The rate of secretion of dopamine (DA) was investigated along with those of norepinephrine (NE) and epinephrine (E) in the case of hemorrhagic hypotension. Blood samples were collected directly from the adrenal vein and DA, NE and E contents were measured using a gas-liquid chromatograph equipped with an electron-capture detector (GLC-ECD). Under the condition of hemorrhagic hypotension, the rates of secretion of DA, NE and E increased from 0.22, 3.4 and 13.7 ng/kg/min to 10.7, 89.7 and 361.4 ng/kg/min in 90 min, respectively. After reinfusion, the levels of DA, NE and E decreased. The concentration of DA in the femoral artery was 1.0 and 1.5 ng/ml at 70 and 90 min after hemorrhage. In spinal transected preparations, increases in the rates of DA, NE and E secretion did not occur during hemorrhagic hypotension. Thus, the rates of secretion of DA, NE and E varies with the hemodynamic changes, the secretion of DA from the adrenal gland is primarily controlled by the central nervous system, and the pattern of increase in level of DA differs from the patterns in the case of NE and E in hemorrhagic hypotension.
From 1965 to 1980, reoperations for residual or recurrent stones were performed on 78 out of 962 Japanese patients with cholelithiasis. The majority of patients who required reoperation had intrahepatic stones. Most of the causes of reoperation were residual stones due to incomplete removal or the non-detection of intrahepatic stones at the previous surgery. Very careful examination of the intrahepatic biliary trees should be done in patients with biliary tract diseases, because in many, the first operation was done during their youth. To remove the intrahepatic calculi completely, hepatic lobectomy should be considered as a final procedure. The causes of reoperation of common duct stones were residual in 60 per cent and recurrent in 40 per cent. Definitive surgery should be done at the first or at least the second operation to avoid irreversible hepatic disorders which have untoward effects on the prognosis. It is important not only to remove the stones but also to relieve the bile stasis in the biliary tract.
The patient, a 53-year-old man, was admitted to the Department of Neurosurgery, Nagasaki University Hospital in August 1979, because of a painful tumor in the lt-parieto-occipital region. Physical examination on admission revealed a fist-sized tumor in the lt-parieto-occipital region. The liver edge was palpable at two finger breadths beneath the right costal margin, but the liver itself was not palpable as a mass. Liver function was almost good except for GOT and ChE. Plain craniography showed an osteolytic change of about 10 cm X 10 cm in size without marginal hyperostosis and spicular formation. Bilateral external carotid angiography demonstrated a hypervascular mass, which was fed by the bilateral superficial temporal arteries and occipital arteries. CT scan also demonstrated a fist-sized tumor originating from the diploë with destruction of the skull bone. Tumor was strongly enhanced. A radical removal of tumor was performed. Tumor was solid with profuse bleeding and adhered tightly to the dura and the skin. The postoperative course was uneventful and postoperative CT scan demonstrated complete removal of the tumor. Histological diagnosis was hepatocellular carcinoma and the skull tumor was considered to be metastatic. CT scan of the liver showed a circumscribed low density area with ring enhancement at the posterior ventral site. As no metastasis was found in other organ except the skull, so primary lesion was radically removed at the Second Department of Surgery. The incidence of metastasis to the bone in hepatocellular carcinoma in autopsy studies has been considered rarely approximately from 4 to 14%. Moreover, the common sites of skeletal metastasis are the vertebral column, the ribs, the long bones and so on. Metastases to the skull bone was exceptionally rare and all cases hitherto reported had multiple metastases in other organ(s) and no literature was found about the distant metastasis to the skull bone only.

A novel convenient procedure was introduced for enzyme labelling of neocarzinostatin (NCS) with beta-D-galactosidase using a new hetero-bifunctional reagent N-(gamma-maleimidobutyryloxy) succinimide (GMBS). With the enzyme labelled NCS and a rabbit anti-NCS serum which was elicited in a rabbit immunized with NCS in complete Freund's adjuvant, a highly sensitive enzyme immunoassay which can quantify a femto mol order NCS was developed. Accuracy and precision of the assay as well as a comparison with the immunodiffussion method were studied with satisfactory results. This enzyme immunoassay of NCS was applicable to monitoring serum NCS levels of patients in clinical treatment of the antitumor agent.

A case of mediastinal thoracic duct cyst is described. It is believed to be the first to be diagnosed before thoracotomy. Seven surgically treated cases have been reported but in none was the diagnosis made before operation. This case report concerns a 49-year-old woman who was operated upon because of a thoracic duct cyst that had been diagnosed by lymphangiography before the operation. Symptoms were caused by pressure of the cyst on the esophagus. The operation and immediate postoperative course were uneventful. One year later, the patient was doing well and no further investigation was recommended.
In order to evaluate the efficacy and safety of FOY injectable ([ethyl-4-(6-guanidinohexanoyloxy) benzoate] methane sulfonate) on acute pancreatitis, a comparative clinical study was carried out using Trasylol as the control at 38 hospitals in Japan. Favourable results were obtained in 60 (71%) out of 84 patients in the FOY group and 29 (44%) out of 66 patients in the Trasylol group. The results showed that these both drugs were effective and the statistical analysis revealed considerable difference (X2 = 10.464, p less than 0.005) between the two groups in this condition of clinical trial. In addition, a double blind trial was carried out at 4 hospitals using FOY-305 ([n.n-dimethylcarbamoylmethy 4-(4-guanidinobenzoyloxy)-phenylacetate] methane sulfonate) oral capsule and inactive placebo. Favourable results were obtained in 18 (69%) out of 26 patients in the FOY-305 group and 8 (32%) out of 25 patients in the inactive placebo group, indicating a significant difference between the two groups (X2 = 8.930, p less than 0.01). The results of the present study suggest that synthetic protease inhibitor, FOY or FOY-305, is beneficial in the treatment of acute pancreatitis.
