S-100 protein immunostaining in cells of dendritic morphology within reactive germinal centers by ABC immunoperoxidase method.

The "dendritic" and the "interdigitating" (IRC) reticulum cells are commonly reported to have different topographical distribution in the human lymphoid tissue and some peculiar cytochemical and immunohistochemical features. These include the detection of S-100 protein by PAP (peroxidase anti-peroxidase) method only in IRC located in the thymus-dependent areas. In 15 reactive lymphoid tissue specimens (11 lymph nodes, 3 tonsils, and 1 adenoid) the presence of S-100 protein was tested by ABC (avidin-biotin complex) immunoperoxidase method. IRC were constantly positive. Other positive cells were located within the follicular germinal centers; these immunostained cells appeared as a striking network composed of dendritic-shaped processes displaying a finely granular positivity for S-100 protein. It is suggested that by using this very sensitive technique, S-100 protein can also be detected in intrafollicular cells of dendritic morphology.

Demonstration of S-100 protein distribution in human lymphoid tissues by the avidin-biotin complex immunostaining method.

Immunoreactivity for S-100 protein was investigated immunohistochemically in a series of 49 fixed and paraffin-embedded normal, reactive, and neoplastic human lymphoid tissue specimens. The avidin-biotin complex immunoperoxidase method was used, with overnight (12-hour) incubation with a commercially available antiserum to S-100 protein. In addition, cryostat sections were tested with DRC 1 monoclonal antibody to dendritic reticulum cells (DRCs) in three cases and with OKT6 antibody to interdigitating reticulum cells (IRCs) in nine cases. All tissues, including lymph nodes, tonsils, adenoid, spleens, appendices, thymuses, and tissues containing nodular reactive lymphoid infiltrates, demonstrated a consistent immune staining pattern. A striking network composed of dendritic processes that showed finely granular S-100 protein immunoreactivity was observed in most of the follicular germinal centers; a similar dendritic pattern was observed in the follicular centers when the corresponding frozen sections were immunostained with DRC 1. In the extrafollicular areas, the S-100-positive cells topographically and morphologically resembled the IRCs that were demonstrated by OKT6 antibody in the corresponding frozen sections. The results seem to indicate that cells topographically and morphologically similar to IRCs and DRCs in human lymphoid tissues from different sites share immunoreactivity for S-100 protein. The present study confirms the unexpected presence of S-100 protein in dendritic cells of follicular germinal centers by a simple and currently available method.

Hemangioendothelioma of bone. A report of an unusual case with lymph node metastasis.

An unusual case of hemangioendothelioma of bone with regional lymph node metastasis in a 54-year-old woman is reported. The patient died nine weeks after admission of circulatory collapse. Six possibly similar cases were found in reviewing the literature. It is concluded that lymph node metastases from hemangioendothelioma of bone appear to be an extraordinary event and may represent an extremely unfavourable prognostic finding.

Phase II study of teniposide (VM-26) in multiple myeloma.

From September 1979 to December 1983, a phase II trial with teniposide (VM-26) in multiple myeloma (MM) was conducted at our institution. Of the 30 patients entered, 25 were evaluable for response, 12 previously treated with M-2 protocol and 13 previously untreated elderly (greater than or equal to 70 years) patients. A median of nine cycles (range 1-21) of VM-26 was administered. Seven responses (28%) according to Myeloma Task Force criteria were observed with a median duration of 4 months (range 2-12+). Four responses (33%) were observed in the 12 previously treated patients. Overall toxicity was mild. VM-26 seems an active drug in MM, without significant toxicity even in elderly patients.

Antitumoral activity of human fibroblast interferon administered intranodularly.

Human fibroblast interferon was given intranodularly to 14 patients with cutaneous metastases from breast cancer and malignant melanoma; 1,000,000 units/cm3 of tumor tissue was administered daily for 8-10 days. 13 patients were evaluated. Complete response was achieved in 1 of 7 breast carcinoma nodules and in 2 of 6 melanoma nodules; partial response was achieved in 1 of 7 breast carcinoma and in 3 of 6 melanoma nodules. The overall objective response was therefore 7 of 13 (53.8%) metastatic nodules. Pathological complete response was confirmed in 2 of 3 complete clinical responses. Necrosis, lymphocytic infiltration and fibrosis were observed in all the specimens pathologically examined. In spite of a clear antitumoral activity, this approach does not appear to have clinical significance due to its extremely localized effect.

A phase II trial of teniposide (VM 26) in advanced non-Hodgkin's lymphoma, with emphasis on the treatment of elderly patients.

Fifty-four patients entered a phase II trial of teniposide (VM 26) in Stage III (35 patients) and stage IV (19 patients) non-Hodgkin's lymphoma (NHL) classified according to modified Rappaport system. The median age was 71 years (range, 19-85). Thirty-two patients were previously treated at least with combination chemotherapy and radiotherapy, whereas 22 were elderly (range, 70-85 years) untreated patients with a median Karnofsky score of 70. VM 26 was given by IV infusion at 100 mg/m2 weekly for at least 3 doses in "unfavorable" histologic subtypes, and for at least 6 to 9 doses in "favorable" subtypes, prior to the evaluation of response. The overall objective response rate was 43% in the 51 evaluable patients. The median duration of the 12 complete responses (CRs) was 7+ months (26+ to 2). According to the histology, VM 26 was very effective in the six patients with diffuse "histiocytic" (DH) subtype (four CRs, one partial response [PR]), and in the 8 patients with mycosis fungoides (MF) (two CRs, two PRs). Diffuse lymphocytic poorly differentiated and lymphoblastic NHL were less sensitive subtypes to VM 26. Among the 20 evaluable elderly patients a 50% objective response rate was obtained with five CRs. Four CRs and one PR were obtained in the five patients with DH subtype; no response was obtained in the only patient with MF. Toxicity, usually hematologic, was mild, even in elderly patients; neurotoxicity occurred in four instances. VM 26 seemed to be an effective and well-tolerated drug in advanced NHL; this drug should be further evaluated as first line chemotherapy in elderly (greater than or equal to 70 years) previously untreated patients with poor general conditions and DH histology.

A psychoeducational program dealing with child abuse for elementary school children.

This paper reports the outcome of an attempt to teach children in grades 5 and 6 about child abuse and neglect. The Child Abuse Component of the Human Relations Program described and evaluated here is a unique effort to provide children with an opportunity to discuss and learn about this aspect of family violence. An impact assessment was conducted by documenting possible changes in student knowledge and attitudes regarding child abuse as a result of exposure to the Child Abuse Component of the Human Relations Program. The test group of children consisted of 315 boys and girls in grades 5 and 6. A comparison group of 298 children was closely matched with the test group on the basis of age and sex. These groups were formed by randomly selecting 12 teachers from all teachers agreeing to use the Component and 10 teachers choosing not to employ the Component. The students in the classrooms of these teachers received the same curriculum for their grade level with the exception of the field-test group additionally receiving the Component. Selected child abuse knowledge, attitude, and personality (security) measures were given all subjects in a post-test with comparison group research design. Teacher knowledge and attitude were also assessed. The results of the data analysis, teacher, and special observer reports suggest that the Component was effective in meeting its objectives and was not disturbing to test subjects. Apparently, the test children profited by experiencing the Component mainly because of the opportunity it provided for class discussion. Moreover, the knowledge levels and interest of the children in both groups was seriously underestimated.

Malignant lymphoma, undifferentiated, Burkitt's type in north-east Italy.

Burkitt's lymphoma occurred in 15 Italian patients seen at Pordenone General Hospital over a period of seven years representing 8% of all malignant lymphomas seen in the same period. All patients were born and lived in a region of 100 miles radius around the north-eastern Italian town of Pordenone. Five patients lived in the same rural village, and another two were distant relatives. There were eleven males and four females; the median age was 29 years (range 9-68) with ten patients older than 15 years. Primary tumor was in the gastrointestinal tract regions in nine patients and in lymph nodes in six patients. Epstein-Barr virus serology was positive in one of the 12 patients tested. According to the Ziegler staging system, three patients were stage A, three stage AR, five stage C and four stage D. Treatment consisted in surgical removal of bulky tumor followed by combination chemotherapy in seven patients, combination chemotherapy alone in seven patients and palliative radiotherapy in one. Median survival was significantly higher in patients who have obtained an objective response than in those who have not (11.5 versus 4 months). Median survival was not significantly influenced by age, stage and treatment. This report would emphasize that in Europe there are areas of relatively high incidence of Burkitt's lymphoma, and the province of Pordenone is one of these: it must be considered in the histological diagnosis of lymphoma in these areas.

The effect of cadmium on the environment.

Information on the effect of cadmium in the environment reported during the past several years has raised considerable questions about the validity of past conclusions. For example, it appears that the critical concentration of cadmium in the renal cortex may be significantly higher than previously thought, and also, there is growing evidence that cadmium was not responsible for Itai-Itai disease in Japan. These findings concerning cadmium are encouraging and indicate that while cadmium is certainly toxic, the new information should be considered by governmental authorities before steps are taken to enact overly stringent regulations based on assumptions and regulations that may no longer be correct.

Botryoid rhabdomyosarcoma of the nasopharynx.

A case report of a Stage III botryoid rhabdomyosarcoma of the nasopharynx associated with a six-and-a-half-year survival is presented. Treatment consisted of surgery, radiotherapy (6,000 rads TCT) to the nasopharynx and maxillary sinuses bilaterally, and six cycles of polychemotherapy with Vincristine, Adriamycin, Cyclophosphamide and DTIC, without major loss of function or cosmetic deformity. The histology of the lesion is discussed with reference to recent classification and prognosis. The authors suggest that the histological type and prognosis of rhabdomyosarcoma of the nasopharynx in children may be better correlated in future studies.
