Posterior flank approach to the peritoneum for ventriculoperitoneal shunting.

We report a technique for insertion of a distal shunt catheter into the peritoneal cavity through a posterior incision. This has been helpful in patients with impaired access anteriorly. It may also be useful in elective situations. We have performed this procedure on 3 infants where the traditional anterior approaches would have been difficult due to subcutaneous scar tissue, ostomy sites and anterior intra-abdominal adhesions. This technique was safe and provided easy access to the peritoneal cavity. Complications would be similar to those for the standard ventriculoperitoneal shunts including bowel perforation, damage to the kidney or liver, infection and occasionally posterior cervical webbing that may cause pain locally or pull the head into an extended position.

Spinal stenosis: a review of 23 cases.

Twenty-three cases of spinal stenosis suspected clinically and confirmed by computed tomography (CT) scanning are reviewed. The usually prolonged delay from the onset of symptoms to the time of diagnosis can be reduced by an awareness of the frequently bizarre, but usually bilateral clinical features. Confirmation of stenosis has required myelography, but CT scanning frequently may confirm the diagnosis without myelography and, being noninvasive, may facilitate earlier and more appropriate treatment.

Alpha-1 antitrypsin protease inhibitor typing in immobilized pH gradients.

Alpha-1 antitrypsin deficiency is a cause of liver disease in neonates and emphysema in adults. Protein phenotypes are identified by isoelectric focusing using polyacrylamide gels. The Pharmacia Phastsystem was utilized for electrophoresis in miniature gels to identify heterozygotes for the deficiency. Protein phenotypes were identified by isoelectric focusing in a fixed pH gradient from 4.3 to 5 using the Pharmacia Phastsystem for automated electrophoresis and staining of gels. The gradient is formed with Immobilines to create gels of dimensions 50 x 43 x 0.5 mm. The processing time for 16 specimens is one hour and 45 minutes. This method is a rapid, automated method for the analysis of alpha-1 antitrypsin phenotypes and for establishing the diagnosis of a genetic deficiency of this protein.
