[Clinical usefulness of 67Ga scintigraphy in pulmonary sarcoidosis].

The relationship between the intensity of gallium-scintiscans and pathologic findings on transbronchial lung biopsy were studied in 49 patients with untreated pulmonary sarcoidosis. There was a strong correlation between the intensity of gallium uptake in pulmonary parenchyma and the detection rate of granuloma. Most of the pulmonary gallium accumulation was found in activated alveolar macrophages. These observations indicate that the intensity of the gallium uptake reflects mainly the degree of granuloma formation in the lung, and gallium scintigraphy may be an indicator for predicting the degree of activity of pulmonary sarcoidosis.

[A case of pulmonary sarcoidosis which exacerbated bronchial asthma in remission].

A 34-year-old woman had history of bronchial asthma in childhood, was admitted complaining of coughs and wheezes. Chest X-ray examinations showed bilateral hilar lymphadenopathy. A diagnosis of bronchial asthma was made because of her past history, reversible airflow limitation and moderate bronchial hyperreactivity. In addition, findings of chest X-ray films and TBLB specimens were indicative of pulmonary sarcoidosis. When bilateral hilar lymphadenopathy disappeared, her wheezes and bronchial hyperreactivity also improved. In this case, it was suggested that pulmonary sarcoidosis was a factor which worsened bronchial hyperreactivity and bronchial asthma which had been in remission.

[Effect of aminophylline on thallium-201 washout rate during myocardial perfusion imaging with dipyridamole].

Thallium-201 myocardial perfusion imaging with dipyridamole was performed on 78 patients (pts) with suspected coronary artery disease (CAD). Twenty eight pts had normal coronary arteries (control group), and 50 pts had 50% or greater stenosis in one or more major coronary arteries (CAD group). In control group, aminophylline (Am) was infused intravenously on 12 pts during dipyridamole testing. Mean washout rate (WR) of these 12 pts was greater than that of 16 pts without Am (42% vs 35%, p less than 0.01). Then normal limit of WR was defined separately from the pts of control group with and without Am. The sensitivity for detecting the pts with CAD by WR analysis which normal limit was calculated separately with and without Am was 92% (46/50). When normal limit of WR was determined from all pts in control group, the sensitivity decreased to 84% (42/50). Am significantly affected thallium-201 washout during dipyridamole testing. WR of CAD pts should be estimated by specific set of normal limit with and without Am.

Lymph node metastasis in resectable esophageal cancer.

The prognostic significance of lymph node metastasis was analyzed in 41 patients with locally resectable epidermoid carcinoma of the esophagus. The 5-year survival rate was 50.9% if no metastases or only single node metastasis was present and 29.8% when the lymph node metastasis was confined to one anatomic compartment. All patients with metastasis to two nodes died within 4 years of operation, and all with involvement of three nodes or more died within 3 years. Even with no metastases or single node metastasis, three of 22 patients (13.6%) died of recurrence or metastasis or a combination of the two. Although nodal dissection may be beneficial for selected patients, our results indicate that the survival period is short in the presence of a small number of lymph node metastases, even if the local disease is resectable and despite nodal dissection. Cure is unlikely when cervical or abdominal nodes are involved, and a conservative approach may be indicated for such patients.

Structure of phosphonoglycosphingolipid containing pyruvylated galactose in nerve fibers of Aplysia kurodai.

A phosphonoglycosphingolipid, designated as FGL-IIb, was identified in nerve fibers of Aplysia kurodai by two-dimensional thin layer chromatography (Abe, S., Araki, S., and Satake, M. (1986) Biomed. Res. (Tokyo) 7, 47-51). FGL-IIb was isolated from the nervous system of A. kurodai by Iatrobeads column chromatography using three solvent systems. Pyruvic acid was identified by thin layer chromatography as its 2,4-dinitrophenylhydrazone and established by permethylation studies to be attached as a ketal to O-3 and O-4 of the terminal galactose of the oligosaccharide chain in FGL-IIb. By sugar analysis, permethylation studies, fast atom bombardment-mass spectrometry, and proton magnetic resonance spectrometry, the structure of FGL-IIb was concluded to be [3,4-O-(1-carboxyethylidene)]Gal beta 1----3GalNAc alpha 1----3(Fuc alpha 1----2) (2-aminoethylphosphonyl----6)Gal beta 1----4Glc beta 1----1ceramide. Its major aliphatic components were palmitic acid, octadeca-4-sphingenine and anteisononadeca-4-sphingenine. This is the first report of the occurrence of pyruvylated galactose as a constituent of animal sphingolipid.

Spontaneous chromosome fragility in band 3q21, 11p11, or 11q13 of cultured bone marrow cells from two patients with hematologic disorders.

Chromatid gaps and breaks clustering to band 3q21, 11p11, or 11q13 were observed prior to chemotherapy in short-term cultured bone marrow cells from two patients with hematologic disorders, one with acute monoblastic leukemia having +8 as the sole karyotypic abnormality and the other with pernicious anemia having no chromosome abnormality. The mitogen-stimulated peripheral blood lymphocytes of both patients, however, yielded a negligible frequency of chromosome aberrations. Because of no notable history of clastogen exposure in these patients, the observed chromosome fragility is most probably spontaneous, which might be correlated with the patients' physiologic condition at examination, i.e., an unusually low level of folic acid or vitamin B12, both being involved in DNA synthesis. Although band 11q13 is known to contain a common fragile site, chromosome fragility in bands 3q21 and 11p11 has not yet been reported in either normal or neoplastic cells. The present findings appear to favor the in vivo expression of chromosome fragility.

Endothelin-induced Ca-independent contraction of the porcine coronary artery.

Front surface fluorometry and porcine coronary arterial strips loaded with fura-2 were used to investigate the effect of endothelin on cytosolic Ca concentrations, [Ca]i, and on contractile force, the objective being to elucidate the mechanism of action. Both in the presence and absence of extracellular Ca, endothelin induced rapid and dose-dependent increases in [Ca]i and in contraction. When caffeine-sensitive and histamine-sensitive intracellular Ca stores were depleted, in Ca-free medium, a transient contraction but no increase in [Ca]i followed the subsequent application of endothelin. This Ca-independent component was largely inhibited by the relative protein kinase C inhibitor, H-7, but not inhibited by W-7, calmodulin antagonist. This component is probably linked to activation of protein kinase C.

Glomeruloid blood vessels in ethylnitrosourea-induced rat gliomas. Histological and immunohistochemical studies.

Glomeruloid blood vessels (GBVs), a characteristic histological feature of most human malignant gliomas, were recognized with high incidence in autochthonous rat gliomas induced by transplacental administration of ethylnitrosourea. To evaluate some of the biological properties of these GBVs, we carried out a study using histological methods and immunohistochemical staining for glial fibrillary acidic protein, factor VIII-related antigen (VIII Ag) and bromodeoxyuridine (BrdUrd). Of 22 animals with large, massively growing gliomas in the CNS, GBVs including conglomerate aggregations of small blood vessels with endothelial hyperplasia and strong VIII Ag expression were observed in 13 large gliomas histologically consisting of primitive neuroepithelial neoplasms (PNN; so called ependymoma) and mixed-type gliomas in combination with astrocytoma and PNN or anaplastic astrocytoma. The anaplastic gliomas in our series were devoid of GBVs. These findings indicate that GBV formation takes place in a histological variety of experimental gliomas. Furthermore, the GBVs were frequently associated with the vasculo-mesenchymal stroma in the parent gliomas, suggesting an intimate relationship with the morphogenesis of GBVs. In addition, it was shown that the GBVs had a higher BrdUrd-labelling index than that of other blood vessels in gliomas and also that of neoplastic cells in most parent gliomas, except for anaplastic gliomas. Based on these results, the possible mechanism of GBV morphogenesis is discussed with regard to the roles of macromolecules in the induction and regulation of GBVs.

Primary malignant lymphoma of the brain: demonstration of immunoglobulin gene rearrangements in four cases by the Southern blot hybridization technique.

Using the Southern blot hybridization technique, four cases of the primary malignant lymphomas of the brain, histologically diffuse large cell lymphoma, were examined for the immunoglobulin gene rearrangements. In three lymphomas, the rearrangements were observed in both heavy and light chain genes, providing strong evidence for a B cell lineage of these tumors. On the other hand, in the remaining lymphoma, the rearrangement was observed only in the heavy chain gene. Despite this, immunohistochemical examination revealed positive stainings for heavy and light chain immunoglobulins in tumor cells, suggesting the occurrence of light chain gene rearrangement at the undetectable level. Thus, B lymphocytic differentiation at the gene level was demonstrated in three, or possibly all, of the primary intracerebral malignant lymphomas examined. Since no more than two rearrangements were detected in each immunoglobulin gene, these lymphomas were considered to be monoclonal in nature.

An immunohistochemical analysis of T cells in primary B cell malignant lymphoma of the brain.

An immunohistochemical study was performed on small lymphoid cells present in frozen tissue sections of seven cases of primary B cell malignant lymphomas of the brain by using monoclonal antibodies to T cell (Leu-1, OKT-11, Leu-3a, and Leu-2a) and B cell (BA-1 and Leu-12) surface markers. In all the seven cases, positive reaction for Leu-1 and OKT-11 was seen in the majority of the small lymphoid cells which were dispersed among the lymphoma cells or clustered around blood vessels. The large neoplastic cells were unstained by these antibodies. Staining for T cell subsets with antibodies to Leu-3a and Leu-2a showed heterogeneous staining in each case. The ratio of Leu-3a+ to Leu-2a+ cells was less than one in six cases, demonstrating a suppressor/cytotoxic phenotype predominance. Most of these small lymphoid cells were negatively stained by antibodies to BA-1 and Leu-12. From these findings, it was shown that the small lymphoid cells observed in primary B cell lymphomas of the brain were of T cell lineage, distinct from the neoplastic cells, and probably reactive in nature. The implications of these findings are discussed.

Relationship of alternans of monophasic action potential and conduction delay inside the ischemic border zone to serious ventricular arrhythmia during acute myocardial ischemia in dogs.

We studied the relationship of conduction delay and monophasic action potential (MAP) at an ischemic border zone to serious ventricular arrhythmia (VA). Measurements were made in 36 adult mongrel dogs 15 minutes after occlusion of the left anterior descending coronary artery. We recorded MAPs and bipolar electrograms simultaneously inside and outside the ischemic border zone in the dog hearts. Of the 36 dogs, 24 that had favorable MAP recordings were chosen and were divided into two groups according to an occurrence of VA. In group A (n = 12), an onset time of VA was 5.1 +/- 0.8 minutes after ischemia, while in group B (n = 12) VA did not occur during ischemia. The conduction delay at the ischemic border zone was significantly greater from 3 minutes after ischemia in group A than in group B. MAP alternans, defined as the difference in duration of two consecutive MAPs inside the ischemic border, was marked in group A but mild in group B. Duration of a small MAP of alternans was markedly shorter than that of a corresponding MAP outside the ischemic border in both groups, but was followed by VA in group A. Duration of a large MAP was markedly prolonged compared with that of a corresponding MAP outside the ischemic border in seven dogs of group A and in only one dog of group B (p less than 0.01), and was followed by VA in seven dogs of group A. Our results suggest that the mechanism of VA is due not only to reentry but also to an injury current flowing from ischemic to nonischemic tissues, because VA was preceded by conduction delay and by a difference in MAP durations when comparing regions inside and outside the ischemic border.

Chromosomal in situ hybridization and Southern blot analyses using c-abl, c-sis, or bcr probe in chronic myelogenous leukemia cells with variant Philadelphia translocations.

The Philadelphia (Ph) chromosome is a cytogenetic hallmark of chronic myelogenous leukemia (CML). Whereas the majority of Ph-positive CML patients show the standard Ph translocation involving chromosomes 9 and 22, t(9;22)(q34;q11), the minority of cases exhibit a variant type of Ph translocation involving these two and other chromosomes (complex type) or those involving #22 and chromosomes other than #9 (simple type). To get an insight into the nature of variant Ph translocations and the process of their formation, we examined the localization of the c-abl and c-sis oncogenes and the breakpoint cluster region (bcr) gene by chromosomal in situ hybridization in ten variant Ph translocations of CML including five simple and five complex ones as initially interpreted. In situ hybridization showed that c-abl localized to band 9q34 and c-sis localized to band 22q12-q13 were translocated on the Ph and on one of the rearranged chromosomes other than #9, respectively, in all the variant translocations examined. On the other hand, bcr localized to band 22q11 was translocated on various chromosomes but mostly on chromosome 9. Parallel Southern blot analyses on DNA from leukemic cells of five patients including two with simple translocations and three with complex ones revealed rearrangements of bcr with breakpoints occurring mostly in a 5' portion of 5.8-kb BamHI/BglII sequences, which are quite similar to those detected so far in CML cases with the standard Ph translocation. The present findings strongly suggest that variant Ph translocations of CML are all complex, and some of them are formed stepwisely from the standard translocation.

Effects of heart rate and diltiazem hydrochloride on alternans of ST segment elevation and ventricular arrhythmia during acute myocardial ischaemia in dogs.

Although the incidence of ventricular arrhythmias following myocardial ischaemia lessens as ischaemia improves, it is not clear whether this is correlated with a reduction in the degree of ST segment elevation. To explore this further we examined the effects of change in heart rate and the administration of the calcium antagonist diltiazem, 0.02 mg.kg-1.min-1, on ST segment elevation and the alternans of ST segment elevation (STA) and on serious ventricular arrhythmia induced by 10 min occlusion of the left anterior descending coronary artery in 86 mongrel dogs. The dogs were divided into three groups: 26 dogs paced at a rate of 180 beats.min-1 (group A); 44 dogs paced at a rate of 120 beats.min-1 (group B); and 16 dogs paced at a rate of 180 beats.min-1 and given diltiazem intravenously from 25 min before the coronary occlusion (group C). The degree of ST segment elevation and STA within 3 min of ischaemia was significantly lower in group B than in group A. There was no marked difference in the degree of ST segment elevation between groups A and C, but the STA was lower in group C than in group A. Incidence of ventricular tachycardia and ventricular fibrillation was significantly lower in groups B and C than in group A, and the timing of their first appearance was 4.5 (SEM 0.6), 4.2(0.9) and 3.0(0.4) min, respectively. We suggest that the reduction in serious ventricular arrhythmias associated with the decrease in heart rate was caused by the improvement of STA secondary to the improvement of ST segment elevation.(ABSTRACT TRUNCATED AT 250 WORDS)

Characterization of phosphonoglycosphingolipids containing pyruvate: localization in Aplysia nerve bundles.

A phosphonoglycosphingolipid, designated as FGL-IIb, was first identified in nerve fibers of Aplysia kurodai by two-dimensional TLC (Abe, S. et al. (1986) Biomed. Res. 7, 47-51), and its chemical structure has been determined to be 3,4-O-(1-carboxyethylidene)]Gal beta 1----3GalNac alpha 1----3(Fuc alpha 1----2)(2-aminoethylphosphonyl----6)Gal beta 1----4Glc beta 1----1ceramide (Araki, S. et al., submitted). Cryostat and paraffin sections of the nervous tissue and skin of Aplysia were examined immunohistochemically with antiserum against FGL-IIb. With this antiserum, only nerve bundles were stained distinctly: nerve cells in ganglia and in subcutaneous and muscular tissues and other cell elements were not stained. From histochemical findings in cryostat sections pretreated with chloroform-methanol (2 : 1, v/v) and from the results of Western blot analysis of the nervous tissue, the staining was concluded to be due to glycolipid antigens. The antiserum reacted with FGL-IIb and other phosphonoglycosphingolipids named FGL-I, FGL-IIa, FGL-V, and F-9 on TLC plates. This reactivity of FGL-IIb was abolished by mild acid-methanol treatment, and the lost reactivity was recovered by alkaline hydrolysis. These findings suggest that the free carboxyl group of the pyruvic acid of FGL-IIb is essential for the immunological reaction and that all the glycolipids listed above have the same epitope as that of FGL-IIb. Immunohistochemical findings indicated that these glycolipids including FGL-IIb are localized specifically in nerve bundles of Aplysia.

Construction of less neurovirulent polioviruses by introducing deletions into the 5' noncoding sequence of the genome.

Viral attenuation may be due to lowered efficiency of certain steps essential for viral multiplication. For the construction of less neurovirulent strains of poliovirus in vitro, we introduced deletions into the 5' noncoding sequence (742 nucleotides long) of the genomes of the Mahoney and Sabin 1 strains of poliovirus type 1 by using infectious cDNA clones of the virus strains. Plaque sizes shown by deletion mutants were used as a marker for rate of viral proliferation. Deletion mutants of both the strains thus constructed lacked a genome region of nucleotide positions 564 to 726. The sizes of plaques displayed by these deletion mutants were smaller than those by the respective parental viruses, although a phenotype referring to reproductive capacity at different temperatures (rct) of viruses was not affected by introduction of the deletion. Monkey neurovirulence tests were performed on the deletion mutants. The results clearly indicated that the deletion mutants had much less neurovirulence than with the corresponding parent viruses. Production of infectious particles and virus-specific protein synthesis in cells infected with the deletion mutants started later than in those infected with the parental viruses. The rate at which cytopathic effect progressed was also slower in cells infected with the mutants. Phenotypic stability of the deletion mutant for small-plaque phenotype and temperature sensitivity was investigated after passaging the mutant at an elevated temperature of 37.5 degrees C. Our data strongly suggested that the less neurovirulent phenotype introduced by the deletion is very stable during passaging of the virus.

Determinants in the 5' noncoding region of poliovirus Sabin 1 RNA that influence the attenuation phenotype.

A number of recombinants between the virulent Mahoney and attenuated Sabin strains of type 1 poliovirus were constructed by using infectious cDNA clones of the two strains. To identify a strong neurovirulence determinant(s) residing in the genome region upstream of nucleotide position 1122, these recombinant viruses were subjected to biological tests, including monkey neurovirulence tests. The results of the monkey neurovirulence tests suggested the important contribution of an adenine residue (Mahoney type) at position 480 to the expression of the neurovirulence phenotype of type 1 poliovirus. This nucleotide, however, had only a minor effect, if any, on viral temperature sensitivity. Monkey neurovirulence tests on the recombinant virus whose genome had a guanine residue (Sabin type) at position 480 and variants generated from this recombinant virus in the central nervous system of monkeys strongly suggested that only one nucleotide change, from adenine to guanine, was not sufficient for full expression of the attenuation phenotype encoded by this genome region. These results suggest that the expression of the attenuation phenotype depends on the highly ordered structure formed in the 5' noncoding sequence and that the formation of such a structure is possibly influenced by the nucleotide at position 480. Furthermore, in vitro biological tests performed on viruses recovered from the central nervous system of monkeys injected with a temperature-sensitive recombinant virus showing the small-plaque and d phenotypes revealed that most of the recovered viruses had even higher temperature sensitivities and that all of the recovered viruses that had acquired the large-plaque phenotype had lost the d phenotype to some extent. These results indicate that there may be an unknown selection pressure(s) in the central nervous system and that common determinants might be involved in the expression of the small-plaque and d phenotypes.

Significance of tubulointerstitial lesions in biopsy specimens of glomerulonephritic patients.

To evaluate the significance of tubulointerstitial lesions in the cortical area of renal biopsy specimens, clinicopathological studies were performed on 101 cases of IgA nephropathy, 31 cases of IgA-negative (non-IgA) proliferative glomerulonephritis and 75 cases of idiopathic membranous glomerulonephritis. The degree of tubulointerstitial lesions was assessed semiquantitatively by light microscopic observation and was correlated with the several histopathological and clinical parameters at biopsy, as well as with status at final follow-up (average follow-up period: 72 months). In these three types of glomerulonephritis, the degree of tubulointerstitial lesions in the cortical area was clearly correlated with the severity of glomerular injury, the prevalence of segmental sclerosis, global sclerosis, arteriolosclerosis, decreased renal function (GFR less than 70 ml/min) and hypertension (greater than 150/90 mm Hg) at the time of biopsy. The prevalence of stable renal function at final follow-up was statistically higher in the cases without tubulointerstitial lesions or with those whose lesions included less than 20% of the cortical area. From the above data, it was concluded that a semiquantitative evaluation of tubulointerstitial lesions in the cortex would reflect the severity of glomerular injury and also contribute to the assessment of prognosis in such primary glomerulonephritic patients.
