The clinical picture of chondromalacia patellae is outlined by the patellar syndrome. The diagnostic value of double contrast arthrography by tangential X-ray (skyline-view)-technique of the femoro-patellar-joint is stressed. If conservative treatment is not satisfactory, osteotomy and ventral transposition of the tuberositas tibiae should be performed. When the patella and the extensor apparatus are dislocated laterally, it is necessary to displace the tuberositas in medial direction. From 1976 to 1980 159 patients have been treated this way. Follow-up study in 86 cases (1 to 5 years after operation) shows excellent and good results in a percentage of 77.
Pancreatic heterotopia is a developmental anomaly. When it produces complications, symptoms depend on the site of the lesion and may reflect the whole range of pathology affecting pancreatic tissue. In the ileum, pancreatic heterotopia is usually associated with a Meckel's diverticulum and may cause intestinal obstruction by intussusception. Isolated ileal pancreatic rests are rare and usually are discovered incidentally during surgery for other conditions. A case of a polypoid mass in the ileum containing aberrant pancreatic tissue that had been causing recurrent small bowel obstruction is described. Resection and primary anastomosis were curative.
Two independent, three-dimensional structures of yeast tRNAAsp, mainly differing by the conformation of the D loop, have been obtained from a multiple isomorphous replacement (MIR) X-ray analysis at 3.5-A resolution. The folding of the ribose-phosphate backbone is similar to that found for tRNAPhe; major differences concern the relative positioning of the acceptor and anticodon stems, and the conformation of the loops in the two molecules. Crystal packing involves self-complementary GUC anticodon interactions.

The ability of a mouse mammary tumor cell line to abrogate antibody neutralization of vesicular stomatitis virus was shown to be due to the presence of mycoplasma. The mycoplasma was isolated from the cell line and typed as Mycoplasma orale. Colonies of this mycoplasma were used to deliberately infect cell cultures which then gained the capacity to reactivate antibody-neutralized virus. The extent of the reactivation depended on the source of neutralizing antiserum. Other species of mycoplasma were tested and were found to reactivate neutralized virus, indicating that this may be a general phenomenon of mycoplasma contamination.
Two types of deoxyoligonucleotides, a nona- and two dodecamers have been synthesized as a basis of various aspects in structural and functional studies of the DNA. Using different methodological approaches highly purified deoxyoligomers were obtained and characterized by UV absorption and CD measurements. As an example the results presented are concerned with the synthesis of the following dodecanucleotide containing the recognition site of Hind III : dC-A-C-A-A-G-C-T-T-G-T-G. CD properties and some structural related effects of two complementary and a selfcomplementary deoxyoligomers are described.
The increasing use of vagotomy for the surgical treatment of ulcers has stimulated interest in the structure and function of the gastric wall innervation. This paper presents a survey based on microphotographs (rat, rabbit, and guinea pig) of the stomach innervation. Special attention is paid to the relationship between the vagus nerve and the intramural nervous system and the ganglion cells, synaptic junctions in the gastric wall, the innervation of the mucosa and its relationship to mast and parietal cells. The functional significance of the findings is discussed: (1) the electron microscopic evidence of synaptic junctions on parietal cells shows that hydrochloric acid secretion is influenced directly by the nervous system. The nerves terminating at the parietal cells originate exclusively from the myenteric plexus where the vagus fibres already terminate without entering the mucosa. (2) The innervation of mast cells and their proximity to the parietal cells makes some indirect nervous influence on hydrochloric acid secretion likely.
Cloned fibroblasts from women heterozygous for X-linked ichthyosis (steroid sulfatase deficiency) were examined to see whether or not this locus is subject to X-inactivation. Of 103 clones examined, all had normal levels of steroid sulfatase activity. Two of the women studied were also heterozygous for glucose-6-phosphate dehydrogenase deficiency. This allowed the demonstration that both X chromosomes were represented as the active X in various clones and that selection did not account for these findings. Thus, the steroid sulfatase locus, like the Xga locus to which it is linked, appears to escape X-inactivation in man.

Seven autopsy cases of intramedullary metastases, four in the cervical spinal cord, are reported and the literature reviewed. Whereas lung and breast cancer, malignant melanomas and lymphomas are reported as the most common primary tumors, the present series included three cases of breast carcinoma and two cases each of colon and oat cell carcinoma of the lung. Neither the clinical symptoms nor the neurological signs distinguished intramedullary metastases from the more common extradural deposits, but radiological evidence of vertebral metastases and myelographic stop were present in only one case each, and CSF cytology was negative. Intramedullary deposits in this series were neither associated with extradural tumor nor with spread into the subarachnoid space, while cerebral metastases were present in four cases. This favors hematogenous dissemination rather than direct transdural or perineural spread of these lesions.
A woman, aged 58, who had undergone prolonged treatment with corticosteroids for generalized necrotizing vasculitis, and had received azathioprine (Imurel) for 18 months, 4 years after discontinuing the latter treatment, developed peripheral neuropathy and a rapidly progressing cerebral disorder suggesting a basal meningeal process. CSF cytology suggested malignant lymphoma with meningeal involvement. Immunological studies showed an increase of Null-lymphocytes in the peripheral blood. Autopsy disclosed a primary malignant lymphoma of the CNS with the histological appearance of a multilocular immunoblastoma showing almost ubiquitous meningocerebral involvement. Clinical and postmortem examinations failed to demonstrate any systemic extraneural lymphoproliferative disorder. In addition, there was peripheral polyneuropathy of the axonal type with denervation atrophy of skeletal muscle, but without lymphomatous involvement of the neuromuscular system. The possible relations between primary malignant lymphomas of the CNS and previous immunosuppressive treatment of immunoinflammatory disease are discussed in view of the concept of impaired immunoregulation. The pathogenetic background of peripheral polyneuropathy is unknown.
Postoperative survival time and recurrence-free intervals in 116 consecutive patients with supratentorial grade III and IV gliomas (glioblastomas, gliosarcomas, anaplastic astrocytomas, and ependymomas) were compared in unselected groups receiving different forms of treatment. Postoperative high-voltage radiotherapy (31 patients, dosage 4,000--6,000 rads) and combined chemotherapy consisting of CCNU, vincristine, amethopterine, and procarbazine in 15-day circles (COMP protocol) (12 patients) showed the same median survival time of 10.6 months and comparable recurrence-free intervals of 6.8 and 7.0 months, respectively. These results were significantly different from a control group (39 patients) receiving best postoperative supportive (conventional) care (median survival 5.4 months, free interval 3.7 months). Combination of postoperative radiotherapy with simultaneous polychemotherapy (COMP protocol), evaluated in 18 patients, did not significantly change the recurrence-free interval (median 7.0 months), but increased the median survival time to 12.9 months, which was significantly superior to the two other treatment groups. The toxic side effects of COMP therapy were moderate and essentially haematological. In general, simultaneous radiation and chemical treatment was well tolerated after major tumour resection. These preliminary results of postoperative combination of radiation and polychemotherapy for anaplastic supratentorial gliomas appear encouraging, but further trials for optimization of combined therapeutic strategies are warranted.
In studies of some sequence dependent structural factors and stabilizing effects of oligonucleotides the octanucleotide d(pG-A-T-C-T-T-T-T) was of particular interest in view of the presence of an endonuclease cleavage site. Its chemical synthesis is reported as well as the structural effects in CD spectral properties of the octanucleotide and of some related compounds.
Over a thirteen year period (1965 to 1977) a total of 4790 skin scrapings and hair samples of animals were examined mycologically. 887 strains of dermatophytes were isolated out of 885 of these samples (= 18,5%). Most frequently Trichophyton verrucosum was identified in samples from cattle, followed by Microsporum canis isolated from cats, dogs and zoo animals. T. mentagrophytes was mainly found on guinea pigs, chinchillas and dogs and T. equinum on horses. Although the total number of the samples examined within the last 8 years increased, the total of the dermatophytes isolated remained proportionately the same. The relative numbers of the various species of dermatophytes isolated did not change within the period of investigation.
Steroid sulphatase activity was determined in cultured fibroblasts from 25 individuals with X-linked ichthyosis from four countries. All those with X-linked disease had markedly reduced enzyme levels compared with controls and patients with other types of ichthyosis. X-linked ichthyosis seems to be the result of a common mutation affecting the expression of steroid-sulphatase activity.
