[The Berlin follow-up study of eating disorders in adolescence. Part 3: Evaluation and prognosis].

This third report from the Berlin follow-up study of eating disorders in adolescence reports findings pertaining to evaluation and prognosis. Based on a systematic analysis of the relevant literature on the course of eating disorders, the following items were analyzed with respect to their value in predicting the psychopathological status of the patient at discharge from inpatient treatment and at follow-up: BMI at admission, age at onset of the disease, duration of symptoms before admission, personality features such as depression and neuroticism, the number of physical complaints, diagnosis (anorexia vs. bulimia), type of eating disorder (purger vs. restrictor), premorbid eating disorders or behavioral abnormalities, socioeconomic status, duration of in-patient treatment, and out-patient psychotherapy following discharge from the hospital. None of the factors predicted psychopathological status at either discharge or long-term follow-up.

[Eating disorders in adolescents in East and West Berlin in the 80's].

Data collected in the 1980's, before the reunification of Germany, and comparing 39 patients in former East Berlin and 60 patients in former West Berlin are presented in this second report on the Berlin longitudinal study of eating disorders in adolescence. The clinical profiles of patients of former East Berlin were found to differ from those of their West Berlin counterparts in the following assessed measures: younger age at disease onset and at admission for inpatient treatment; greater number of siblings; fewer accounts of the clinical symptoms lanugo, bradycardia, hypothermia, constipation; lower rate of premorbid psychopathological features; lower number of psychopathological findings; less frequent sibling rivalry; lower rate of psychopathological attributes among the siblings; lower incidence of maternal mental and physical illness; higher parental educational status; fewer number of complaints, and better sense of well-being.

Correspondence between the clinical assessment of eating-disordered patients and findings derived from questionnaires at follow-up.

Questionnaire scores were compared across three outcome groups in a follow-up of eating-disordered patients with onset of the disease during adolescence. Among a total of 40 patients, 7 continued to suffer from either anorexia or bulimia nervosa. Seven additional patients displayed partial syndromes and 26 had recovered. Both a semantic differential measuring body image and the Eating Attitudes Test (EAT) demonstrated limited evidence of clinical sensitivity by differentiating the three outcome groups. The Eating Disorders Inventory (EDI) did not discriminate the three outcome groups. Obviously, clinical interviews and self-report questionnaires tackle different facets of the eating disorders and, therefore, both should be used in outcome studies.

Outcome in adolescent eating disorders.

A consecutive series of 60 adolescent patients with eating disorders was followed up after a mean period of 58 months. At intermediate follow-up, 4 (6.65) of the former patients were reported deceased, and 6 could either not be traced or refused to cooperate. Follow-up assessment revealed the following diagnostic pattern in 50 patients who were followed up: 5 (10%) of the patients still suffered from anorexia nervosa, 2 (4%) from anorexia nervosa with bulimia, 7 (14%) from partial syndromes of anorexia nervosa, 2 (4%) from partial syndromes of bulimia nervosa, and 34 (68%) had recovered. Questionnaire findings indicated that the improvements that occurred during inpatient treatment were intact at follow-up. No significant findings were revealed by the analysis of a large scale of potential prognostic factors.

Short-term and intermediate-term outcome in adolescent eating disorders.

Within a prospective longitudinal outcome study of the eating disorders with onset during adolescence, 26 former patients were reassessed at short-term (19-38 months) and intermediate-term (48-72 months) follow-up periods. It was shown that the outcome was worse at short-term follow-up. In addition, the changes of diagnostic categories within the spectrum of the eating disorders were studied. It is concluded that outcome studies require repeated assessments to study the various patterns of course of the eating disorders.

[Berlin follow-up study of eating disorders in adolescence. I. Inpatient follow-up].

Following an introductory comment regarding the purpose of this longitudinal study on eating disorders in adolescence, findings based on assessment of the illness course during inpatient treatment are presented. A multidimensional research design that included both self rating and rating by others demonstrates a clear reduction of symptoms in a number of different areas. This is evident in the 'semantic differential' self-evaluation with regard to body-image and personality, the patient's attitudes toward eating, specific characteristics of the eating disorder syndrome, the self-concept, various personality attributes, physical complaints, and the sense of well-being. The analogous assessments made by therapists and ward staff members also indicated definite improvement with regard to those symptoms that are specific to eating disorders.

Application of impediometry to rapid assessment of liquid culture media.

The impedance method provides as unique opportunity to determine microbial activity and kinetics. Since the metabolic processes depend on the nature and quality of the culture medium, impediometry allows the assessment of liquid culture media. Impedance microbiology represents an approach to quantitative microbiology. We investigated the influence of pH, composition and variation of the amounts of industrially made dry media, overheating during the dissolving or sterilisation processes, and qualitative differences between batches of the same culture medium. Using glucose broth as an example, we showed that impediometry allows quantitative, microbial assessment of culture media. Inaccurate preparation of the culture medium could be detected quickly by the use of impediometry. The method is very simple to perform, requires no sample preparation, allows rapid assessment of liquid culture media, and interprets results automatically with the aid of a microcomputer.

[Reference values for spirometry exemplified by FVC and FEV1 parameters--comparison of normal values of the Pathophysiology of Respiration Section and the European Society for Coal and Steel].

Reference values published by the Section Pathophysiology of Breathing in the Society for Pulmology and Tuberculosis of the GDR and reference values of European Community for Coal and Steel (EGKS) were compared on example of FVC and FEV1. It was found a relative well accordance of reference values of FVC and FEV1 in younger and middle-aged men of a height from 160-200 cm (between -5 to +15%). In women with a body height of 150 to 200 cm a well correlation for FVC and FEV1 is also given (-10 to +10%). Reference values are showing a more important difference with increasing age and independent of sex. There are problems concerning the reference values in male and female lower than 155 cm body height.

[The use of impedance measurements in medical microbiology].

The versatility of the impedance method makes it a valuable tool in clinical microbiology. Impedance can be used to estimate total number of bacteria or to determine bacterial susceptibility to antimicrobial agents. The impedance method provides a unique opportunity to determine microbial activity and kinetics. Impedance microbiology represents a different approach to qualitative and quantitative microbiology. The method is very simple to operate, requires no sample preparation, allows rapid detection of bacterial growth, and interprets with microcomputer results automatically.

Carcinoid of the cervix: natural history and implications for therapy.

Carcinoid tumors rarely occur in the cervix. We present a patient with FIGO stage IB disease who developed brain metastases 4 years after initial diagnosis and treatment. The clinical course of a collected series of patients with carcinoid of the cervix is reviewed. Even patients with low stage lesions frequently die of disseminated disease. Adjuvant chemotherapy may have a role in the treatment of such patients.

[Effects of nitrate and ascorbic acid on carcinogenesis in the operated rat stomach].

In the last few years, a great deal of attention has been paid to the hypothetical relationship between high nitrate intake, N-nitroso-compounds and gastric cancer. This long term study deals with the effect of nitrate and ascorbic acid contained in drinking water on the development of cancer in operated stomachs of rats without additional application of a carcinogen (tumor model). A total of 147 F1-Hybrids of WELS/Fohm X BD IX--rats were subjected to stomach operation (Gastroenterostomia antecolica anterior, truncular vagotomy and pyloroplasty). Carcinomas in stomach occurred within 33 weeks (post operationem). The surgical procedures altered the gastric flora, resulting in an increase and predominance of nitrate-reducing bacteria. The maximal tumor rate (10 of 13 rats) occurred in rats with gastroenterostomy and additional nitrate exposition. It may be possible, the nitrate is a precursor of gastric carcinogenesis. But we could not give any significant evidence of this fact. The addition of ascorbic acid to the drinking water lead to a suppression of the development of tumors. Our experience with the tumor model "operated stomach" underline the multifactorial genesis of stomach cancer in rats.

Molecular organization of the maternal effect region of the Shaker complex of Drosophila: characterization of an I(A) channel transcript with homology to vertebrate Na channel.

We have cloned 215-kb DNA containing the maternal effect region (ME) of the Shaker gene complex (shC) at 16F of the Drosophila X chromosome. Five translocation and deletion breakpoints have been mapped on the cloned DNA allowing a correlation of the genetic map to transcription units. The ME region spans 100 kb. The genetic behavior of this region correlates with the occurrence of maternal RNAs in this part of the ShC. Two transcripts have been identified in the vicinity of chromosomal rearrangements which cause a Sh phenotype. These are a 4.5-kb transcript interrupted by T(x;2)B27 and a 2-kb transcript interrupted by T(X;3)Sh and T(X;Y)W32. The latter transcript is derived from a primary transcript which spans >65 kb genomic DNA. The cDNA-sequencing data show that this Shaker (I(A)channel) gene can encode a protein of 35 kd with three alpha-helical membrane-spanning sequences near its carboxyl terminus. These have a striking homology with membrane-spanning sequences of the vertabrate Na channel.

[Helpful adjustment--helpless accommodation. Physicians and the administration of Lower Saxony during the extermination of psychiatric patients at the time of national socialism].

After a brief introduction describing the development and organisation of the National Socialist extermination programmes to eliminate patients suffering from mental diseases, a description is given of the way these programmes were translated into reality in the then province of Hannover. The provincial government implemented these programmes hesitatingly but nevertheless as ordered, whereas the administrative decrees were complied with in a widely different manner by the individual psychiatric hospitals. These differences in the daily routine on-the-spot realisation of the extermination programmes suggest raising the question of professional ethics in psychiatry, as outlined by the author.

[Acute respiratory failure in a medical intensive care unit. Incidence and prognosis].

Acute respiratory failure had occurred in 89 of 1594 patients in a medical intensive care unit (5.6%), 26.8% of all patients (332) on long-term mechanical ventilation. Compared with the other chronically ventilated patients those with acute respiratory failure averaged a lower age, the proportion of women was higher and the duration of ventilation longer. The death rate was significantly higher (78.7% compared with 58.3%). The important prognostic factors included the underlying disease, additional abnormal organ function, severity of pulmonary gas exchange abnormality, and advanced age. If there was septicaemia, peritonitis, liver cirrhosis with bleeding oesophageal varices or polytrauma with acute renal failure the death rate was over 80%; after hypovolaemic shock, pancreatitis or postoperative pulmonary failure it was less than 65%. Patients who had abnormal function of at most one other organ in addition and an inspiratory arterial pO2 difference below 250 mm Hg, measured 12 hours after onset of mechanical ventilation, had a relatively favourable prognosis with a death rate of 33%, while in the other groups of patients it was 86-100%.

Puffing activities and binding of ecdysteroid to polytene chromosomes of Drosophila melanogaster.

Salivary glands of third instar Drosophila melanogaster larvae were incubated in vitro in the presence of 5 x 10(-6) M 20-hydroxy-ecdysone. Steroid hormone was localized on the polytene chromosomes of the salivary gland by a combination of photoaffinity-labeling and indirect immunofluorescence microscopy. Steroid hormone binding to chromosomal loci and their puffing activity was correlated for the larval/prepupal puffing cycle characterized by puff stages 1-10. In general, there was a good correlation between the sequential and temporal puffing activity induced by 20-hydroxy-ecdysone and the binding of ecdysteroid hormone to these puffs. Ecdysteroid hormone was detected at intermolt, and at early and late puffs with two notable exceptions. Ecdysteroid was not detected at the two well-studied puffs at 23E and at 25AC, the former being an early puff, which is activated in the presence of 20-hydroxy-ecdysone, and the latter being an intermolt puff, which regresses more rapidly in the presence of hormone. Ecdysteroid hormone was present at puffs as long as the respective puff was active. Also, it apparently accumulated at late puff sites after induction. Since ecdysteroid binding to chromosomal loci is temporal as well as sequential during the larval/prepupal puffing cycle, additional factors besides steroid hormone are necessary for sequentially regulating puffing and concomitant gene activity during development from larvae to prepupae.

The actin gene in yeast Saccharomyces cerevisiae: 5' and 3' end mapping, flanking and putative regulatory sequences.

The 5' and 3' flanking regions of the yeast actin gene have been sequenced and the ends of the actin mRNA were determined by the single-strand nuclease mapping procedure. The mRNA starts with a pyrimidine residue 141 (or 140) nucleotides upstream from the initiation codon. The actin gene lacks a typical "TATA" box 30 base pairs upstream from the mRNA start site but it contains a region homologous to the canonical sequence 5'-GGCTCAATCT-3' which is found in several eukaryotic genes 70 to 80 bp upstream from the mRNA cap site. Judging from the S1 nuclease mapping, there are two populations of actin mRNA terminating 98 and 107 nucleotides downstream from the stop codon. The 3' termini are preceded by three AATAAA sequences found in most eukaryotic polyadenylated mRNAs.
