A new method for separate lung ventilation.

Separate lung ventilation is obtained with selective intubation of the main bronchus by an appropriate cuffed tube inserted through a standard orotracheal tube. Ventilation is carried out separately through the bronchial tube on one side and the residual tracheal tube lumen on the other side. This method, used in 144 patients, greatly simplifies the technique of bronchial intubation and offers many advantages over commercially available double-lumen tubes.

Study on pulmonary, prostatic and renal (medulla and cortex) distribution of sagamicin at different time intervals.

In order to assess the therapeutic activity of an antibiotic, not only its serum kinetics but also its kinetics in the peripheral tissues must be determined, thus evaluating the specific power of penetration of the drug. Sagamicin is an aminoglycosidic basic antibiotic closely related to gentamicin and its penetration rate into lungs, kidney and prostate, together with its serum concentrations time-course, were investigated. The findings obtained showed that this antibiotic attained a good distribution in the peripheral tissues, where it easily reached therapeutic levels.

Ciprofloxacin: multiple-dose pharmacokinetic and clinical results in patients with hypercrinic bronchopulmonary diseases.

Ciprofloxacin, a 4-quinolone derivative with a wider spectrum of activity as compared to classic quinolones employed in the therapy of urinary tract infections, was studied in view of its possible application in the therapy of bronchopulmonary infections. An oral dose of 500 mg every 12 h was administered and both the clinical response and the pharmacokinetic profile were investigated. A complete recovery was reached in 87.5% of patients and an improvement in 12.5%; no failure was recorded. A very good penetration in sputum was confirmed by the sputum/serum area under curve ratio, providing evidence for a high bioavailability in bronchial secretion. Lung tissue concentrations confirmed the good peripheral distribution of ciprofloxacin. A twelve-hour administration schedule allows high peripheral concentrations to be obtained superior or equal to the minimum inhibitory concentrations for many pathogens.

A new method for one lung anaesthesia in thoracic surgery is described. [Total parenteral nutrition and dialysis. Complementary importance in the therapy of acute renal insufficiency in aortic surgery].

The acute renal insufficiency (A.R.I.) consequent on surgical pathology of the aorta is in the Milan School, one of the most important postoperative complications and requires the use of total parenteral feeding (T.P.F.). Parenteral infusion of AAe and hypertonic glucose in patients with A.R.I. has given positive results, not only insofar as it improves the general nutritional state, but also because it facilitates recovery of renal function and improves survival. Reutilisation of endogenous nitrogen gives a synthesis of structural proteins to the benefit of metabolic homoeostasis and the patient's clinical condition. On the basis of recent nephrology studies on uraemic toxicity, the therapeutic problem of A.R.I. in surgical patients has been examined: early peritoneal dialysis associated with T.P.F. and the combination, in the postoperative stage, of parenteral feeding and periodic peritoneal dialysis. The average duration of this treatment has been personally found to be about 8-18 days. The example is given of a clinical case of A.R.I. in a patient operated by aneurysmectomy for rupture of an aneurysm of the abdominal aorta.

[The use of methylprednisolone in the therapy of renal transplants from cadavers].

The role of steroids, and specifically of methylprednisolone, in the pharmacological treatment of cadaver kidney transplants is discussed. The action mechanisms of the substance, and the modalities for its use in basic treatment of the transplanted organ and in rejection crisis are described. Finally, the results obtained in 147 transplanted patients, the complications involved and the measures taken to eliminate, or at least reduce, the problems of prolonged steroid therapy are reviewed.

[Therapy of shock].

Shock therapy is a problem which readily leads to considerations of a practical nature. Routine clinical experience is the basis for all evaluations of results. Drugs, doses, therapeutical tactics are of value only in so far as experience has successfully made use of them.

[Intensive care units in surgery].

The reasons why intensive care units have become essential are examined. The purpose of such units is to modify the organization of hospital assistance according to the concept of progressive treatment in line with the gravity of the disease concerned. Monitoring techniques as a function of the patient's gravity are outlined and the use of electronic processing as a valuable contribution to the surgeon in providing him with a complete and speedy physiological profile of the seriously ill patient is recommended. Finally, the cost of intensive care units is examined and to keep it down careful attention must be paid to the type of pathology admitted to such units. In spite of this by no means negligible factor, institution of such units is still recommended as quickly as possible, their organization being moulded to the situation in question.
