Comparison of Argatroban and Hirudin for the Reperfusion of Thrombotic Arterial Occlusion by Tissue Plasminogen Activator.

Despite theoretical advantages of direct thrombin inhibitors, recent clinical studies failed to show the superiority of hirudin over heparin in patients with acute coronary syndromes. However, these inhibitors have important in vitro differences for the inhibition of clot-bound thrombin that may translate into different in vivo relative efficacy. The effects of two direct thrombin inhibitors, argatroban and hirudin, on the reperfusion of thrombotic arterial occlusion by t-PA were compared. In anesthetized rabbits thrombotic occlusion was induced in the femoral artery. t-PA, aspirin, and various doses of argatroban (1.25, 2.5, and 5.0 mg/kg/h) or hirudin (2.5 and 5.0 mg/kg/h) were administered (six animals in each group). Blood flow was measured for 4 hours. Animals treated with 2.5 mg argatroban more rapidly achieved full reperfusion than those treated with high-dose argatroban or hirudin (P < 0.05). At the doses that induced comparable prolongation of bleeding time, argatroban showed a significantly faster and higher level of reperfusion than hirudin. In animals treated with hirudin, there was a positive correlation between the aPTT and the mean reperfusion blood flow (r = 0.70, P < 0.05). In animals treated with argatroban, this correlation did not exist and the high-dose argatroban was paradoxically less effective in promoting thrombolysis despite greater anticoagulation effects. In this animal model of arterial thrombosis, argatroban was more effective than hirudin in inducing rapid, full reperfusion with t-PA. Although they are both direct thrombin inhibitors, these two agents showed important dose-related differences in efficacy and anticoagulant effects.

[History of the Rostock University Gynecology Clinic 1887-1997: what remains?].

110 years after the opening of the Department of Obstetrics and Gynaecology, which is still functioning, a historical review about those scientific facts published during these years and continuing to do so up to now is presented. Scientific highlights in obstetrics and gynaecology created in Rostock are the publication by Friedrich Schatz on intrauterine tocography, the anatomic description of the menstrual cycle by Robert Schröder, and the studies by Helmut Kyank on the physiology and pathophysiology of pregnancy, especially pre-eclampsia and eclampsia.

[Secondary resection of soft tissue sarcomas within the scope of a multimodality therapy concept].

We treated 95 patients with primary soft tissue sarcoma of the extremities or trunk between 1988 and 1995, who were operated on initially elsewhere and their tumors were supposed to be excised completely. We performed a primary re-excision in all patients and found residual tumor in 46%. After a median follow-up of 61 months, 16 patients developed a local recurrence and 6 patients died due to the tumor disease. Primary re-excision is therefore indicated in most cases where the histology report of an assumed benign tumor reveals malignancy.

Signal transduction in macrophages by glycosylphosphatidylinositols of Plasmodium, Trypanosoma, and Leishmania: activation of protein tyrosine kinases and protein kinase C by inositolglycan and diacylglycerol moieties.

The perturbation of various glycosylphosphatidylinositol (GPI)-anchored surface proteins imparts profound regulatory signals to macrophages, lymphocytes and other cell types. The specific contribution of the GPI moieties to these events however is unclear. This study demonstrates that purified GPIs of Plasmodium falciparum, Trypanosoma brucei, and Leishmania mexicana origin are sufficient to initiate signal transduction when added alone to host cells as chemically defined agonists. GPIs (10 nM-1 microM) induce rapid activation of the protein tyrosine kinase (PTK) p59(hck) in macrophages. The minimal structural requirement for PTK activation is the evolutionarily conserved core glycan sequence Man alpha1-2Man alpha1-6Man alpha1-4GlcN1-6myo-inositol. GPI-associated diacylglycerols independently activate the calcium-independent epsilon isoform of protein kinase C. Both signals collaborate in regulating the downstream NF-kappa B/rel-dependent gene expression of interleukin 1alpha, tumor necrosis factor (TNF) alpha, and inducible NO synthase. The alkylacyl-glycerol-containing iM4 GIPL of L. mexicana, however, is unable to activate protein kinase C and inhibits TNF expression in response to other agonists, establishing signaling specificity among structurally distinct GPIs. GPI alone appears sufficient to mimic the activities of malaria parasite extracts in the signaling pathway leading to TNF expression. A mAb to GPI blocks TNF induction by parasite extracts indicating that GPI is a necessary agent in this response. As protozoal GPIs are closely related to their mammalian counterparts, the data indicate that GPIs do indeed constitute a novel outside-in signaling system, acting as both agonists and second messenger substrates, and imparting at least two separate signals through the structurally distinct glycan and fatty acid domains. These activities may underlie aspects of pathology and immune regulation in protozoal infections.

[Salvage therapy of advanced squamous cell carcinoma of the mouth cavity and oropharynx. Results of interstitial high-dose-rate brachytherapy combined with ablative tumor surgery].

PURPOSE: The clinical effect of high-dose-rate (HDR) interstitial brachytherapy combined with tumor resection was investigated in this retrospective study on patients with recurrent oral and oropharyngeal squamous cell carcinoma. PATIENTS AND METHODS: Oral and oropharyngeal squamous cell carcinoma in 38 patients were treated over a period of 7 years (1988-1994) by HDR interstitial brachytherapy using Gammamed 12i equipment. Pretreatment of patients in terms of irradiation and surgery differed (33 irradiated with a total dose between 60.0 and 75.6 Gy, including 8 patients with additional surgery; 1 patient with surgery alone, and 4 patients without any pretreatment). Indications for interstitial brachytherapy differed according to individual responses to treatment and medical histories. RESULTS: Interstitial brachytherapy was successful in the majority of patients, i.e. complete remission: 12, partial remission: 19, no change: 2, progression: 5. Local control and overall survival including patients with surgical treatment was 59% and 81% at 6 months and 47% and 49% after 12 months, respectively. CONCLUSION: Interstitial HDR brachytherapy with Iridium 192 is recommended in patients with local recurrences or second primary carcinomas after previous external radiotherapy in the head and neck region. Combination of interstitial brachytherapy and surgery is preferable for these patients.

[The development of radiotherapy in cervical carcinoma at the University Clinic of Rostock].

PURPOSE: Doubtlessly modern high voltage radiation treatment has less side effects than former techniques. It may be better than conventional methods with respect to 5-year survival rates, but that has to be proven using a large number of cases. PATIENTS AND METHODS: Retrospective study using the material of one department of gynecology and obstetrics, practicing both operation and irradiation. 1830 cervical carcinomas are differentiated in stages and then divided in 3 historical collectives. From 1959 to 1968 there was a combination of traditional contact therapy with conventional X-ray irradiation. The period from 1969 to 1972 is characterized as a changing one with partly therapy as described above, partly traditional contact therapy was combined with biaxial telecobalt therapy. From 1973 to 1986 a reduced contact therapy doses was applied to pelvic high voltage irradiation. RESULTS: Five-year survival rate in stage I was 79% in conventional treatment and with ultra high energy 77%. The corresponding figures in stage II are 56% respectively 52%, in stage III 26% respectively 25% (Table 1) of gynecology and obstetrics, practicing both operation and irradiation. CONCLUSION: Application of ultra high energy radiation in higher doses in Rostock had not the consequence of elevating the 5-year survival rate, but the quality of life is much better.

Partnership in chronic health care: care management as an integral part of the paediatric hospital system.

Care management for children with complex care needs in the hospital setting is not just about care managers developing and maintaining partnerships between hospital and community professionals, children and their families. It also requires organizational commitment to a system which is patient centred, recognizes parental expertise and offers clinical skills which are co-ordinated across specialty areas.

Nutritional and functional blood vessels of anagen and telogen vibrissal follicles in the cat.

The nutritional blood vessel system in the anagen (growing) vibrissal follicle consists of a rete capillare papillae pili, rete capillare bulbi pili, rete capillare folliculi pili, and rete capillare canalis pili. Due to degeneration of papilla and bulb during the sinus hair cycle, the basal capillary networks are absent in the telogen (resting) vibrissal follicle. Thus, nonpermanent and permanent capillary networks have to be distinguished. A functional blood vessel system originates from arterioles that enter the sinus cavernosus at its basolateral side. Blood flow continues into the sinus anularis. Efferent capillaries, originating from the apical-axial area of the sinus anularis, build up a separate network for the collection of blood from the sinuses. These capillaries continue towards the dermis, contribute to the nutritional vascularization of the sebaceous gland, and finally connect with the subepidermal capillary loops.

[Histochemical and biochemical studies of the circannual aspects of carbohydrate-dependent energy metabolism of the hair follicles of Turkish Angora goats].

Based on biopsy material, the study describes circannual variations of the accumulation of glycogen and organic carbon in the hair follicles. The results have been obtained by histochemical and biochemical methods. The histochemical-densitometric analysis of glycogen contents in the cells of the outer epithelial root sheath of the hair follicles exhibited a clear circannual course, with maxima in May/June and December, and minima in March and August. The content of organic carbon of the hair follicles was 7.61 micrograms/mg (max. 21.56 micrograms/mg) DW on average, with a corresponding annual cycle. The findings are discussed with regard to the energy metabolism of the hair follicle, especially in view of the fact that a biphasic cycle of hair growth becomes obvious in the Turkish Angora goat, a phenomenon that is obscured by other methods.

All-trans retinoic acid and short-time, high-dose cytarabine in two children with acute promyelocytic leukemia.

We report on two girls, 3 and 13 years old, with acute promyelocytic leukemia (APL) who were successfully treated with all-trans retinoic acid (ATRA) 45 mg/m2/day. "Retinoic acid syndrome" was prevented with short-time treatment of high dose (4 x 1.5 g/m2) cytarabine. This regimen was well tolerated, although both children were critically ill. They achieved a complete remission confirmed by light microscopy, but reverse transcriptase polymerase chain reaction remained positive after ATRA, underlining the need of further chemotherapy.

Neutrontherapy in soft tissue sarcomas: a review of European results.

The purpose of this publication is to review the data on European neutrontherapy facilities in the treatment of soft tissue sarcoma. Data on 1171 patients treated at 11 centers were analyzed. Neutrontherapy was of advantage especially for patients with primary radiotherapy of inoperable tumors, and those with macroscopic disease after surgery. Local control was about 50% in these groups. Results for postoperative neutrontherapy after R0 and R1 resection were similar to those for photon therapy. The incidence of late side effects was considerable. This was influenced by the fact that most patients were treated with first-generation machines.

European results in neutron therapy of malignant salivary gland tumors.

In Europe to date, 501 patients with salivary gland tumors have been treated with neutrons alone or with combined modalities. The most common histological types are adenoid cystic carcinomas, mucoepidermoid carcinomas and malignant mixed tumors. The results of conventional radiotherapy are suboptimal in inoperable or incompletely resected salivary gland tumors and in recurrent disease. The pooled data of some international series for low linear energy transfer radiation show a local control of 28%. Especially in advanced tumors neutron therapy can improve local control and should be the treatment of choice. The clinical data from different therapy centers in Europe show local control of 67% in gross disease.

Impact of diagnostic laparoscopy in the management of the unilateral impalpable testis.

OBJECTIVE: To evaluate the impact of laparoscopy on the management of children with a unilateral impalpable testis. PATIENTS AND METHODS: The study population consisted of 27 children who underwent a primary inguinal exploration for a unilateral impalpable testis. RESULTS: Twelve of 27 (44%) children had inguinal or 'peeping' testes and 10 of 27 (37%) had blind-ending vasa and vessels in the inguinal canal; four of these 10 had atrophic tubular tissue in the excised remant. Four of 27 (15%) had blind-ending vasa and vessels proximal to the internal ring. Only one child had a testis proximal to the internal ring. Only the four children (15%) with blind-ending vasa and vessels proximal to the internal ring would have benefited from a laparoscopy by avoiding an inguinal exploration. CONCLUSIONS: Because of the time, expense and limited usefulness of laparoscopy in altering the management of children with a unilateral unpalpable testis, we reserve laparoscopy for cases where inguinal exploration has failed.

Quality of life and supportive care in the treatment of NSCLC.

The definition of quality of life (QOL) for individual patients with non-small cell lung cancer (NSCLC) is unclear and its evaluation difficult because of the heterogeneity of the patients involved. More research is needed to determine the best means of routinely expressing and comparing QOL assessments and implementing the results of these studies into daily patient care. The role of broad supportive care in optimizing patient comfort and functionality is also an important issue. Because it incorporates not only specific palliative treatment, but also nontumor-specific measures and medical intervention, supportive care is relevant to all patients with NSCLC, whether terminally ill or suffering treatment-related toxic reactions. More education, research, and financial support are needed to optimize QOL and supportive care of patients.

[Results of radiotherapy in recurrences of soft tissue sarcomas].

PURPOSE: Published data for radiotherapy in soft tissue sarcomas refer in most of the cases to the use of irradiation for primary tumors. Experiences of radiotherapy in local recurrences are limited. The own treatment results of a multimodal treatment approach in a large number of patients will be analyzed for local tumor control rates, survival rates, side effects and prognostic factors and will be compared to the literature. PATIENTS AND METHODS: At the University Hospital Hamburg-Eppendorf a total of 98 patients with local recurrences of soft tissue sarcomas were irradiated between 1980 and 1993. The median age of the patients was 55 years. There was a large variety of different histologies. Grading was evaluable in 95 cases: G1 24 patients, G2 21 patients and G3 50 patients. Localisation was dominated by the extremities with 64.3%. Recurrences were reclassified: 21 patients had rT1-tumors and 77 patients had rT2-tumors. Twelve cases showed lymph node metastases and 9 patients distant metastases. Generally treatment included surgery and postoperative irradiation. R-classification showed R0 in 25, R1 in 20 and R2 in 48 cases. Neutrontherapy was prefered in 57 cases and neutron- and photontherapy in 22 cases. Local tumor control rates and survival rates were calculated using the Kaplan-Meier method. Side effects were scored using the RTOG/EORTC scoring system. Univariate and multivariate analyses were applied to evaluate prognostic factors. RESULTS: Local tumor control rates at 5 years were 56.4%, for G1 75.2%, G2 52.5%, G3 47.1%, rT1 66.0%, rT2 53.6%, R0 63.4%, R1 42.3% and R2 53.2%, N0 61.1% vs. N1 31.7%. Survival rates at 5 years were 45.2%, for G1 64.8%, G2 66.9%, G3 25.0%, rT1 60.0%, rT2 41.5%, R0 64.2%, R1 49.3% and R2 32.7%. Acute side effects were scored as grade 1 and grade 2. The rate of grade 3 and 4 late effects was about 6%. Lymph node status was a significant factor for local control. Grade, residual tumor status, type of irradiation and the applied neutron dose were significant factors for survival. CONCLUSIONS: Nowadays no standard treatment exists for local recurrences of soft tissue sarcomas. Treatment should be interdisciplinary. Local recurrences should be avoided by the consequent use of surgery and radiotherapy. It is important that local recurrences should be detected early. Neutrontherapy may bring advantages for local control of recurrences with macroscopic tumor residuals.
