[Usefulness of ultrasonography-guided fine needle aspiration cytology in a case of jejunal leiomyosarcoma].

The patient was a 45-year-old man with the symptoms of abdominal pain and constipation. Laboratory data showed no remarkable changes. Ultrasonic and CT-scan examinations revealed a mass deeply situated in the abdominal cavity. Ultrasonography guided needle aspiration cytology was performed. Obtained specimen showed a large cluster accompanied by numerous smaller clusters, and dispersed single cells. Most of cells were spindle in shape with abundant cytoplasm, and uniform in size. These cells have moderately hyperchromatic nuclei with finely granular pattern, which were round or oval, and have tiny nucleoli. Some cells were binucleated. The nuclear membrane was sharp and well preserved. Giant cells or mitotic figures were not observed. As a result, cytological diagnosis was "suspicious of leiomyosarcoma". The tumor was resected. The specimen consisted of a segment of the jejunum with an irregularly-shaped, and reddish soft first-sized mass continued to jejunum muscle layer. The cut surface of the mass showed central cavitation and necrosis in part. Histological diagnosis was well differentiated leiomyosarcoma originated from the jejunum. The tumor was composed of smooth muscle like cells arranged in bundles with some bizarre multinucleated cells and rare mitotic figures.

Latent prostatic carcinoma: pathological and epidemiological aspects.

In conclusion, a review of the epidemiology and pathology of latent prostatic cancer suggests this tumor to be the probable initial phase of a biological continuum that ends with metastatic cancer. Histological grade and tumor volume are highly correlated, as are histological grade and mitotic index. It has also been suggested that atypical intraductal hyperplasia is, in turn, a precursor of carcinoma. This concept gains support from the close association of the two conditions among younger (less than 60 years of age) patients and from their similar mitotic and labeling indices.

Trends in frequency of latent prostate carcinoma in Japan from 1965-1979 to 1982-1986.

By analyzing serial step-sections of whole prostate obtained at autopsy, we determined the chronologic change of the frequency of latent prostate tumor in Japan in two periods: 1965-1979 and 1982-1986. Methods of specimen preparation and examination were identical for both periods. The frequency with which latent prostate carcinoma was found in the 660 samples for 1982-1986 was 34.6% and was significantly higher than the 22.5% seen in the 576 observed for 1965-1979 (P less than .0001). This significant finding can be attributed to an increase in the frequency of latent infiltrative tumor (LIT). However, the increase in the frequency of noninfiltrative tumor (LNT) was less significant (P = .045). Both sets of specimens were subsequently combined and reanalyzed according to the year of birth of the decedents. The LIT has progressively increased in frequency in each age-specific category. By morphometry, we determined that the mean tumor volume decreased due to an increase in the number of smaller tumors during the study periods. Although the frequency of latent prostate carcinoma and the LIT:LNT in the samples obtained most recently were comparable to those of U.S. whites, cancer incidence and mortality rates remain lower in Japan. Apparently, the initial step in the induction of prostate carcinoma in indigenous Japanese is now similar to that in U.S. whites. The rates of clinical carcinoma in Japan are still low when compared with those in the United States and countries in Western Europe, but our findings may presage a time when these differences may be greatly reduced or nonexistent.

[A case report of adenolipoma of the breast].

A rare case of breast adenolipoma is presented. A 49-year-old, otherwise healthy woman had a tumor of the left breast for approximately 25 years. Ultrasound mammography demonstrated a well-demarcated, high echoic mass with islands of a low echoic area in the center of the tumor. The well-defined, oval, soft tumor, measuring 7 x 5 x 3.5 cm in size, was surrounded by a thin capsule which was removed, and its yellow cut surface was found to be mottled by greyish-white areas. Microscopically, the tumor was diagnosed as a breast adenolipoma consisting of fatty tissue interspersed with islands of glandular tissue. The findings of ultrasound examination on adenolipoma of the breast are described and its histogenesis was discussed.

Incidental prostatic carcinoma: morphometry correlated with histological grade.

The histological grades of prostatic carcinoma, as defined by Gleason, were correlated with three methods of morphometry in 254 step-sectioned prostates obtained at autopsy. The variables studied were 1) the number of tumours in each prostate; 2) bilaterality and 3) tumour volume. Each characteristic yielded a statistically significant correlation with histological grade. The strongest correlations were obtained using tumour volume. These autopsy studied help to explain the inconsistent results obtained from morphometric analyses of surgical material, and lend support to the Gleason system as a means of predicting tumour behavior.

Early carcinoma of the gallbladder, noninvasive carcinoma originating in the Rokitansky-Aschoff sinus: a case report.

A patient was reported who underwent cholecystectomy under the diagnosis of chronic cholecystitis with calculi and adenomyomatosis, but it proved to be an early carcinoma of the gallbladder originating in and limited to the Rokitansky-Aschoff sinus (cyst-like structure). Early carcinoma of the gallbladder in this category is rare and this case report may be the first one in the literature.

[Observer variability in the histopathologic grading of prostatic carcinoma].

To evaluate the feasibility of histopathological grading systems for prostatic carcinoma, interobserver variability was studied. Fifty patients with prostatic carcinoma were independently observed by five experienced pathologists according to each grading system (Gleason, Mayo Clinic, Mostofi, Gaeta, M.D. Anderson Hospital and Japanese Uro-Pathological Committee). Consensus reading was made in each case, and subsequently a mean discrepancy score was calculated in each grading system. Among the six systems, the lowest discrepancy score was obtained by the Japanese Uro-Pathological Committee's. It seems that this system is the most suitable for the histopathological grading of prostatic carcinoma.

Nonhepatocytic malignant mixed tumor primary in the liver. Report of two cases.

Among primary tumors of the liver, the group classified as malignant hepatic mixed tumor or hepatoblastoma in adult is among the least common. In this report, two rare cases of nonhepatocytic malignant mixed tumor primary in the liver are reported. Diagnosis was established after autopsy in one and surgery in the other, who is still alive 2 years and 8 months after removal of the tumor by hepatic resection and may be the longest survivor among the nonhepatocytic malignant mixed tumor primary in the liver.

Reliability of cytological grading of prostatic carcinoma compared with histological grading.

Thirty prostatic carcinomas diagnosed simultaneously by fine needle aspiration cytology and punch biopsy were graded into three groups according to their cytological and histological appearances. Among them 23 carcinomas were graded equally by both methods. Cytological examination appears to be suitable for grading prostatic carcinoma, thus being a possible indicator of the patient's prognosis and response to hormonal therapy. In addition to the results obtained here, fine needle aspiration cytology has many advantages such as simplicity, quickness and safety. With increase of attention of urologists and pathologists to fine needle aspiration cytology, this evaluation will become comparable to the histological examination not only as a routine diagnostic tool.

[Atypical hyperplasia and latent carcinoma of the prostate as an early stage of carcinoma].

The prostate gland exhibits various borderline lesions and latent prostatic carcinoma is important as a preceding lesion of clinically manifest carcinoma. In this report, we describe a study of these lesions which was carried out using preparations from five different races. Atypical hyperplasia (AH) is the most important of these borderline lesions. Histologically, latent carcinoma exhibits a wide range, from poorly to well differentiated types, and is widely accepted to be the preceding state of clinically manifest carcinoma. Both lesions were common in all five races with a frequency several hundred times greater than the mortality rate for prostatic carcinoma. A significant correlation was found between the prevalences of AH and latent carcinoma, both of which had a tendency to show multifocal growth. Although there were many latent carcinoma cases exceeding 130 mm3 in size and poorly or moderately differentiated, rare cases would advance to clinically manifest carcinoma. Since the formation of tiny carcinomas and progression to clinically manifest carcinoma are two different phenomena, it is considered important to study the factors affecting the latter phenomenon.

[Experimental and clinical effect of hypertransfusion and OK-432 on granulocyte recovery].

The experimental and clinical studies were carried out to alleviate the bone marrow suppression by antineoplastic drugs. Faster recovery of granulopoiesis was observed by pretreating recipients with RBC hypertransfusion and/or OK-432 (Picibanil, biological products of beta-streptococci.) In experimental mice, higher granulocyte counts could be maintained with hypertransfusion in the peripheral blood, and the recovery of granulopoietic series and CFU-S of bone marrow cells were found to be more rapid after cyclophosphamide administration. OK-432 also resulted in higher peripheral granulocyte, whereas the total nucleated cell counts and CFU-S were decreased in the bone marrow, suggesting sparing bone marrow granulocyte reserve and its migration to the peripheral blood. The mechanism of higher granulocyte count after hypertransfusion was not clearly explained but it was considered that erythroid suppression caused colateral flow of multipotential stem cells to granulopoiesis. The effect of both combinations was unexpectedly less significant in the recovery of granulopoiesis, but it was thought that the optimal time interval should be sought between pretreatment and the administration of anti-neoplastic agents. The clinical use of hyper transfusion and OK-432 also proved the alleviation of granulocytopenia, and rapid granulocyte recovery at the time of consolidation therapy among children with AML.
