Geographic pathology of latent prostatic carcinoma.

Prostates obtained at autopsy from black and white males in the United States of America, from Colombians, from Japanese migrants in Hawaii, and from Japanese in Japan (all over 50 years old) were serially step-sectioned and examined microscopically using identical techniques and diagnostic criteria. The age-adjusted overall prevalence of latent carcinoma was significantly higher in US blacks (36.9%), in US whites (34.6%) and in Colombians (31.5%) than in Japanese in Japan (20.5%). There was no significant difference in prevalence between Japanese migrants in Hawaii (25.6%) and Japanese in Japan. When the carcinomas were subdivided into latent infiltrative type (LIT) and latent non-infiltrative type (LNT), the LIT component reflected upon the overall prevalence of latent carcinoma. There was an increase in the overall prevalence of latent carcinoma and in the prevalence of LIT tumors with age in Japanese migrants to Hawaii and in Japanese of Japan but only a suggestive increase in blacks and whites in the United States. There was no significant relation between age and prevalence in Colombia. In LNT tumors, there was no consistent trend related to race or age. The size of LIT tumors was significantly greater than that of LNT tumors in all races or in each decade of total material. There were many large LIT tumors in blacks and whites in the United States. These results suggest that the prevalence of LIT tumors shows a race variation similar to the incidence of clinical carcinoma of the prostate, and that LNT tumors probably remain latent during the individual's life span.

Primary malignant melanoma of the oral cavity. Case report and establishment of melanoma cell line.

Malignant melanomas rarely grow in the oral mucosa and few established melanoma cell lines originate from the gingiva. Our patient was a 59-year-old man with a dark mass at the upper incisive gingiva and a large lymphadenopathy in the submandibular region. The gingival biopsy showed malignant melanoma. Because of the marked metastases, he received both chemotherapy and immunotherapy. Although regression and discoloration of the tumors were seen, he died of respiratory failure 9 months after admission. At autopsy, metastatic amelanotic and melanotic melanomas were found in various organs. Cells were obtained for tissue culture from pleural fluids of the patient. Eagle's MEM or RPMI1640 with 20% f.c.s. was used for the culture medium, and cells were found to attach to the bottom of the culture tube and to show no contact inhibition. Electron microscopy of primary cultured cells showed premelanosomes in the cytoplasm. The cells were transplantated into nude mice for studies of tumorgenesis and for serial transplantation of the tumor. A progressive tumor appeared. At present (June 19, 1980), 110 passages have been made and the cells are growing well. They were named HMG.

Malakoplakia of the stomach. Report of a case and review of the literature.

A case of malakoplakia of the stomach associated with well-differentiated adenocarcinoma of the transverse colon was studied by light and electron microscopy. The gross appearance consisted of a slightly elevated plaque with umbilication of the covering mucosa. Histologically, the lesion was composed of submucosal proliferation of histiocytes with the typical cytoplasmic inclusions known as Michaelis-Gutmann bodies. Ultrastructurally, these inclusions had a dense central calcified body and concentric alternating rings. Twenty-four cases of gastrointestinal malakoplakia from the literature, including two cases of gastric malakoplakia, were reviewed.

[A case of neuro-Behcet's syndrome in a woman (author's transl)].

An autopsy case of Neuro-Behçet's syndrome is reported with review of the previous 13 cases. A 48-year-old woman who had recurrent oral aphthae and genital ulcers beginning at the age of 27 years suddenly developed several neurological symptoms including headache, paraplegia and disturbance of speech. She died of septicemia due to the urinary infection 22 years after the appearance of the first symptoms and one year after the neurological manifestation. Autopsy revealed a marked atrophy of the frontal lobes of the cerebral hemispheres. Microscopic changes in the central nervous system were consisted of edema and lymphocytic cuffing around the vessels, small foci of softening of the white matter and accumulation of fat-laden cells around these foci, and demyelination. These histologic lesions showed a diffuse distribution in the entire brain without a characteristic localisation, but involvement of the basilar nuclei and the pons was most intense in the present case as observed in most of the previous reports. Microscopic examination of other organs revealed severe pyelonephritis and fatty changes of the liver.

Geographic pathology of occult thyroid carcinomas.

Thyroid glands obtained from patients in southeastern Canada, northeastern Japan, southern Poland, western Colombia, and from Japanese living in Hawaii were serially step-sectioned and examined microscopically using identical techniques and diagnostic criteria. The prevalence of occult papillary thyroid carcinoma was significantly higher in Japan (28.4%) and in Hawaiian Japanese (24.2%) when compared with Canada (6%), Poland (9.1%), and Colombia (5.6%). The carcinomas were all papillary except for a single follicular lesion from Colombia. There was no significant sex prevalence. Most of the patients were between 40 and 79 years of age, but there was no particular predominant decade. Only the Colombian series had a large number of younger patients, and they showed a slightly lower prevalence of occult carcinomas before age 40. Most papillary thyroid carcinomas grow slowly and probably remain occult for the life of the patient.
