The effect of pipemidic acid and five new synthetic antibacterial agents--norfloxacin, enoxacin, ofloxacin, ciprofloxacin, and pefloxacin--on the serum level of theophylline was studied in healthy male adult volunteers after concomitant oral administration of these agents with a slow release preparation of theophylline. The results indicated that enoxacin, ciprofloxacin, and pipemidic acid might decrease the clearance of theophylline in the liver, and the attention should be paid in clinical use when enoxacin or pipemidic acid is coadministered with theophylline.
An open clinical study of ofloxacin in respiratory tract infections was conducted with patients receiving daily doses of ofloxacin 300 mg, 400 mg or 600 mg. The duration of treatment was 6 to 14 days for 70% of the patients. Ofloxacin was effective in 668 of 828 patients analysed (80.7%). Of 293 patients with upper respiratory infections, the efficacy rate was 85.3%. In 535 cases with lower respiratory infections, ofloxacin was effective in 78.1%. It is noteworthy that a 70% efficacy rate was obtained in 80 cases with intractable chronic diffuse panbronchiolitis primarily associated with Pseudomonas aeruginosa. There was no difference in the efficacy rate among various daily doses or severity of infections. In lower respiratory infections the bacterial eradication rate was 80.9% for Gram-positive aerobes (including 80% for Staphylococcus aureus and 76.5% for Streptococcus pneumoniae) and 72.1% for Gram-negative aerobes (including 92.6% for Klebsiella pneumoniae, 32.3% for P. aeruginosa and 97.1% for Haemophilus influenzae). Although there were no serious cases, adverse reactions were noted in 46 of 843 patients (5.5%): 38 cases (4.5%) of gastrointestinal tract reactions (nausea, vomiting, heartburn, etc.), 4 cases (0.5%) of hypersensitivity (e.g. eruption) and 19 (2.3%) of central nervous system effects (e.g. dizziness). Abnormal changes in laboratory findings included elevations of AST (1.2%) and ALT (1.5%) and an increase in the eosinophil count (1.7%).
An alteration in the serum elastase 1 level in a previously non-diabetic patient, who unfortunately developed the hyperosmolar hyperglycemic non-ketotic syndrome (HHNS), was observed after intravenous hyperalimentation for 6 days. The patient underwent the therapy because of the occurrence of severe persistent anorexia which appeared as a side effect of treatment of lung cancer with combined anticancer drugs. In parallel with progressive dehydration, levels of serum elastase 1 and urine glucose became greatly elevated at an earlier stage of HHNS. A slight increase in serum alpha 1-antitrypsin was observed. However, there were no significant changes in serum amylase activity and serum alpha 2-macroglobulin level before or during HHNS. The elevation of the serum elastase 1 level was considered to be due to serum electrolyte abnormalities and the defect of serum alpha 2-macro-globulin elevation. Rehydration therapy with half-normal saline solution immediately produced negative urine glucose, but the serum elastase 1 level only gradually normalized after improvement of HHNS.
To investigate the involvement of oxygen free radicals and their scavenger systems in the defenses of compromised hosts against pulmonary infections, we determined superoxide anion (SOA) and superoxide dismutase (SOD; EC 1.15.1.1) concentrations in the blood of compromised hosts and noncompromised hosts, with or without pneumonia. In the compromised hosts without pneumonia (compromised controls), SOD concentrations were lower than in noncompromised hosts (healthy controls). However SOA values in compromised controls did not differ statistically from that in healthy controls. Similar changes were observed in noncompromised hosts with pneumonia. In compromised hosts with pneumonia, SOD concentrations were further decreased by pulmonary infections. By contrast, SOA values were increased in pneumonia. There were, however, no differences in the values for ceruloplasmin among all the groups. The values for alpha 2-macroglobulin and alpha1-antitrypsin were within normal limits in compromised controls but were greater in compromised hosts with pneumonia. These results suggest that a decreased activity concentration of SOD in compromised controls may be partly responsible for the depression of the host's immune defenses.

SM-4300, a newly developed human immunoglobulin for intravenous use, has been studied for the safety and effectiveness/efficacy in the 7 patients with severe respiratory infections and a patient with fever of undetermined origin. Severe infections in the patients considered of 3 cases of pneumonia, 2 of obstructive pneumonia due to lung cancer, 2 of diffuse panbronchiolitis and 1 of fever of undetermined origin. Clinical effects of SM-4300 were excellent in 1 case, good in 1, fair in 2 and poor in 4. The efficacy rate was 25.0%. The appearance of subjective and objective clinical side effects and abnormal laboratory findings related to SM-4300 administration were not noted.
