. The chest X-ray films showed reticulonodular shadows bilaterally in the upper and middle lung fields, segmental infiltration in the right lung, and no BHL. Tuberculin reaction was negative. Serum angiotensin converting enzyme level was 26.5 IU/ml. Precipitating antibodies for Thermophilic actinomycetes and M.f. were negative. BAL showed moderate lymphocytosis (24.3%), and CD 4/8 was 1.67. A biopsy specimen of right scalene lymph node showed epithelioid cell granulomas and TBLB epithelioid cell granulomas and spot-like calcification. Pulmonary sarcoidosis with calcification in the lung is very rare.

[Left ventricular function in right ventricular overload: asymmetry of the left ventricular ejection].

This study clarified regional and global functions of the distorted left ventricle due to right ventricular overload by means of gated radionuclide ventriculography (RNV). Cardiac catheterization and RNV were performed in 13 cases of atrial septal defect (ASD), 13 of pure mitral stenosis (MS), 10 of primary pulmonary hypertension (PPH), and 10 of normal subjects (NL). Right ventricular systolic pressure (RVSP) was 32.9 +/- 13.9, 45.0 +/- 12.2, 88.3 +/- 17.1, and 21.2 +/- 4.5 mmHg, respectively. RNV was performed with a 99mTc-red blood cell in a vivo labeling technique. The end-systolic LAO view of the left ventricle was halved into septal and free-wall sides. The end-diastolic halves were determined in the same plane. Ejection fractions of the global left ventricle (LVEF), global right ventricle (RVEF), the septal half of the left ventricle (SEPEF), and the free-wall half of the left ventricle (FWEF) were obtained. LVEF was 56.8 +/- 9.8% in NL, 52.8 +/- 10.5% in ASD, and 49.5 +/- 12.9% in PPH. In MS, LVEF (47.0 +/- 13.0%) was smaller than those in the other groups. RVEF was 37.0 +/- 5.2% in NL, 43.7 +/- 15.5% in ASD, and 32.8 +/- 11.5% in MS. In PPH, RVEF (25.0 +/- 10.6%) was smaller than those in the other groups. SEPEF was smaller in AS D (42.5 +/- 13.2%), MS (40.4 +/- 13.1%), PPH (40.5 +/- 12.5%) than in NL (53.5 +/- 8.5%). Systolic function of the septal half of the left ventricle was disturbed by right ventricular overload. RVEF (r = -0.35, p less than 0.05) and SEPEF (r = -0.51, p less than 0.01) had negative correlations with RVSP. As RVSP rose, systolic function of the septal half of the left ventricle was more severely disturbed. FWEF was the same among the four groups; NL (57.0 +/- 12.6%), ASD (48.6 +/- 15.2%), MS (50.5 +/- 12.0%), and PPH (51.1 +/- 12.3%). Right ventricular overload does not affect systolic function of FWLV. There was a good correlation between SEPEF and LVEF in NL (r = 0.81), though in PPH this correlation was poor (r = 0.64). In patients with PPH the septal side of the left ventricle does not act as a part of the global left ventricle. Systolic function of the septal side of the left ventricle is disturbed due to the distortion of the ventricular septum, but systolic function of the free-wall side is maintained within a normal range, when the left ventricular myocardium is kept normal.(ABSTRACT TRUNCATED AT 400 WORDS)
