[A case of bronchial artery aneurysm demonstrating a mass shadow on chest X-ray film].

A 66-year-old woman was admitted on May 13, 1989, because of an abnormal round shadow in the right hilum on chest X-ray film. Bronchoscopy revealed a stenosis of the right B8 caused by a bulging lesion. An aortogram showed a large meandering bronchial artery and a saccular aneurysm. The aneurysm was about 25 mm in diameter and associated with a bronchopulmonary anastomosis. It was surgically removed by right lower lobectomy. Pathological examination proved that the resected lesion was an arterial aneurysm. Patchy arteriosclerotic change was present in the intima of the aneurysm, but there was no evidence of inflammatory change in the arterial wall and the adjacent alveoli and bronchi. Twenty-six cases of bronchial artery aneurysm have been previously reported in the literature, but this is the first case that appeared as a solitary nodule on conventional chest X-ray.

[Coronary flow characteristics in hypertrophic cardiomyopathy--a study with Doppler catheter].

UNLABELLED: We compared the pattern and reserve of coronary flow in 8 cases of hypertrophic non-obstructive cardiomyopathy (H) with those in 20 cases of chest pain not accompanied by organic heart disease (N). A catheter-tip Doppler velocimeter was positioned in the proximal portion of the left anterior descending (LAD), circumflex (LCX) and right coronary (RCA) arteries. Coronary flow velocity (Vs: systolic peak, Vd: diastolic peak, Vm: mean) was recorded and the area under the velocity curve was divided into systole (* s) and diastole (* d). The time interval between the dicrotic notch in aortic pressure and the peak of diastolic flow velocity was measured (Tpv). Vm was measured before and after intracoronary injection of 6 ml of contrast media, and peak to resting velocity ratio (PRVR) was calculated as an index of coronary flow reserve. RESULT: In LAD, N showed diastolic predominant coronary flow pattern without backward flow. In H, diastolic predominance was more prominent with systolic backward flow, resulting in decrease in * s/* d(H: 0.07 +/- 0.04, N: 0.25 +/- 0.02, p less than 0.01). In H, Vd (H: 20.1 +/- 2.8, N: 9.2 +/- 1.4 cm/sec, p less than 0.05) and Vm(H: 9.5 +/- 1.3, N: 4.9 +/- 0.7 cm/sec, p less than 0.05) were higher, while PRVR was lower (H: 1.7 +/- 0.1, N: 2.6 +/- 0.1, p less than 0.05). In both N and H, the flow pattern of LCX was diastolic predominant with two peaks (one in systole and the other in diastole).(ABSTRACT TRUNCATED AT 250 WORDS)

[Vena caval flow patterns in patients with constrictive pericarditis: analysis by catheter-tip Doppler flowmetry].

Changes in superior and inferior vena caval flow patterns were analyzed in 5 patients with constrictive pericarditis and were compared with those of 10 normal control subjects. Caval flows were measured using catheter-tip Doppler flowmeters. The normal controls showed biphasic M-shaped flow patterns; the peaks of the first forward flow (S wave) and of the second forward flow (D wave) appeared coincident with mid-systole and mid-diastole, respectively. Reverse flows fell during the atrial contraction period (A wave) and late systole (V wave). In the normal controls, the ratios of the S wave to the D wave (S/D ratio) and the A wave to the S wave (A/S ratio) were 2.15 +/- 0.41 and 0.18 +/- 0.10, respectively, and there was a disproportionate respiratory variation in the S and D waves in the normal controls. In constrictive pericarditis, superior and inferior vena caval flow velocities were lower than those in the normal controls. The S/D and A/S ratios were 1.46 +/- 0.27 (p < 0.05 vs control) and 0.66 +/- 0.15 (p < 0.01 vs control), respectively, with the A wave increasing in proportion to the severity of constrictive pericarditis. In addition, there was only a minimal respiratory variation in constrictive pericarditis. In conclusion, recognition of the patterns of the superior and inferior vena caval flow velocities may be useful for diagnosing constrictive pericarditis.

[Real-time coronary blood flow measurement in patients with atrial fibrillation: a study using a catheter-tip Doppler velocimeter].

To clarify the influence of changes in the cardiac cycle length (R-R) and aortic pressure on the coronary blood flow, a catheter-tip Doppler velocimeter was applied for 16 patients with chronic atrial fibrillation (11 with valvular heart disease, 2 with coronary artery disease, 2 with cardiomyopathy and one with atrial septal defect). An area under the coronary flow velocity curve during systole (integral of S), diastole (integral of D) and one cardiac cycle (integral of T) for the proximal portion of the left anterior descending artery (LAD: 12 cases) or the right coronary artery (RCA: 10 cases) was calculated in beat-by-beat. Then, the correlations between each area and the R-R, systolic period (S), diastolic period (D) and aortic pressure were assessed. In both the LAD and RCA, prolongation of R-R associated with prolonged D increased integral of D, which caused an increase of integral of T. Integral of D correlated with D (p < 0.05), but integral of S did not correlate with S, and the degree of change in integral of S or S was much less than that in integral of D or D. R-R or D of the preceding beat correlated inversely (p < 0.05) with integral S in 11 of 12 LAD cases. In the RCA, positive correlations between R-R or D of the preceding beat and integral of S were observed in cases with mitral stenosis (n = 6) or coronary heart disease (n = 1), but not in other cases; a case with aortic regurgitation or hypertrophic cardiomyopathy, negative, and dilated cardiomyopathy, no correlation.(ABSTRACT TRUNCATED AT 250 WORDS)

[Non-invasive evaluation of coronary reperfusion using myoglobin: significance of quantitative and semi-quantitative latex agglutination methods].

We assessed the usefulness of myoglobin measured by latex agglutination methods as a parameter for successful reperfusion of the infarct-related artery. Plasma myoglobin levels were measured for 25 patients with acute myocardial infarction in whom thrombolysis and/or percutaneous transluminal coronary angioplasty (PTCA) were performed within 8 hours after the onset of symptoms. Blood samples were obtained before and 56 +/- 26 min (mean +/- SD) after commencing treatment. Plasma myoglobin levels were measured by quantitative and semi-quantitative latex agglutination methods, which required a procedure time of about 10 min. Coronary reperfusion was achieved by thrombolysis or PTCA in 22 cases (Group 1) but was not achieved by thrombolysis in 7 cases (Group 2). Before treatment (4.2 +/- 1.2 hours after the onset of symptoms), plasma myoglobin levels measured quantitatively and semi-quantitatively were 434 +/- 330 ng/ml and 4.6 +/- 4.2 fold, respectively. After treatment, the increased rate of myoglobin was significantly higher in Group 1 (8.4 +/- 7.5 and 9.2 +/- 7.1) than in Group 2 (1.8 +/- 0.5 and 1.7 +/- 0.5). Plasma myoglobin levels measured by quantitative and semi-quantitative latex agglutination methods showed a close correlation (r = 0.91, p < 0.01). In conclusion, measurement of plasma myoglobin levels by latex agglutination methods is a rapid and reliable way to confirm successful coronary reperfusion.

[Effects of cerebral cortical lesions on the ipsilateral thalamus].

An injury to the central nervous system causes a focal logical disturbance, and further may affect the blood flow, metabolism, and function of other brain regions. Recent studies using PET or SPECT have demonstrated that impairment of regional hemodynamics or metabolism in cerebrovascular disease involves not only the site of the lesion itself but also more remote areas. Although depression of the metabolism of the ipsilateral thalamus in patients with cerebral cortical lesions has been shown by PET study, the pathophysiological implications of this remain unclear. The functional and morphological effects of cortical infarcts on the ipsilateral thalamus were studied by assessment of cerebral blood flow using 123I-IMP SPECT and by determining atrophic changes on CT or MRI. Nine out of 17 patients with cortical infarcts showed hypoperfusion of the ipsilateral thalamus, especially patients with larger infarcts involving the frontal or parietal cortex. Thalamic hypoperfusion persisted from early after the insult to several months or even years later. In addition, atrophy of the ipsilateral thalamus was not uncommon following larger cortical infarcts. This tended to be evident about 1 year after the infarct and progressed over several years. Furthermore, atrophic changes in the thalamus was often demonstrated in such patients as hypoperfusion in the later stages. Thus, cortical lesions had functional and morphological effects on the ipsilateral thalamus ranging from early hypoperfusion to later irreversible atrophic changes.(ABSTRACT TRUNCATED AT 250 WORDS)

[A long-term follow-up of a patient with DIDMOAD (Wolfram) syndrome].

A 19-year-old Japanese woman developed diabetes mellitus, diabetes insipidus and optic atrophy. Other abnormal ocular findings included color blindness, elevated dark adaptation threshold and constriction of visual fields. Diabetic retinopathy, which is considered to be rare in this syndrome, also was found in the fundi of this patient. During the nine-year follow-up period, diabetic retinopathy deteriorated despite treatment by photocoagulation and vitrectomy, suggesting the importance of ophthalmological examinations in patients with DIDMOAD syndrome.

[Development of slow releasing anticancer drug based with absorbable biomaterial chitin].

To have a comparatively more slowly releasing anticancer drug with effectiveness, Plachitin was prepared by chemical combination of CDDP and chitin (poly-N-acetyl-D-glucosamine). Chitin is absorbed by the living body over several months. To investigate the slow releasing property, it was implanted in thigh muscle of mice and rabbit. Pt level in different organs and in urine was measured at regular intervals. Pt level in implanted muscles was higher in comparison to low serum level in mice. It was released slowly over 1 to 2 months in mice, whereas in rabbit it took about three weeks. Pt releasing period of the Plachitin was different according to the adopted method of implantation. Anticancer effect of Plachitin was investigated by injecting 180 sarcoma cells in mouse peritoneal cavity and subsequent implantation of Plachitin. In control groups chitin was used instead of Plachitin. The survival rate of mice in the Plachitin group after 14 days was higher than in the chitin group, and the anticancer effect of the Plachitin was confirmed.

[The functional relationship between the cerebral cortex and deep brain areas in normal individuals].

Recent PET or SPECT studies have demonstrated a reduction of blood flow and metabolism in the cortex ipsilateral to a deep-seated lesion, and in the thalamus and/or basal ganglia ipsilateral to a cortical lesion. A close relationship between the cerebral cortex and deep areas of the brain has been shown in pathological conditions, presumably because of functional interconnections between the cortex and deeper area. The present study was designed to investigate the relationship of cerebral blood flow in the cerebral cortex to that in the deep gray matter including the basal ganglia and the thalamus in normal subjects. Twenty-two healthy subjects were studied using SPECT with N-isopropyl-p[123I]iodoamphetamine while in a resting state. The asymmetry index (AI) of blood flow in both the cerebral cortex and deep gray matter was calculated as follows; AI = (R-L)/(R+L)/200 (%) (R: right side, L: left side). The AI in deep gray matter was significantly correlated with the AI values in the upper frontal cortex (r = 0.54, p less than 0.01), and parietal cortex (r = 0.58, p less than 0.01), as well as the mean cortical hemispheric AI (r = 0.48, p less than 0.05). Our results suggest the existence of a functional relationship between the cerebral cortex and deep areas not only in pathological conditions but also in the resting state in normal subjects. This functional relationship is likely to be mediated by neuronal mechanisms through the projectional fiber connections between the cortex and the deep gray matter.

[Functional relationships between the cerebrum and cerebellum or the cerebral cortex and deep gray matter in normal subjects and patients with Alzheimer's type dementia].

Since the various parts of the central nervous system are functionally interconnected, functional relationships between one region and another would be expected to be present. We investigated whether functional relationships between the cerebrum and cerebellum or between the cerebral cortex and deep gray matter exist in normal subjects and patients with Alzheimer's type dementia (AD), using the indicator of asymmetry in cerebral blood flow. Eighteen normal subjects (average age: 72.6 +/- 11.4 years) and 21 patients with AD (average age: 75.5 +/- 8.1 years) were studied in the resting state using SPECT with 123I-IMP. The asymmetry index (AI) of blood flow for matched left-right regions of interest was calculated as follows: AI = (R-L)/(R + L)/200(%) (R: right side, L: left side). For both the normal and AD groups, we found a negative correlation between AI in the cerebrum and AI in the cerebellum. The AIs of widespread cortical subregions, except the occipital cortex, were significantly correlated with AI in the cerebellum in the AD group. On the other hand, the only significant correlation found in normal subjects was between the AIs of the frontal cortex and cerebellum. A positive correlation between the AIs in the cerebral cortex and deep gray matter was observed in both the normal and AD groups. Though only the upper frontal and parietal cortices showed significant correlations with the deep gray matter in normal subjects, more extensive cortical subregions showed significant correlations in AD patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Serosal invasion as the single prognostic indicator in stage IIIA (T3N1M0) gastric cancer.

Prognostic factors of gastric cancer with positive serosal invasion and regional lymph node metastasis were evaluated by multivariate analysis. Sixty-seven patients with the T3N1M0 subgroup of stage IIIA disease were evaluated, in which 34.9 +/- 13.4 nodes were dissected in extended lymph node dissection, and 6.5 +/- 6.2 were metastatic. Routine postoperative systemic chemotherapy with mitomycin C and N1-(2'-tetrahydrofuryl)-5-fluorouracil was administered. With this approach, the 5-year survival rate of stage IIIA disease was 47.6%. By Cox proportional hazards model, diameter of serosal invasion was the only significant determinant of prognosis in the T3N1M0 subgroup. The predicted 5-year survival rate for 24 patients with serosal invasion less than 3.0 cm in diameter was 59.5%, compared with 11.5% for 38 patients with invasion of 3.0 cm or larger in diameter. The number of metastatic nodes and the type of operation (total gastrectomy or less than total gastrectomy) did not affect the prognosis. When gastric cancer has both positive serosal invasion and metastatic regional lymph nodes, the diameter of serosal invasion is the more important factor for predicting prognosis.

[Smoking habit and cardiovascular diseases].

The relationship between smoking and ischemic heart disease was discussed, in terms of the smoking habit and the mechanisms of acute and chronic effects of smoking on the cardiovascular system as one of the coronary risk factors, with reference to exercise capacity and coronary flow reserve. The smoking habits of 1000 consecutive patients with ischemic heart disease, who were evaluated with coronary angiography, were analyzed. High percentages of smokers were observed in the younger generation. It was up to 86% in the 4th decade, though it was only 48% in the 8th decade. There was no large difference in other risk factors between smokers and non-smokers. The exercise capacity with and without smoking was evaluated with treadmill exercise test in 6 healthy volunteers. The exercise time was decreased with smoking, compared to without smoking, indicating a decrease in exercise capacity due to smoking. The elevated concentration of carbon monoxide in blood decreased the ability of oxygen transport. The increased lactic acid level in blood with smoking suggested anaerobic energy production acting as a part of the energy source. The smoking increased the myocardial oxygen consumption in relation to increase in heart rate and blood pressure. It decreased coronary flow reserve, shown by a peak to resting flow velocity ratio measured with the Doppler flow velocimeter. In coronary heart disease, therefore, the threshold of myocardial ischemia was decreased by smoking. The decrease in coronary flow reserve recovered with cessation of smoking for more than 2 days.

[Ureteral cancer resembling pulmonary edema induced by recombinant human tumor necrosis factor].

A 73-year-old woman with ureteral cancer and multiple systemic metastasis was admitted complaining of dyspnea on exertion after administration of recombinant human tumor necrosis factor (rH-TNF). On chest examination, coarse crackles were heard during inspiration throughout the lungs. Chest roentgenogram revealed a ground glass infiltration, an air bronchogram in the right lung field and an ill-defined right pulmonary artery. Her symptom and chest roentgenological finding improved and coarse crackles were not heard after discontinuation of rH-TNF. Neutrophils and lymphocytes of bronchoalveolar lavage fluid increased and transbronchial lung biopsy specimens showed slight thickening of alveolar septa with infiltration of inflammatory cells. The pulmonary edema was thought to be caused by rH-TNF which induces adherence of neutrophils to endothelium and stimulates them to increase lysosomal enzyme release and oxygen radical production. As a results, it is thought pulmonary permeability might be increased.

[Functional relationship between the cerebrum and cerebellum in normal subjects].

To determine whether a functional relationship between the cerebrum and cerebellum exists in normal subjects, the correlation between asymmetry in cerebral blood flow and asymmetry in cerebellar blood flow was investigated. Twenty-one healthy right-handed subjects were studied using SPECT with N-isopropyl-p-(123I)iodoamphetamine while in a resting state. The asymmetry index (AI) for both the cerebral and cerebellar hemisphere was calculated as follows. AI = R - L/R + L/200 (%) (R: right side, L: left side). A negative correlation was found between AI in the cerebellum and AI in cerebrum. Especially, AI in cerebellar hemisphere was significantly correlated with AIs in the upper frontal cortex (r = -0.58, p less than 0.01), middle frontal cortex (r = -0.55, p less than 0.02), lower frontal cortex (r = -0.49, p less than 0.05), and mean cerebral hemisphere (r = -0.52, p less than 0.02). These results suggest the existence of a functional relationship between the cerebral hemisphere and the contralateral cerebellar hemisphere in the resting state of normal subjects. We strongly suspect that the frontal cortex exert an influence on the function in the contralateral cerebellum, probably due to a transneuronal mechanism, mainly through the corticopontocerebellar pathway.

[Simultaneous assessment of wall motion and myocardial perfusion by technetium-99m methoxy isobutyl isonitrile].

The aim of this study was to assess whether or not myocardial uptake of Technetium-99m methoxy isobutyl isonitrile (Tc-MIBI) indicated myocardial viability. We performed simultaneous Tc-MIBI angiography and myocardial SPECT at rest on 12 patients with suspected coronary artery disease. Left ventricle was divided into 3 segments, and regional wall motion was graded as normal, hypokinesis and akinesis/dyskinesis. Myocardial uptake of Tc-MIBI was assessed as normal, reduced and absent in each segment. In segments with normal and reduced Tc-MIBI uptake, 7% (2 of 28) and 33% (2 of 6) showed wall motion abnormalities of akinesis/dyskinesis, respectively. However, all segments with absent Tc-MIBI uptake had asynergy of akinesis/dyskinesis (2 of 2, 100%). Myocardial Tc-MIBI uptake at rest indicated wall motion abnormalities and was considered to be useful for the evaluation of myocardial viability. First-pass radionuclide angiography followed by myocardial SPECT with Tc-MIBI demonstrated to be useful for the simultaneous assessment of the left ventricular wall motion and myocardial perfusion.

[A case of traumatic cerebrospinal fluid rhinorrhea manifested ten years after head injury].

In patients with head injury, the total incidence of CSF rhinorrhea was 1 - 3%, and in almost all cases, CSF rhinorrhea occurred within the first three months after injury. We report here a case of a 26 year-old male with CSF rhinorrhea manifested with meningitis who had once been admitted to hospital 10 years previously with a head injury. He had a fracture which we thought had caused a CSF leakage in the left frontal skull base. After conservative treatment for a month we eventually undertook bifrontal craniotomy and repaired the cleft of the skull base with dura and fibrin. In cases in which CSF rhinorrhea had occurred more than 10 years after head injury, including several reports in Japan, surgical treatment had always been required. We think bifrontal craniotomy should be performed in such cases in which it is difficult to locate a cleft in the skull base before the operation. The field of view is wider during the intradural approach. To restore an area with a lack of dura, highly anti-infectionary convexity dura is used a lot, and it is also thought that fibrin is effective in dural repair. The dural deficient areas take 3 to 4 months to repair perfectly, and postoperative observation of the patient's progress is required.

[A long-term follow-up case of sarcoidosis with fibrosis and silicotic nodules in the lung].

An autopsy case of a 70-year-old woman with sarcoidosis followed up for 16 years is reported. She was admitted for examination of bilateral hilar lymphadenopathy on chest X-ray film and uveitis in 1971. Sarcoid granuloma was revealed by Daniels lymph node biopsy. After steroid therapy, bilateral hilar lymphadenopathy and uveitis disappeared. However, small nodular shadows appeared in bilateral lower lung fields on chest X-ray film in 1973. Gradually small nodular shadows increased and reticular shadows appeared. In 1980, chest X-ray film revealed fibrotic change in the bilateral lung fields and giant bulla in the left upper lung field. She developed acute pneumonia and died of acute progression of chronic respiratory failure in 1987. Autopsy revealed sarcoid granulomas and honeycomb lesions in bilateral lungs and also silicotic nodules near the sarcoid granuloma in the upper lobes. This case suggests that inhaled mineral particles may play an important role in the development of pulmonary fibrosis in sarcoidosis.
