The diagnosis of antisocial and hysterical personality disorders. An example of sex bias.

The author submits evidence to support the contention that antisocial and hysterical personality disorders are essentially sex-typed forms of a single condition. The features of each condition, he argues, are either common to both disorders or are related to sexual stereotypic traits. He reports on a study which indicates that mental health professionals tend to label men as antisocial personalities and women as hysterical personalities, even when these patients have identical clinical features. It is suggested that a unification of the two labels could have a beneficial effect on research and on understanding and treating the disorder and related conditions.

Vanadate is a potent (Na,K)-ATPase inhibitor found in ATP derived from muscle.

A potent (Na,K)-ATPase inhibitor purified from "Sigma Grade* ATP has been identified as vanadium using electron probe microanalysis and confirmed by microwave-induced emission spectroscopy and electron paramagnetic resonance spectroscopy. Sodium orthovanadate (Na3 VO4) is identical with the purified inhibitor with respect to ultraviolet absorbance, migration on thin layer chromatography, and inhibition of (Na,K)-ATPase. The (Na,K)-ATPase is in-inhibited 50% by 40 nM Na3 VO4 under optimal conditions (28 mM Mg2+) and the inhibition is 100% reversible by millimolar concentrations of norepinephrine. The physiological significance of this inhibition is discussed in relation to vanadium concentrations in vivo.

Witchcraft and soul loss: implications for community psychiatry.

The author uses the case study of a Chicana patient who believes in witchcraft and soul loss to illustrate the importance of understanding alternate theories of disease causation. He describes the patient's unsuccessful attempts to find treatment appropriate for her culturally determined beliefs and feels her efforts were hampered because she was a member of a culture in transition. She was finally treated through home visits and medication. The author discusses factors related to the underuse of mental health services by Chicanos and the role of the indigenous therapist. He also suggests how mental health centers can be made more relavant to Chicanos by providing such services as home visits and family therapy.

Mental development of phenylketonuric children on or off diet after the age of six.

Eleven children with phenylketonuria (PKU) taken off diet after the age of six showed significant decreases in rate of mental development compared with 26 control children children of comparable IQ and with 17 PKU children of comparable IQ who remained on the diet. Changes in rate of mental development were significantly and inversely correlated with plasma phenylalanine levels in treated PKU.

Adventitious hysterogram during intravenous pyelography.

This study reports our experience with uterine opacification noted during infusion urography. The phenomenon is observed in 21.7 per cent of urograms performed on adult females. Examples of this entity are presented and the pathophysiology of the uterine opacification discussed.

Cryotherapy of metastatic carcinoid tumors.

BACKGROUND: To describe the use of hepatic cryotherapy to treat patients with symptomatic carcinoid metastates. METHODS: Hepatic cryotherapy was performed on five patients with carcinoid syndrome resulting from metastatic carcinoid tumors. Intraoperative ultrasound was used to guide the cryotherapy and to assess the adequacy of freezing. RESULTS: All five patients had relief of the carcinoid syndrome after treatment. In four of the five patients, the relief was prolonged (>3 months); in one patient, the relief of symptoms was transient (2 months). Four of five patients had a transient reduction in hormonal tumor markers (the fifth patient did not have hormonal-level follow-up). During a follow-up period of 2.5 years, four of the five patients died. The 6-month survival rate was 80%, the 1-year survival rate was 60%, the 2-year survival rate was 40%, and the 2.5-year survival was 20%. One patient is alive 30 months after treatment. CONCLUSION: Hepatic cryotherapy can provide symptomatic relief for patients with hepatic metastates producing the carcinoid syndrome.

Schizophrenia and the environment: speculative interventions.

OBJECTIVE: Starting from research on environmental factors that influence schizophrenia, the author speculates on innovations that could reduce the occurrence of the illness, improve its course and enhance the quality of life of sufferers and their relatives. RESULTS: Ranging from education about obstetric risks to a stigma-reducing campaign, the suggestions could be of interest, not only to clinicians, but also to advocates, policy-makers and communications specialists. CONCLUSIONS: Several of the suggestions, which would be innovative in the United States, are already established features of Italian mental health care. These features include social enterprises, family support payments and a disability pension system without severe disincentives to work.

Design and assembly of an 8 tesla whole-body MR scanner.

PURPOSE: The purpose of this report is to describe the design and construction of an 8 T/80 cm whole-body MRI system operating at 340 MHz. METHOD: The 8 T/80 cm magnet was constructed from 414 km of niobium titanium superconducting wire. The winding of this wire on four aluminum formers resulted in a total inductance of 4,155 H. Gradient subsystems included either a body gradient or a head gradient along with a removable shim insert. The magnet and gradient subsystems were interfaced to two spectrometers. These provided the control of the gradient amplifiers and the two sets of four RF power amplifiers. The latter provide in excess of 8 kW of RF power from 10 to 140 MHz and 10 kW of RF power from 245 to 345 MHz. A dedicated computer-controlled patient table was designed and assembled. The entire system is located in a clinical setting, facilitating patient-based studies. RESULTS: The 8 T/80 cm magnet was energized without complication and achieved persistent operation using 198.9 A of current, thereby storing 81.5 MJ of magnetic energy. Exceptional performance was observed for nearly all components both in isolation and when combined within the complete system. CONCLUSION: An 8 T/80 cm MRI system has been assembled. The magnet subsystem is extremely stable and is characterized by good homogeneity and acceptable boil-off rates.

Survey: use of clear and full liquid diets with or without commercially produced formulas.

A survey of hospital dietitians was taken to determine the usage of clear and full liquid diets in the United States. All of the 299 who responded had these diets in their hospital manuals. Clear liquid diets in 82.3% of the hospitals provided less than 1000 kcal/day, while 27% of full liquid diets contained less than 1500 kcal/day. Both diets were usually taken for 3 or less days. Nearly 75% of those responding rarely used commercially prepared formulas with clear liquid diets whereas commercial formulas were often given with full liquid diets. Higher kilocalories per day are provided when these liquid diets are supplemented with commercial formulas. Prescribing foods only because they are liquid or liquify at room temperature regardless of their osmolarity or kilocalories has little rationale. The addition and/or substitution of newer commercially prepared formulas is suggested.

Stroke rehabilitation: benefits of educational initiatives.

The supportive education of stroke survivors and their carers has been identified as a key area of interest to rehabilitation nurses. The Audit Commission recommends the widespread use of stroke units within the NHS (Audit Commission, 2000). The aim of this review is to determine the authority of available evidence and to discuss how such evidence might influence practice. The literature tended to assume that education had a positive effect on the outcome of rehabilitation. Various education interventions were attempted, including information-giving through the use of written material as well as more supportive and facilitative approaches. Factors that may affect the ability to learn or absorb information were identified and attempts have been made to evaluate the effect of educational interventions.

The relationship between language and disease concepts.

The Sapir-Whorf hypothesis, well-known in anthropological linguistics, postulates that language may not only describe the world we inhabit but also mould the way we experience it. This paper argues that an individual's language may similarly determine his conception of disease. Possible relationships between linguistic features and disease concepts are cited for the Eskimo, the Navaho, and the Chines, and it is suggested that, in European languages, the extensive use of spatial metaphors to express abstract concepts may encourage a more rigid categorization of disease and inhibit the ability to conceive of multiple factors in disease causation. The use of nouns rather than verbs to express the idea of illness could lead to a static view of disease and tends to separate illnesses as distinct entities rather than defining them as aspects of bodily functioning. The bipolar structure of Indo-European languages, setting subject against predicate and noun against verb, may play a part in the persistence of the mind-body dichotomy and restrict the holistic perception of man with nature. These linguistic features, in leading to a conception of disease as a rigidly defined, unchanging, unicausal thing, may encourage an over-use of surgery and lead to difficulty in perceiving social and psychological factors in disease.

The effectiveness of nursing in stroke units.

BACKGROUND: Richard Warner examines how nurses might contribute to the effectiveness of organised stroke care by linking the theoretical basis of nursing with outcomes that could be more meaningful to the stroke survivor. CONCLUSION: Although research shows that specialised stroke units are effective in limiting morbidity and mortality, and in promoting functional ability, the various care roles of the multidisciplinary team are not clear.
