Suprachoroidal injection of sodium hyaluronate as an 'internal' buckling procedure.

Various fractions of sodium hyaluronate were injected into the suprachoroidal space of rabbit eyes via the pars plana as 'internal' buckles. The areas of elevated choroid were diffuse, usually extending over approximately one quadrant. The buckling effect was short-lived, in the range of 12-72 h, but sodium hyaluronate actually survived in the suprachoroidal space up to 10-14 days or more. No significant differences between buckles created by sodium hyaluronate 1 and 2%, or by cross-linked sodium hyaluronate were observed. Internal buckling of the choroid may prove to be a viable adjunct or alternative to conventional buckling procedures in the future.

Incidence of squamous cell carcinoma of the head and neck: report of 1,000 cases.

The purpose of this paper is to define the incidence of squamous cell carcinoma (SCC) of the head and neck as seen at the Free University Hospital, Amsterdam, the Netherlands, in the last six years, according to the latest revised classification system of the International Union against Cancer (UICC, 1982). In addition, male to female ratios, relative frequencies at the various sites, age distribution, incidence of second or third primary and the relationship of distant metastases to the nodal metastases and the T-stage of the primary lesion are reported. These incidences are compared with those in other countries and the major differences are discussed.

Lysis of nitrofurantoin-resistant strain of Vibrio el tor.

Both nitrofurantoin-sensitive and nitrofurantoin-resistant strains of Vibrio el tor were found to lyze in the presence of Tris-EDTA at alkaline pH. The rate of lysis was appreciably enhanced by lysozyme. The amounts of intracellular components, viz. proteins and carbohydrates, released from the nitrofurantoin-sensitive strain by Tris-EDTA treatment, were significantly lower than those from the nitrofurantoin-resistant strain. Differences in periplasmic proteins released from Tris-EDTA treated cells of nitrofurantoin-resistant and -sensitive strains were revealed by gel electrophoresis.

Role of myocutaneous flaps in reconstruction of the head and neck.

To otolaryngologists involved in ablative surgery for tumours, restoration of function and appearance after extirpation has always been a matter of concern. This is particularly important for two reasons. In the first place, the function of the pharynx as a pathway for deglutition and respiration makes reconstruction of this area of vital importance. Secondly, while scars, defects and prosthetics in other parts of the body can be easily hidden and camouflaged, this is difficult in the head and neck area. For almost a century skin grafts, free, pedicled or as rotation flaps, have been used for reconstruction. In recent years the development of myocutaneous flaps has considerably altered this picture and this field is rapidly advancing. The purpose of this paper is to acquaint ourselves with this advance and to present our own experience.

Compressive neuropathy as a complication of anticoagulant therapy.

Three cases of compressive neuropathy after anticoagulant therapy are reported. The first known description of ulnar and radial nerve involvement in this syndrome is provided. A review of the literature along with a discussion of some of the clinical features, the diagnosis, and the management of this entity are presented.

Studies on membrane ATPase activity and lipid level in vibrio el tor under normal and nitrofurantoin resistant conditions.

Nitrofurantoin resistance brought about decreased level of membrane bound ATPase activity in Vibrio el tor as compared to the normal strain while phospholipid as well as total lipid levels in the membrane fraction was increased. Mg++ was found to be the mose effective co-factor for ATPase. At higher concentrations of sodium and potassium ions, ATPase of normal and nitrofurantoin resistant Vibrio el tor responded quite differently. A soluble ATPase extract having low phospholipid content was found to be activated by the phospholipid extracts of Vibrio el tor, phosphatidyl ethanolamine, and also by phosphatidyl choline which is not a constituent of the membrane preparation of Vibrio el tor strains.

Studies on the protective effects of L-ascorbic acid in chronic chlordane toxicity.

The influence of extraneous supplementation of L-ascorbic acid in chronic chlordane toxicity has been studied in rats. Oral administration of chlordane brings about a marked growth retardative effect, stimulates vitamin C synthesis in the system, elevates the vitamin C status of the liver and kidney tissues and also the urinary excretion. It inhibits the activities of acid and alkaline phosphatases, SDH and Mg2+-ATPase of both hepatic and renal tissues. The activities of serum and mitochondrial GOT, serum alkaline phosphatase, and glucose-6-phosphatase of both hepatic and renal tissues are markedly stimulated. The normal histological patterns of both liver and kidney tissues are grossly altered under chlordane toxicity condition. There occurs marked increase in the hepatic lipid composition. Supplementation of L-ascorbic acid in high doses to the chlordane treated rats can effectively counteract some of these alterations in respect of enzyme activities, morphological characteristics and of hepatic lipid composition.

Staffieri's procedure revisited.

Our long-term experience with Staffieri's procedure in 42 patients is presented. Our results were evaluated regarding voice production, aspiration problems, and pharyngocutaneous fistula formation. Approximately 50% of the patients were successfully rehabilitated. Twenty-five percent of the patients did not use the neoglottis for speech production, mostly because of stenosis of the neoglottis. The remaining 25% had serious aspiration problems that needed treatment. It was striking that half of these patients initially benefitted from the Staffieri procedure and had no aspiration problems. These 42 patients were compared with a group of 43 patients who underwent conventional total laryngectomy in the same period. After total laryngectomy and Staffieri's procedure, greater than 35% of the patients had a pharyngocutaneous fistula, while after conventional total laryngectomy the fistula rate was less than 20%. The average postoperative stay in the hospital was longer in the Staffieri group. In the long run, the results of this surgical technique do not justify its further use, especially after previous radiotherapy.

Laryngeal reconstruction with free nonvascularized grafts from the buccal mucosa.

PURPOSE: This study was designed to investigate the suitability of nonvascularized healthy buccal mucosal graft in reconstruction of subglottic defects. PATIENTS AND METHODS: The study was conducted over an 11-year period from 1985 to 1995. Seven nonvascularized healthy buccal mucosal grafts were used to provide coverage for subglottic defects. The indications for surgery included chondrosarcoma of the cricoid including two recurrent tumors and one patient with recurrent subglottic benign laryngeal stenosis. No stenting was used. Period of follow-up ranged from 1 to 10 years. RESULTS: Graft take was successful in all cases without complications. CONCLUSION: Healthy nonvascularized buccal mucosal graft is suitable for reconstruction of subglottic defects. Stenting is not required.

Knowledge, attitudes and beliefs about menarche of adolescent girls in Anand district, Gujarat.

A questionnaire survey of knowledge, attitudes and beliefs towards menstruation was made in 22 schools in Anand district, Gujarat state. Of 900 schoolgirls aged 11-17 years, only 38.5% felt comfortable about menarche and only 31.0% believed that menstruation was a normal physiological process. Many (37.2%) had not been informed about menarche before its onset and 48.2% felt they were not mentally prepared. The major sources of information were the mother (60.7%) or an elder sister (15.8%); teachers and others relatives played a small role. In this area of India, many families continue the custom of celebrating the first menarche and observing social restrictions.

Blue field entoptoscopy in diabetic retinopathy.

Blue field entoptoscopy was performed in 50 consecutive patients with diabetic retinopathy. Several statistically significant correlations were obtained. The number of "flying corpuscles" was reduced in eyes harboring a vitreous hemorrhage, and in severe and proliferative diabetic retinopathy. On the other hand, patients with a history of focal or panretinal photocoagulation had better corpuscle counts than patients who had never received any treatment. It appears therefore, that argon laser photocoagulation is beneficial in preserving the blue field entoptic phenomenon.
