Reye's syndrome in three siblings. Association with type A influenza infection.

Reye's syndrome occurred simultaneously in three siblings. All had characteristic changes in liver histology ascertained by light and electron microscopy. The two patients with the most severe disease also had serum that caused marked stimulation of mitochondrial state 4 respiration in vitro. Histocompatibility locus antigen typing showed that no unique haplotypes were shared by all three children. There was no evidence of exposure to aflatoxins, pesticides, or other substances that have been previously postulated to be associated with Reye's syndrome. Influenza A/Brazil/11/78-like virus was isolated from one patient, and all three patients had a fourfold or greater rise in hemagglutination-inhibiting antibody to H1N1 influenza virus.

The use of preoperative plasma CEA levels for the Stratification of patients after curative resection of colorectal cancers.

One hundred forty-five patients with colorectal cancer were analyzed in order to correlate the preoperative plasma carcinoembryonic antigen (CEA) levels with the sites and times of disease recurrence. The median follow-up periods of these patients was 50 months (range 36-72 months). Twenty-one patients were found to have metastases at the time of their operation. None of the seven patients whose primary tumors were classified as Dukes/Kirklin A have had tumor recurrence. Seventeen per cent of the patients with Dukes/Kirklin B tumors have had tumor recurrences, and 63% of the patients with Dukes/Kirklin C colorectal primary tumors have had tumor recurrence. No correlation was found between preoperative CEA values and subsequent risk of tumor recurrence or times to recurrence among the patients with Dukes/Kirklin B colorectal primary cancers. In Dukes/Kirklin C patients, however, elevated preoperative CEA values predicted a higher risk of tumor recurrence. Ninety per cent of the patients (19/21) with preoperative CEA levels greater than 5.0 ng/ml have had relapses, with a median time of 17 months before disease recurrence. Only 39% (9/23) of the patients with Dukes/Kirklin C lesions and CEA levels less than 5 ng/ml have had relapses and there is insufficient follow-up data as yet to determine the median survival time. If those patients whose Dukes/Kirklin C primary tumors were poorly differentiated on histologic examination are excluded, the contrast between patients having CEA levels greater than 5.0 ng/ml and those having CEA levels less than 5 ng/ml is even more marked. Sixteen of the 18 remaining patients whose CEA levels were greater than 5.0 ng/ml prior to curative resection have had relapses as compared with only three of 15 patients whose preoperative CEA values were less than 5. We conclude, therefore, that CEA is an important factor in stratifying patients after curative resection of their Dukes/Kirklin C colorectal tumors.

The changing role of pancreatic arteriography in the era of computed tomography.

Computed tomography (CT) in the assessment of suspected pancreatic disease, although an excellent screening procedure, has certain shortcomings, such as a significant percentage of inaccurate studies, a low predictive value of the finding of a pancreatic mass, failure to detect small lesions, and inability to differentiate localized masses caused by pancreatitis from those caused by adenocarcinoma. Arteriography provides important additional information in patients with clinically suspected pancreatic lesions and positive findings on CT or other noninvasive screening studies in whom surgical resection is contemplated. This procedure helps determine the presence and resectability of adenocarcinoma and can also demonstrate lesions which may resemble pancreatic adenocarcinoma on CT but which, in reality, are nonmalignant or nonpancreatic or both. Arteriography should no longer be used as a screening procedure but should be performed whenever a potentially resectable pancreatic mass, either cystic or solid is suggested by CT.

Four new dialyzers.

Dialyzers with a variety of sizes and performance characteristics are presently available for the dialysis of adult patients. We evaluated four currently available disposable adult dialyzers in vitro and in vivo during treatment of adult patients. All four dialyzers, Gambro GLP 1.36m2, 17 mu, Cordis Dow 3500, Gambro GF 120M, and Travenol HD 1000 were well suited to the dialysis of adult patients. By considering the clinical status of the patients and the performance characteristics of the dialyzer (solute removal, priming volume, ultrafiltration, etc.) it is possible to employ a hemodialyzer which more precisely meets the needs of the patient without increasing risk factors associated with hemodialysis.

Toxicity of amphotericin b in children with cancer.

The pattern of amphotericin B toxicity was assessed retrospectively in a group of 20 children with cancer who had received one or more courses of the drug for treatment of systemic fungal infection. Azotemia was the most frequent complication, developing during 23 of 24 treatment courses. Other major toxic effects, in decreasing order of frequency, were anemia, hypokalemia, thrombocytopenia, and neutropenia. Infusion side effects, including drug-related fever, chills, and nausea, were also frequently seen. Seventeen of 20 patients were treated for disseminated histoplasmosis. Nineteen of 20 patients had acute leukemia. Although interaction with other agents could not be excluded, amphotericin B appeared to be the major causative agent for the toxic reactions noted. In no patient, however, was administration of amphotericin B stopped because of drug toxicity.

Gangliosides and other lipid micelles. A study of amine binding by a dialysis/fluorescence method.

The binding of serotonin to bovine adrenal medulla gangliosides (BAMG), bovine brain gangliosides (BBG), and BBG-synthetic lecithin mixed micelles has been demonstrated by a rapid technique using fluorescence monitoring of dialysis rates. BAMG micelles bound 0.10 mM serotonin at about half the efficiency of BBG micelles having the same sialic acid concentration (0.50 mM) in water. In water, the effect of BBG micelles was essentially identical to that of mixed micelles containing the same quantity of BBG. Ca2+ (1.20 mM), however, cancelled the effect of the mixed micelles on serotonin dialysis and reduced the effect of BBG micelles to about one quarter of that observed in water. These and other observations are related to earlier studies and to neurochemical processes which may involve gangliosides.

Nasogastric suction in alcoholic pancreatitis.

In order to assess the usefulness of nasogastric suction in acute alcoholic pancreatitis, 37 patients with alcoholic pancreatitis were prospectively investigated. The study failed to demonstrate efficacy of nasogastric suction in those patients with mild disease. Application of a system of prognostic signs proved useful in discriminating between mild and severe disease. Routine use of ultrasound examinations detected three pancreatic pseudocysts before they became clinically apparent. In instituting appropriate therapy in mild pancreatitis, factors such as patient comfort should be considered in the absence of proven significant value of nasogastric suction.

Interspecies comparison of carcionogenic potency.

For guidance in decisions on how to safeguard humans from carcinogens, it is necessary to use data on carcinogenesis in animals. This paper disucsses how such data, combined with human experience, may be used quantitatively in such decisions. It is demonstrated empirically that good correlations exist between different species for suitably defined carcinogenic potencies for various chemicals. This allows sufficient accuracy in extrapolating form animal data to human risk to support a logical scheme for the evaluation of such risks. Some recommendations for future research are given.
