[Iridium 192 interstitial radiotherapy of carcinoma of the penis. Carcinologic results, dermatologic evaluation (about 87 cases) (author's transl)].

87 cases of epidermoid carcinoma of the penis treated by Ir 192 interstitial radiotherapy are studied. The 5 years survival rate, estimated on a 51 patients group, is 61 p. 100. Among these 51 patients, 12 had a local recurrence. The post-radiotherapy early reaction is estimated on a 79 patients group, with a mean duration of 3, 4 months. The early reaction is correlated to tumor size and not correlated to number of radio-active lines. The later dermatologic evolution is studied in 59 patients. In 22 cases, the scar remained supple, without any further local change. Local changes occurring in the 37 other cases are studied in their clinical aspect, signification and frequency. Of 34 patients followed longer than 40 months, 14 had local complications with 4 cases of local recurrence. The carcinologic reliability of this method is similar to that by surgery. It seems however it must be restricted to tumors limited to glans penis.

[Current persistance of pleural empyema. Study of 31 observations (author's transl)].

Antibiotic treatment of pleuro-pulmonary infectious diseases with ordinary germs brought a notable decrease in the frequency of pleural empyema without preventing them completely. The authors report 31 cases observed over the last 10 years. The initial signs corresponded to the classical picture but were rapidly masked by antibiotic treatment; it resulted in delay in hospital admission of over one month for more than half of the cases, for diagnosis was overlooked for a long time. In 19 patients, the pleural liquid was aseptic probably because of the antibiotics and of the lack of identification of anaerobic germs. The authors insist on the practical modalities of a medical treatment by puncture-lavage which, when associated with general antibiotic treatment, cured patients in 22 cases. On the other hand, the existence of bronchopleural fistula entailed surgery.

[Cutaneous psoriasis. Symptoms and therapy].

Skin psoriasis, a multi- causal disease with unknown mechanism, has several clinical aspects. Besides ordinary psoriasis with mica-like plaques and its special locations, two less known notions are underlined: the localized forms are often less typical (for example: scalp, ears, folds of the hand, foot and nails). They may be relevant to the rhumatologist for the diagnosis the inflammatory forms are often disseminated with pustules and keratosis and constitute another groups where the rhumatologic manifestations are more frequent and more severe. Various forms of treatment are active on the cutaneous symptoms, chronic or relapsing topical therapy with tar, corticoids, nitrogen mustard, photochemytherapy, oral derivatives of retinoic acid, and exceptionally immunosuppressors. Each technic has its advantages, its disadvantages on a short or long term. Therefore each patient should receive the therapy which is adapted to his form of psoriasis, the need and possibilities of the patient. Collaboration is a must and each patient should receive the adequate care making possible an improvement of his condition.

[Woringer and Kolopp's disease (author's transl)].

Woringer and Kolopp's disease is a rare skin disease. Erythemato-squamous, slightly infiltrated lesions with round-shaped islets of normal skin are the clinical hall-marks of the disease. The histological picture is highly specific: a dense epidermal infiltrate disrupts the stratum spinosum, contrasting with a spared dermis. Three different aspects of the disease have been reported: a localized benign type as in Woringer and Kolopp's patient; a disseminated lethal type without visceral involvement and without preexisting lesions, and a disseminated lethal type in which lesions appear on erythematous patches clinically and histologically similar to "parapsoriasis en plaques". It is the opinion of the authors that the third type is not Woringer and Kolopp's disease but an extremely epidermotropic variant of mycosis fungoides. The two other types seem to be variants of the same disease. Several hypotheses about the nature of that disorder are discussed. Most authors think that Woringer and Kolopp's disease is a cutaneous lymphoma because of clinical, histological and ultrastructural similarities with mycosis fungoides. We are presenting evidence favoring the hypothesis of a Merkel cell proliferative disease.

[Extensive apical pulmonary fibrosis complicating ankylosing spondylitis. 2 cases (author's transl)].

The authors describe two cases of pulmonary fibrosis in patients suffering from ankylosing spondylitis. The diagnosis of tuberculosis was rejected on the basis of negative studies for tubercle bacilli and the total ineffectiveness of specific therapy. No evidence of microbial or parasitic infection was found. This fibrosis was associated with a considerable worsening in general condition. Nevertheless, the first patient remains alive 15 years after the onset of this pathological manifestation. The second patient died but autopsy was not possible. A wide review of the literature is undertaken in the light of these two cases.

[Streptococcal gangrene. Importance of surgical treatment].

The authors report five cases of streptococcal gangrene and stress the fundamental role of surgical excision. Parenteral anti-streptococcal antibiotics should not lead to this essential step being forgotten, its early application being the most important element in prognosis. It is thus essential to clearly understand the local and general criteria of rapid diagnosis in this condition, which is far from rare.

[Broncho-pulmonary manifestations during non-fistulized esophageal lesions].

Because of inspiration into the tracheo-bronchial aireays, regurgitation from purely oesophageal diseases can provoke various respiratory affections: acute broncho-pulmonary blocking broncho-pneumonia, pulmonary suppuration, night cough, fits of nocturnal suffocation, chronic bronchitis sometimes hemoptic. A mega oesophagus, a diverticulum, stenosis or oesophagus cancer are at the origin. Oesophago-bronchial fistulae are excluded. The decrease of the glottal guarde remains to be explained because it usually protects the trachea from such mishaps (part played by decubitus, sleep, secretions stagnating in the pharynx).
