[Urodynamic examination results for the determination of urge urinary incontinence in the woman].

Before the beginning of the treatment of incontinent females a clarification of the cause of the disease is in every case necessary, since a stress incontinence indicates quite another therapy than an urge-incontinence. In the present study the results of altogether 21 females are demonstrated, whose diseases were classified as urge-urine incontinence on the basis of the anamnestic data as well as the findings of the examination. The results are compared with the references of literature concerning the possibilities of the evidence of the individual investigation methods (cystometry, urethrocystometry, urethrocystography) as well as discussed with the results of an own control group of healthy women.

[Interpretation and meaning of urethrocystographic findings in the diagnosis of urinary incontinence in women].

In this study the author adopts a definite attitude to the diagnostic evidence of the urethrocystography in the urinary incontinence of the female. In these cases female patients were controlled before and after the stress incontinence operation as well as before and during the conservative treatment of the urge-urinary incontinence. As criteria for the evaluation the inclination angle of the urethra, the pubourethral angle and the posterior vesico-urethral angle were used. The final urethrocystographic findings as well as the parameters after the therapy were statistically treated and discussed with the results of literature.

[Relevance of urethral pressure profile to diagnosis of stress incontinence (author's transl)].

Reference is made to the relevance of urethral pressure profile and its parameters, on the basis of urethrocystometric checks of 34 control patients and of another 44 women who had had to undergo surgery for urinary incontinence. In cases of urinary stress incontinence, values prior to surgical treatment generally were lower, but some of the parameters were (statistically insignificantly) enhanced by the operation. - The conclusion is that the urethral pressure profile at rest and under stress (with coughing provoked) provides a more consistent indication of surgery. Yet, accurate diagnosis is not possible on the basis of such examination alone.

[A method for testing the resistance of film-forming polymers against gastric juices (author's transl)].

The authors describe a simple method for preparing and testing isolated polymer-based films, and demonstrate their use by the example of selected polymethacrylate films, mainly containing different concentrations of various plastizisers. The results obtained show that the characterization of film properties by this method may be used for the comparison of filmogenic formulae in the forefield of coating and for prognosticating the film quality to be expected.

[Characterization of storage-induced release reductions in suppositories (author's transl)].

It is reported of the effect of storage at different temperatures on the release of aminophenazone, codeine phosphate, phenobarbital sodium and trapidil from suppositories, and of the physical parameter of the latter. Depending on the state of division of the drugs in the preparations and on the conditions of storage, drug release from suppositories was more or less affected, which alterations were but incompletely reflected in the physical parameters of the suppositories.

[Comparison between urodynamic findings obtained prior to and during treatment of women for urinary urge incontinence (author's transl)].

Described in this study are urodynamic findings obtained prior to and during treatment of women for urinary urge incontinence. Cystometry was found to be highly suitable for diagnosis of this form of incontinence. - The same methods should be re-used for verification of the findings and for comparable results. The findings obtained are discussed together with information derived from literature.

Research on pharmacokinetics during pregnancy.

The pharmacokinetics of some drugs important for treatment in pregnancy were studied in several groups, women in stage III of pregnancy, women in labour and non-pregnant women as a control group. The half-life of elimination of the drugs is one of the most important parameters in pharmacokinetics. The following drugs were used: noramidopyrine methane sulphonate sodium, aminophenyzone, pethidine, methaqualone, ampicillin, cephalothin, chlorsulfamethin, and indocyanine green. The results show that all drugs except cephalothin had a prolonged half-life during labour. In contrast to this the half-life of most of the drugs is diminished in pregnancy. The extent and direction of the changes depend on the pathway of elimination and on the rate of biotransformation of the drug.

[The value of radio-isotope investigations of the urinary tract for the diagnosis of urinary tract calculi during pregnancy (author's transl)].

According to the literature, urinary tract calculi are a rare complication of pregnancy. Because of the radiation effect of excretory urography on mother and fetus, intravenous pyelography can only rarely be used for the diagnosis of calculi in pregnancy. A sequential radio-isotope nephrogram and the simultaneous determination of the J 131 hippurate clearance equivalents were used in two pregnant patients. The cases are reported. The results are discussed and compared to reports in the literature. The exposure to radiation is about 50 mrem for radio-isotope nephrography compared to from 500 to 4000 mrem for excretory urography. Radio-isotope nephrography is therefore useful for the preliminary diagnosis of urinary tract calculi in pregnancy and the simultaneous clearance determination yields additional data on the function of the urinary tract.

A notable Aspergillus from a mortal aspergilloma of the lung. New aspects of the epidemiology, serodiagnosis and taxonomy of Aspergillus fumigatus.

Different basic mycological observations were made in relation to a fatal case of aspergilloma of the lung in a 45-year-old male. Cultural examination of the sputum and autopsy material from the lung revealed the predominant presence of a white fungus along with a scarce growth of the typical Aspergillus fumigatus. The white isolate produced conidial heads almost similar to that of A. fumigatus but with apparent septation in the phialides. With the help of the immunodiffusion test and use of the antigens from the typical A. fumigatus strain and from the white isolate, antibodies could be detected in the serum of this patient and an antigenetic relationship between these fungi could be observed. It is assumed that this white isolate is a variant of A. fumigatus and its presence in the lung is an indication of chronic or progressed aspergillosis caused by A. fumigatus. Cultural characteristics and developmental morphology of this fungus and the serodiagnostic value of the antigen derived from it will be described in detail in separate communications.

[Clinical assessment of renal function in oedemas during pregnancy (author's transl)].

Oedemas during pregnancy make differential diagnosis imperative, since it will be necessary to determine whether they occur primarily in connection with pregnancy or are secondary to a pre-existent renal disease. In the studies presented here were examined women without known pre-existing renal disease, to determine the influence exercised on the parameters of renal function by the oedemas occurring during pregnancy. Examination was effected weekly by means of the previously described programme for risk pregnancies during the individual stages of pregnancy. No statistically significant influence on ureterorenal function was seen in these women compared with groups of controls. The results are discussed on the basis of facts available from literature.

[On the action of various factors on the liberation from suppositories (author's transl)].

The authors deal with the effects of five suppository bases on the liberation of aminophenazone, codeine phosphate, phenobarbital natrium and trapidil from suppositories, with regard to the physical properties (melting time, melting range, melting point, viscosity) and the chemical properties (acid number, hydroxyl value, composition of the bases) of the bases and the suppositories as well as of the drugs used (solubility, particle size). The grouping of the causative factors indicated, which must be weighted differently, leads to the characterization of the mechanisms of liberation and to the interpretation of the considerable differences in liberation.

[The pharmacokinetics of cephalothin during the late stage of pregnancy and in the course of labour (author's transl)].

Using an open one- and two-compartment model, the authors studied the kinetics of cephalothin during pregnancy and in the course of labour as well as in a group of non-pregnant women. They found that the parenteral application of cephalothin leads to therapeutic levels in the serum, tissues and urine also during pregnancy and in the course of labour. The bearing of pregnancy and labour on the kinetics of cephalothin is but insignificant; consequently, no modification of the pharmacotherapy will be necessary.

[Granulometric determination of the effect of polyvinylpyrrolidone on the dissolution rates of difficultyly soluble drugs (author's transl)].

The authors investigated the usability of the TUR ZG 2 gramulometer for determining the dissolution rates of difficultly soluble drugs (prednisolone, nitrofurantoin, nitrofural), especially under the influence of polyvinylpyrrolidone (PVP K 25 and PVP K 90), and for characterizing the changes in dispersity of PVP suspension gels. The dissolution gels of all three drugs were considerably increased by PVP (1--10%). Principally, PVP suspension gels of these drugs underwent changes in dispersity during storage (20 weeks), the particle numbers of prednisolone preparations being doubled. The particle numbers of the nitrofurantoin and nitrofural preparations decreased continuously, beginning with the second week of storage, which may be attributed to an improvement in solubility due to the use of the macromolecular adjuvant. PVP K 90 improved the solubilities of the drugs to a greater extent than PVP K 25.

[On the dropping behaviour of drug solutions (author's transl)].

On testing a preferentially inserted dropping device (marginal-dispensing dropper) of high-pressure process-polyethylene, the authors stated irregular dropping which impaired the dosing of ethanolic-aqueous drug solutions. The addition of macromolecular viscosity-increasing agents (0.2 or 0.3% of hydroxyethylcellulose and 0.5% of methyl cellulose being very suited) permitted to obtain a sequence of uniform easy-to-count drops and to reduce the amount of variation in the individual drop masses. Manufacturing differences among the dropping devices are a substantial source of error which might be reduced by fully automatic control. An improvement in the dropping behaviour of the dropping device under investigation can be expected from suggestions for changes in its design. The use of a "general purpose dropping device" is inconsistent with the specific properties of the drug preparations. Dosing accuracy is higher with central-dispensing droppers than with marginal-dispensing ones.
