Ketamine dissociative anesthesia--emergency department use in children.

An on-going study to determine the efficacy and safety of ketamine dissociative anesthesia in the emergency department for children less than 10 years of age is presented. Preliminary results of the first 30 cases indicate that this technique may be safe and convenient if used within the appropriate protocol.

Gonadal dysgenesis with vulvar carcinoma.

An unusual case of vulvar carcinoma in a gonadal dysgenesis patient is described. In addition to clinical manifestations, certain laboratory data related to endocrine status were gathered from serum and urine analyses. Finally, receptors for estrogens, progestins, and glucocorticoids were determined on biopsies of the vulvar carcinoma.

Municipal solid wastes and their disposal.

A brief overview is given of the sources, characteristics, and toxic constituents of municipal solid wastes. Several methods are presented for handling, treating, and disposal of solid wastes. Monitoring the landfill site is necessary; there has been a trend to recognize that municipal solid wastes may be hazardous and to provide separate secure handling, treatment, and disposal for their dangerous constituents. Under current state and Federal regulations, permits are being required to assure that proper handling of conventional solid wastes and more hazardous constituents are carefully managed.

Carcinoma of the gastroesophageal junction. A ten year experience with esophagogastrectomy.

Eighty-six patients with carcinoma of the gastroesophageal junction were treated between 1966 and 1976. Adenocarcinoma was present in 62 patients and squamous cell carcinoma in 24. Surgical exploration was performed on 68 patients (79%) and 52 lesions (60%) were resected by combined midline laparotomy and right thoracotomy. There were 6 deaths (11%), 2 due to anastomotic leak. The mean survival for all resected patients was 2.1 years with a 22% three year life table survival (10/52). Five of 14 patients with negative nodes are alive more than five years. Palliative resection in 17 patients resulted in a mean survival of 10.5 months and no five year survivals. Esophagogastrectomy utilizing the Lewis technic reduces operative mortality and extends survival for patients with carcinoma of the gastroesophageal junction.

Visual recovery after central retinal artery occlusion: two cases.

Two cases of central retinal artery occlusion of prolonged duration (greater than 6 hours) were treated with retrobulbar anesthesia, anterior chamber paracentesis, and high-dose carbon dioxide and oxygen inhalation. Both cases recovered excellent vision and illustrate that visual recovery after relatively prolonged central retinal artery occlusion is possible.

Cell-free synthesis of a large translation product of prolactin messenger RNA.

RNA prepared from rat anterior pituitaries or from prolactin-secreting pituitary tumors has been shown to direct the synthesis of a large form of prolactin in a cell-free system derived from wheat germ. Immunoprecipitation of cell-free reactions demonstrated the synthesis of a product which was recognized by a specific antiprolactin antisera. Analysis of the immunoprecipitate on sodium dodecyl sulfate containing polyacrylamide gels suggested that the cell-free product has a molecular weight of approximately 28,000 compared to 22,500 for prolactin. RNA prepared by completely different techniques from rat pituitary and a pituitary tumor resulted in identical large translation products. Translation of tumor RNA in a cell-free system from Krebs ascites cells also resulted in a similar large product. The identity of the cell-free product as prolactin was confirmed by comparing peptides derived from the cell-free product and prolactin. The results of these studies suggest that prolactin messenger RNA directs the cell-free synthesis of a product which contains the amino acid sequence of prolactin but which has an addition at one or both ends of the molecule.

Familial colon cancer.

Study was made of a kindred with a strong history of carcinoma of the colon. Twenty-three family members were screened for the tumor with carcinoembryonic antigen (CEA) assay, barium enema, and proctoscopy; one occult colon cancer was diagnosed. Identification and surveillance of families at high colon cancer was diagnosed. Identification and surveillance of families at high risk of cancer can provide unusual opportunities for early tumor detection.

Biliary elimination of diazepam in man.

The metabolism of 14C-5-diazepam has been studied in 5 patients with T tube biliary drainage. A single bolus of 40 to 50 muCi was given intravenously and blood, urine, and bile were analyzed from 5 to 14 days. The mean half-life of elimination from blood was 93.2 hr; the major metabolite noted in blood was N-desmethyl-diazepam. In urine the average recovery of radioactivity was 48.9% and consisted of 3 OH-diazepam, 4'OH-diazepam, and oxazepam. In bile the average recovery of radioactivity was 5.35% (corrected to a bile flow of 700 ml was 15.0%) and consisted of the same metabolites as in the urine. Essentially no diazepam or N-desmethyl-diazepam was found, and therefore an enterohepatic circulation cannot be held to account for the prolonged half-life of these substances in man.

Survivors of cancer in childhood.

Late effects of childhood cancer in 142 patients, presently aged 18 years and older, were examined with a medical record review and questionnaire survey. Seventy-four patients (52%) had major defects in treated organs, while 17 (12%) developed second primary neoplasms (10 malignancies and 7 benign tumors). Despite these disorders, most patients had fully active lives. Eighty-six (61%) had attended college, and many were employed as professionals or managers. Seventy-five (53%) were married; 46 had progeny. Disabling psychological illness attributed to cancer in childhood was uncommon. These findings indicate the potential for high quality of extended life for most survivors of childhood cancer. Comprehensive long-term care of these patients may further reduce adverse physiologic and psychosocial sequelae.

Lesch-Nyhan syndrome: evidence for abnormal adrenergic function.

Subjects with the Lesch-Nyhan syndrome (hypoxanthine-guanine phosphoribosyltransferase deficiency with self-mutilation) exhibit an apparently unique pattern of adrenergic dysfunction characterized by elevated plasma dopamine beta-hydroxylase activity and an absence of pressor response to acute sympathetic stimulation. Patients with a partial deficiency of hypoxanthine-guanine phosphoribosyltransferase without self-mutilation do not exhibit these abnormalities of adrenergic function.
