Case series evaluation of BCG vaccine efficacy against tubercular meningitis in children in Delhi.

Tubercular meningitis (TBM) is a very serious disease of childhood. BCG vaccination under Universal Immunisation Programme is the only available way of preventing this disease. The present series using case-control method, evaluated efficacy of BCG against TBM in children to be 57 per cent. Suggestion has been made to initiate TBM surveillance in children and conduct multicentric prospective case-control study of similar type.

Practices of preparation of oral rehydration solution among mothers reporting to the drug distribution centres in Delhi, India, 1992.

A study to assess the ability for preparing oral rehydration solution (ORS) correctly among the recipients of ORS packets from the Government of India service outlets was carried out in Delhi during July to August, 1992. One centre from each of the municipal zones of Delhi was selected randomly. Four hundred mothers with a child having diarrhoea reporting to the nearest centre were studied using a standardized questionnaire adopted from the WHO household survey schedule. Two paramedical personnel from each of the centres who were responsible for distribution of ORS were also interviewed regarding their knowledge of preparation of ORS. Results indicate that only 10.8% of mothers prepared the ORS correctly and that the majority (83%) of the paramedicals involved in ORS distribution had correct knowledge about its preparation. Communication methods directed at the mothers regarding the preparation of ORS should be stepped up in these communities.

Monitoring the efficacy of temephos application and use of fine mesh nylon strainers by examination of drinking water containers in guineaworm endemic villages.

Application of temephos in unsafe water sources for destroying cyclops, the intermediate hosts of guineaworm, and distribution of fine mesh nylon strainers for promoting prophylaxis against guineaworm are accepted methods of guineaworm control in different endemic countries. The existing methods of monitoring the efficacy of these guineaworm control methods are not fully informative. Examination of drinking water stored at household levels for presence of cyclops with or without Dracunculus larvae can provide information on the efficacy of these control/prophylactic methods, besides serving as a means of interpersonal health education to the community. This paper presents observations carried out in two villages in peninsular India in 1991, which revealed that while in one village complete absence of cyclops from stored water containers was attributable to the use of temephos in the village and straining of drinking water, in the other village, with no temephos application, 15.6 per cent of containers contained varying numbers of cyclops in them. Implications of these observations for guineaworm eradication activities are discussed.

Serum and semen levels of immunoreactive prolactin, LH and FSH in normospermic, oligospermic and azoospermic men with obstructive infertility prior to and after vasoepididymostomy.

Attempts were made to validate RIA for prolactin (PRL), LH and FSH in semen from normospermic, oligospermic and azoospermic subjects. The RIA used to measure PRL and LH in semen fulfilled the criteria of reliability, whereas low levels of FSH in semen precluded the validation of FSH assay in semen. Semen levels of PRL and LH were significantly (P < 0.05) higher than serum levels in all groups of subjects investigated. Semen levels of FSH in azoospermic men after vasoepididymostomy (VEA), were significantly (P < 0.05) higher compared to azoospermic men prior to surgery. Serum levels of PRL were significantly higher (P < 0.05) in normospermic men compared to oligospermic and azoospermic men prior to and after surgery. Semen levels of PRL in normospermic men were comparable with oligospermic and azoospermic subjects prior to and after surgery. Serum levels of LH in oligospermic and azoospermic men who did not undergo surgery and in men reporting oligospermia after VEA were comparable to normospermic subjects but in men showing azoospermia post surgically, serum LH levels were significantly (P < 0.005) elevated. Semen levels of LH in men reporting azoospermia before surgery and in subjects showing oligospermia or azoospermia post surgically were significantly lower (P < 0.05) compared to men with normal sperm count. Serum levels of FSH were significantly elevated (P < 0.05) compared to semen levels in oligospermic men prior to surgery but this increase was not seen in post VEA subjects. These results were discussed.

Epidemiological profile of Japanese encephalitis in Gorakhpur district, Uttar Pradesh, 1982-1988.

An in-depth study of Japanese Encephalitis (JE) situation in Gorakhpur district of Uttar Pradesh from 1982-1988 showed increasing trend in the incidence of JE. Total number of annual cases and case fatality rate (CFR) rose from 118 and 23.7 per cent in 1982 to 772 and 32.2 per cent in 1988 respectively. A definite increase was noticed in the number of cases per block following lull years in 1984 and 1987. Among the total affected 1201 villages, 1083 were affected only once. All age groups were affected and the disease showed marked seasonality during August to November. JE, which came in epidemic form in earlier years has established in the area in endemic form.

Diarrhoeal diseases amongst children under five. A study in rural Alwar.

A house to house survey was done in three villages of district Alwar covering 875 children under five years age. Two week incidence of diarrhoea morbidity was 2.27 episodes/child/year taking into consideration the seasonal correction factor. The incidence decreased with increase of age. Incidence was found significantly more in children of illiterate mother (p < 0.05). Children of poor socio-economic conditions as determined by occupational status (labourers) suffered significantly more often from diarrhoea as compared to children of higher socio-economic status (agriculturist and others). Fifty per cent episodes of diarrhoea were treated with antibiotics, and only one child was given ORS. It is a matter of concern. About 3.7 per cent mothers washed their hands before preparing meals while, 1.6 per cent washed their hands after toilet. Only 2 per cent mothers had the knowledge of preparing the home made salt sugar solution. An intensive health education campaign is therefore, necessary for health professionals as well as mothers.

Review of the Journal of Communicable Diseases from 1982 through 1991.

Review of medical journals is not common in India. A quantitative review of the articles published in the Journal of Communicable Diseases from 1982 through 1991 was undertaken in the present study to give feedback to all concerned and stimulate them for initiating constructive criticism of medical journals published in India. Articles were categorised as per the classes of four predetermined parameters and variations in the proportions of number of articles under different classes of each parameter are briefly discussed.

Steroid metabolism by germ cells and spermatozoa in men after vasoepididymostomy.

The ability of germ cells (spermatocytes and spermatids) and spermatozoa present in human ejaculate to metabolize steroids was studied in men with obstructive infertility who had undergone vasoepididymostomy as corrective surgery. Steroid metabolism by spermatozoa in men who had undergone vasovasostomy was also investigated. Germ cells converted testosterone mainly to androstenedione. In addition to androstenedione, dihydrotestosterone and androstanediols were also formed in incubations using spermatids. Both types of germ cells converted estradiol to estrone. Spermatozoa from subjects who had undergone vasoepididymostomy or vasovasostomy converted testosterone to androstenedione as in normal men, while spermatozoa from infertile subjects converted testosterone mainly to dihydrotestosterone. Seminal fluid, free of germ cells, did not show steroid-metabolizing capability.

Monitoring of temephos (abate) application as a cyclopicide under the guineaworm eradication programme in India.

Temephos application in unsafe water sources at a dose of 1ppm, which started in 1986, is an important activity of the guineaworm eradication programme in India. During March 1989, monitoring of temephos application was carried out in six villages in each of the five guineaworm endemic districts: Kurnool (Andhra Pradesh), Gulbarga (Karnataka), Dhar (Madhya Pradesh), Thane (Maharashtra) and Jhalawar (Rajasthan), selected for the study. Out of 62 unsafe water sources targeted in the sampled villages for temephos treatment, 43 were treated during 1989 in the five districts (coverage 69.35 per cent). Out of these 43 treated sources 79.1 per cent were free of cyclops and the rest 20.93 per cent showed a variable cyclops count. 117 adult cyclops identified from these sources included Mesocyclops byalinus (82) and M. leuckarti (35). People's perception of utility of chemical treatment of water sources varied from negligibly low to moderate levels. Those areas which received timely application of temephos during the preceding year, al sharp decline in the number of cases in 1989 was observed. However, in those villages where no temephos was applied or where application was started as late as April or May, there was no impact on prevalence of the disease.

Field evaluation of measles vaccine efficacy in New Seemapuri, Shahdara Zone-Delhi during 1990.

A field evaluation of measles vaccine efficacy was undertaken in resettlement colonies in New Seemapuri, Shahdara Zone, Delhi, having an estimated population of 20,500 by case-reference method. The standard 30 cluster sampling technique was adopted and 261 measles vaccinated children were matched with 545 measles non-vaccinated children. The attack rate of measles was found to be 4.2 and 30.3 per cent in measles vaccinated and non-vaccinated children respectively, which is statistically highly significant (P less than 0.001). The relative risk of developing measles in non-vaccinated child is seven times more as compared to a vaccinated child. Vaccine efficacy (V.E.) evaluated by different epidemiological and laboratory methods was found to be 86.1 per cent +/- 11 at P = 0.05. Further, a large scale study on measles V.E. is recommended in different parts of the country by case-reference method, as it is found to be simple, reproducible and easy to undertake in a field situation.

A study of two-weekly incidence of ARI in under-five children of rural area of Alwar (Rajasthan).

A retrospective study of two-weekly incidence of acute respiratory infections (ARI) in the under-five children of rural area of Alwar District (Rajasthan) based on the previous fourteen days' recall by the mothers was undertaken during December 1989 to assess the magnitude of the problem and its' risk factors. The two-weekly ARI incidence was reported to be 33 episodes per 100 under-fives. Mothers in higher socio-economic strata reported higher incidence of ARI for their children due to more accurate recall. Majority of ARI cases (76 per cent) were found to be of mild degree. 26 per cent of deaths in under-fives during the previous year were reported to be ARI-related. 50 per cent of ARI cases were left untreated. Among the treated group, private practitioners were found to be the main source of health care.

Seminal fructose in normal and infertile men.

Fructose levels and fructolysis index in human semen were analysed to assess a correlation, if any, between the levels of this glycolysable sugar and sperm concentration. Semen was collected from normospermic men and men with azoospermia or oligospermia. Seminal fructose levels were elevated in men with obstructive azoospermia and in men who remained azoospermic following vasoepididy mostomy done to correct epididymal blockage. Men with sperm concentration of less than 20 million/ml pre-operatively or following vasoepididy mostomy, showed significantly high levels of fructose and lower fructolysis index. Fructose levels in normospermic infertile men, as well as in men with normal sperm counts (more than 20 million/ml), were similar to that in men of proven fertility.

Epidemiological evaluation of BCG vaccine efficacy in Delhi--1989.

A field evaluation of efficacy of BCG vaccine to prevent tubercular meningitis was undertaken in a case control study in Delhi during 1988-89. Each case of tubercular meningitis was matched by age and neighbourhood with 2 controls from the community. Thirty-seven cases of tubercular meningitis were matched with 74 controls and thus 37 triplets were analysed. ODDS RATIO between BCG vaccinated and non vaccinated individuals for acquisition of tubercular meningitis was 11.3. This gives 84 per cent efficacy of BCG vaccine in the prevention of TB meningitis under the field conditions prevailing in Delhi. Paired matched analysis of tubercular meningitis with the control group 1 and 2 separately revealed the ODDS RATIO to be 9.5 and 15 respectively. The corresponding BCG vaccine efficacy worked out were 90 and 93 per cent respectively. Authors suggest that this method of evaluating BCG vaccine efficacy is fairly reliable and cost effective. This methodology could be further simplified by using hospital patients as control and be introduced to evaluate BCG vaccine efficacy in different areas with reference to its role in preventing tubercular meningitis under the programme of immunisation.
