A clinicopathologic study of atypical lesions of the breast further follow up.

On the assumption that both the lobular and intraductal atypical lesions of the breast are precancerous in nature, a group of 572 such patients have been closely followed during the past 12 years. Thirty-four patients eventually developed cancer during this period. The cumulative incidence of breast cancer was 7.74% at 10 years in the group of patients with atypical lesions only and 16.15% at 10 years in the patients with atypical lesions in one breast and cancer in the contralateral breast. The transformation of this probable precancerous state into cancer apparently takes a long interval of time.

Chronic obstructive pulmonary disease: rehabilitation program with continuation on an outpatient basis.

A total of 187 patients with chronic obstructive pulmonary disease (COPD) were treated in a rehabilitation program, initially as inpatients and then scheduled to continue 6 months or more in an outpatient clinic. The mean age was 61 (range, 28-76 years). Changes shown by participants in the outpatient program were compared to those shown by the nonparticipants. For both groups, the mortality rates were similar to published figures, and were strongly related to the levels of forced expiratory volume/sec (FEV 1.0). The FEV 1.0 declined significantly within one year in both groups. Psychologic test scores were unchanged. There was a sharp increase in unemployment. Although rehabilitative therapy must be continued, high priority should be given to early detection of COPD in patients who have airway obstruction but are otherwise asymptomatic. Possibly, at that stage, the elimination of cigarette smoking may slow the process.

Determination of optimal fluoride concentration in drinking water in an area in India with dental fluorosis.

The present study was made to determine the optimal fluoride concentration in drinking water in a fluorotic zone. The area under study was divided in four parts on the basis of fluorides present in drinking water. A sample of 1320 persons was examinmed, where prevalence of dental fluorosis varied from 3.8% to 65.4% depending on the fluoride concentration in drinking water. Statistically significant increase in the prevalence of dental fluorosis was found (a) with the rise in fluoride in drinking water, (b) with the rise in age, peak being found at 15--19 years age group and (c) amongst the bona fide residents of the area. A significant positive correlation was found between fluoride in drinking water and Community Fluorosis Index. While determining the optimal level of fluoride in drinking water, values obtained were 1.05 parts/10(6), 0.8 parts/10(6) and 0.34 part/10(6), respectively, by different formulae in a fluorotic zone of western Indian.

Synthesis and pharmacological evaluation of some 2-N-alkyl-and -aralkyl-3,4:6,7-dibenzomorphans.

2-N-Alkyl or 2-aralkyl-5,9-dimethyl-3,4:6,7-dibenzomorphans 3a-c were synthesized by condensing 1-alkyl or aralkyl-3,4-dimethylquinolinium iodides 1a-c with benzyl magnesium chloride followed by subsequent cyclization of 2-benzyl-1-alkyl or aralkyl-3,4-dimethylquinolines 2a-c. Their structures were established through elemental and spectral studies. The preliminary pharmacological screening of 3a-c and other 3,4:6,7-dibenzomorphans [16] on mice showed that these compounds have a depressant action on CNS.

Isatin-enzyme interactions. VII. Mechanism of inhibition of rat kidney alkaline phosphatase.

Isatin has been found to inhibit rat kidney alkaline phosphatase (EC 3.1.3.1). The inhibition is dependent on isatin concentration and is of un-competitive type. The hydrolysis of disodium phenyl phosphate by the enzyme at different temperatures (17--37 degrees C) obeys the Arrhenius equation. Energy of activation in the absence and presence of isatin has been found to be 9.84 and 10.24 kCal/mol. The hyperbolic profile of isatin inhibition; the lowering of both Km and Vmax in the presence of isatin, and, small changes in enthalpy, free energy and entropy in the presence of isatin suggest a non-allosteric un-competitive inhibition of the enzyme.

Uptake of marker proteins by glycoprotein-containing cells of the pregnant rat uterus and placenta.

A study was made of cells in the pregnant rat uterus and placenta known to contain glycoprotein inclusions to investigate their ability to endocytose marker proteins (fluorescein conjugated serum and horseradish peroxidase) injected into the maternal circulation. The visceral endoderm showed marked uptake of both proteins, though in later pregnancy this was restricted to the area of yolk sac adjacent to the chorio-allantoic placenta. The intracellular distribution of the endocytosed marker proteins resembled that of the glycoprotein inclusions. In the earlier stages some of the giant cell inclusions contained glycoprotein, some showed staining with the Dunn-Thompson technique for haemoglobin, and some showed peroxidase activity. There was endocytosis of both marker proteins by giant cells, and apparently this occurred independently of ingestion of red blood cells. Uptake by the giant cells persisted to a later stage in the area round the margin of the chorio-allantoic placenta than in the other giant cells. Endocytosis occurred in the labyrinthine trophoblast, and the glycoprotein inclusions found in this situation may represent material being transmitted or digested. The glycoprotein-containing granulated metrial gland cells showed no evidence of endocytotic activity, but there was uptake of both marker proteins by the associated stromal cells.

Nature of inhibition of rat testicular alkaline phosphatase by isatin.

Isatin has been found to inhibit rat testicular alkaline phosphatase (EC 3.1.3.1). That the inhibition is non-competitive as well as non-allosteric is evident from a) the hyperbolic curve relating inhibition as a function of inhibitor concentration; b) the small change in enthalpy, free energy and entropy; c) the number of isatin molecules associating with 1 molecule of the enzyme (n=1.29); and, d) the decrease in the values of both Km and Vmax in the presence of isatin.

Regulatory role of guanosine 3',5'-monophosphate in adrenocorticotropin hormone-induced steroidogenesis.

The relation between steroidogenesis induced by adrenocorticotropic hormone and the concentrations of adenosine 3',5'-monophosphate (cyclic AMP) and guanosine 3',5'-monophosphate (cyclic GMP) was studied at different time intervals in isolated adrenal cells. Submaximal and supramaximal steroidogenic concentrations of the hormone did not cause detectable changes in cyclic AMP during the first 30 minutes, whereas there was an increase in the concentration of cyclic GMP that was accompanied by phosphorylation and steroidogenesis. It is therefore suggested that cyclic GMP, rather than cyclic AMP, is the physiological mediator of adrenocorticotropic hormone-induced adrenal steroidogenesis.

[Isatin-enzyme interactions. VI. Inhibition of rat kidney alkaline phosphatase].

Interaction of isatin with rat kidney alkaline phosphatase has been studied. Mode of attachment of isatin with the enzyme protein is most likely through amino group(s), which is also imperative for catalysis. Sulphydryl group(s) do not seem to be involved in enzyme action. Zinc is also needed for enzyme activity. Use of sulphydryl compounds suggests that isatin inhibition of the enzyme is through attachment at the metal site. However, this inhibition may not only be due to simple chelation of the metal by isatin.
