Chronic pain and marital difficulties.

Chronic pain in one of the partners in a marriage causes profound changes in the marital relationship: roles and communication patterns are transformed, sexual relationships deteriorate, and the well partner frequently succumbs to distress and illness. In examining these changes, this article highlights the need for routine marital and family assessment and a family treatment approach for patients and their spouses.

Norfloxacin penetration into human renal and prostatic tissues.

Concurrent norfloxacin concentrations in serum, kidney, and prostatic tissue were determined in 14 patients. Mean ratios of norfloxacin concentration in tissue over concentration in serum were 6.6 +/- 2.8 for the kidney and 1.7 +/- 0.2 for the prostate samples. The levels were above the MICs of most urinary pathogens.

Engel's pain-prone disorder patient: 25 years after.

Engel's concept of pain proneness is reexamined. Child abuse issues, as they are presumed to influence pain proneness, are made explicit. Research literature related to pain proneness is reviewed. Its methodological shortcomings are discussed. Child abuse and child neglect literature was searched, but failed to reveal any association between child abuse and pain proneness. A broadly based research strategy is suggested to explore this association.

Determinant specificities of the groups B and C polysaccharides of Neisseria meningitidis.

A meningococcal group B-specific horse antiserum contains at least two distinct populations of antibodies with specificities for determinants on the group B capsular polysaccharide antigen. These two populations were differentiated on the basis of the ability of only one of them to be absorbed from the antiserum by the structurally related colominic acid. The nature of the colominic acid-specific determinant was elucidated by a radioimmunoassay inhibition technique with the use of a series of linear alpha-(2----8)-linked oligomers of sialic acid as inhibitors. Colominic acid was labeled by prior removal of its N-acetyl groups, followed by their replacement with the use of [3H]acetic anhydride. The conformational nature of the determinant was proposed because of the unusually large size (10 sialic acid residues) of the oligomer required to function as an efficient inhibitor. The structure of the determinant responsible for the second population of group B-specific antibodies has not been determined, but it is obviously based on an as yet undefined conformational or structural feature peculiar to the group B meningococcal polysaccharide. In contrast to the colominic acid-specific group B determinant, the determinant responsible for the group C polysaccharide-specific rabbit antibodies proved to be more conventional. Inhibitory properties of the alpha-(2----9)-linked oligomers maximized with those containing four or five sialic acid residues, which is consistent with the approximate estimated maximal size of an antibody site.

Epizootiological survey of parainfluenza-3, reovirus-3, respiratory syncytial and infectious bovine rhinotracheitis viral antibodies in sheep and goat flocks in Quebec.

A serological survey was conducted in an attempt to detect antibodies against bovine respiratory viruses in sheep and goats from seven geographical areas of Quebec. Sera from 10% of the animals in 182 sheep flocks and 40 goat flocks were collected and specific antibodies against parainfluenza-3, reovirus type 3, respiratory syncytial and infectious bovine rhinotracheitis viruses were detected by hemagglutination-inhibition tests for the former viruses and complement fixation and seroneutralization assays for the latter viruses. Results showed prevalence rates of serological reaction to parainfluenza-3, reovirus type 3 and respiratory syncytial viruses of 28, 72 and 35% in sheep and 26, 64 and 36% in goats, respectively. No antibodies in infectious bovine rhinotracheitis virus were detected in sheep or goats tested. Prevalence rates varied according to the geographical area. No relationships were detected between age, sex, breed, size of flock and prevalence rates of different antibodies except that parainfluenza-3 antibodies were more common in large goat flocks and in sheep flocks with total confinement housing. A relationship between presence of clinical signs in the flocks and prevalence rates of antibodies was only demonstrated for parainfluenza-3 infection in goat flocks.

HLA system, body fat and fat distribution in children and adults.

The association between the genotypes and the alleles at the A, B or C locus of the HLA system and body fat was studied in a total of 1578 individuals subdivided in four cohorts: adult males (n greater than or equal to 204), adult females (n greater than or equal to 184), boys and male adolescents (n greater than or equal to 282), and girls and female adolescents (n greater than or equal to 257). None of these subjects were grossly obese or had known metabolic disorders. Percent body fat from underwater weighing, subcutaneous fat from 6 skinfold measurements, trunk fat (3 skinfolds) and extremity fat (3 skinfolds) were considered in the analysis. Although a few significant associations were encountered, the lack of consistency across samples suggested that they were probably random and biologically not meaningful. The same negative findings were found for the ratio of trunk subcutaneous fat to extremity subcutaneous fat and the ratio of subcutaneous fat (6 skinfolds) to total fat mass. Earlier reports indicating a significant association between high body fat content and antigens B18, Bw35 or Cw4 were not supported by the results of this study. It was concluded that no consistent pattern of association emerged between genotypes or alleles of the HLA system and percent body fat, subcutaneous fat or fat distribution in children and adults of both sexes.

Immunochemical analysis of the types Ia and Ib group B streptococcal polysaccharides.

The types Ia and Ib group B streptococcal type-specific polysaccharides have remarkable immunologic differences despite a great deal of structural similarity. Although these two complex polysaccharides differ only by a single glycosidic linkage, they are antigenically distinct. Furthermore, terminal sialic acid residues appear to be critical to the immunodeterminant on the type Ia polysaccharide, whereas the antigenicity of the type Ib polysaccharide does not show this dependence on sialic acid. In the current investigation we defined better the immunodeterminant of these polysaccharides. With homologous rabbit antiserum, the type Ia native and core polysaccharides demonstrated partial serologic identity, whereas the type Ib native and core polysaccharides demonstrated complete serologic identity. Surprisingly, the type I degalactosylated polysaccharide, degraded structure, was capable of reacting with a population of antibodies present in type Ia antiserum similar to the complete type Ia native polysaccharide, although demonstrating a reduced level of immunodeterminant expression. Unlike the reactions of the type Ia polysaccharides with homologous rabbit antiserum, the Ib native and core polysaccharides were able to react with identical populations of antibodies in type Ib-specific antiserum. A minor population of antibodies was demonstrated in the type Ib antiserum, which was reactive with the degalactosylated polysaccharide. That a population of antibodies reactive toward the degalactosylated polysaccharide is present in both type Ia and type Ib antisera suggests that the Iabc cross-reacting determinant is due to the presence of serum antibodies reactive with this trisaccharide repeating unit, which is shared by both the type Ia and the type Ib native and core polysaccharides.

Protein synthesis in rabbit reticulocytes: requirements for Met-tRNAf . 40S preinitiation complex formation with AUG-codon and physiological mRNAs.

Under standard conditions, in the presence of GTP, highly purified eIF-2 and Co-eIF-2 factor preparations efficiently stimulated AUG-codon dependent but not physiological mRNA-dependent Met-tRNAf binding to 40S ribosomes. Replacement of GTP by a nonhydrolyzable GTP analog, GMP-PNP, in the above system, gave significant stimulation of Met-tRNAf binding to 40S ribosomes dependent on physiological mRNAs. Lower but significant stimulation of Met-tRNAf binding to 40S ribosomes was also observed when GTP was used in the presence of nucleoside 5'-diphosphate kinase (NDK) and ATP. ATP alone in the absence of NDK had no significant effect. This is the first report on the formation of a stable Met-tRNAf . 40S initiation complex dependent on physiological mRNAs and the factor requirements for such complex formation.

Chemical modification and serological properties of the 3-deoxy-alpha-D-manno-2-octulosonic acid-containing polysaccharide from Escherichia coli LP1092.

The alpha-D configuration of the 3-deoxy-D-manno-2-octulosylonic acid residues in the E. coli LP1092 polysaccharide was definitively assigned by a comparison of its c.d. spectrum with those of methyl 3-deoxy-alpha- and -beta-D-manno-2-octulopyranosidonic acid. The c.d. spectrum of the polysaccharide showed a negative n----pi transition at 211 nm, associated with carboxyl groups, which correlated well with the negative band at 218 nm exhibited in the c.d. spectrum of the methyl alpha-D-glycoside. In contrast, the methyl beta-D-glycoside gave a positive band in the same region of its c.d. spectrum, at 221 nm. Treatment of the E. coli LP1092 polysaccharide with 3-(3-dimethylaminopropyl)-1-ethylcarbodiimide hydrochloride yielded a modified polysaccharide containing O-acylisourea groups and intramolecular lactones, in addition to some unesterified carboxyl groups. Both forms of ester could be reduced to hydroxymethyl groups by sodium borohydride. Although immuno-precipitation of an antiserum specific for the E. coli LP1092 polysaccharide is not sensitive to the introduction of O-acylisourea groups into the polysaccharide, precipitation was completely eliminated when approximately half of the carboxyl groups of the polysaccharide were converted either into lactone or hydroxymethyl groups. Failure to precipitate the antiserum can be attributed to significant conformational changes in the polysaccharide brought about by the introduction of the latter two groups.

Psychosocial assessment of chronic headache.

Chronic headache is often a crippling condition for those afflicted by it. The case of a headache sufferer is presented here against a framework that reviews the literature on the psychological, social, and familial issues related to the problem of chronic pain. The intent is to offer practitioners guidelines and a context within which clients with chronic headache can be understood and helped.

Blood transfusions in renal dialysis patients. Effect on cellular immune response.

The effect of blood transfusions, given in low number (less than 5), on the immune response of renal dialysis patients was studied. A significantly lowered response of lymphocytes to mitogen stimulation was observed in patients after as few as one, two or three transfusions, depending on the patient. This led to an increased delta 48/delta 0 ratio reflected by the enhanced response of the cells following delayed addition of suboptimal dose of mitogen. There was no modification of the ratio of helper/inducer to suppressor/cytotoxic T cells subsets (OKT4/OKT8). The use of such simple in vitro tests in a strict protocol of transfusions could allow an adequate follow-up thereby limiting the risks of sensitization. These results demonstrate that important phenomena affecting patients' immune response are turned on following even a low number of transfusions in the majority of uremic patients. This could probably be related to the beneficial effects of blood transfusions on the kidney allograft survival already described.

Improved procedures for the conjugation of oligosaccharides to protein by reductive amination.

The rate of coupling of oligosaccharides having aldose end groups to protein by reductive amination was significantly increased by changing the temperature and pH of the reaction, and even more significantly by the addition of borate ions. Under optimized conditions half of the lysine residues of bovine serum albumin could be derivatized by lactose in 7 h and their complete derivatization was achieved in approximately 24 h. All attempts to carry out similar reductive amination procedures using oligosaccharides having ketose (D-fructose, 3-deoxy-D-manno-octulosonic acid (KDO), and sialic acid) failed owing to the slowness of the reaction. Model studies on the coupling of D-fructose and KDO to glycine indicate that any coupling procedure based on reductive amination of ketose residues would of necessity require the prior introduction of a small functionalized spacer molecule.

Synthesis of acid phosphatase and other proteins by human prostatic tissue in vitro.

We have studied the synthesis of proteins by normal and hyperplastic human prostatic tissue incubated in vitro in the presence of [35S]methionine. The overall pattern of newly synthesized proteins was similar in individuals with an age ranging from 15 to 68 years. The pattern of labeled proteins was quite different from that of total proteins stained with Coomassie blue in two-dimensional polyacrylamide gels, since the major stained proteins was not labeled. Among the most heavily labeled proteins (about a dozen) were several spots representing charge isoforms with molecular weights ranging from 46 000 to 51 000, and these were the only proteins immunoprecipitated by a polyclonal antibody developed against purified acid phosphatase. The other heavily labeled proteins had molecular weights ranging from 26 000 to 72 000. These results show that tissue slices can be used to study the synthesis and processing of acid phosphatase, the major secretory product of the prostate, and of other unidentified proteins.

Differential patterns of alcohol abuse among normal, neurotic, psychotic, and characterological types.

The MMPIs of 201 male alcoholic inpatients were classified into normal, neurotic, psychotic, and characterological types according to the Goldberg rules (1972). Multivariate analysis of variance of the respective Alcohol Use Inventory ( AUI ) scores showed that the groups differed in a multivariate sense. The pattern of significant univariate differences indicated that both the neurotic and psychotic groups reported more severe symptoms and consequences of alcohol abuse than the normal and characterological types. No univariate differences were found between either the neurotic and psychotic types or the normal and characterological types. Results were discussed in terms of functions that alcohol served among the groups.

Presence of antibodies to bovine viral diarrhea-mucosal disease virus (border disease) in sheep and goat flocks in Quebec.

A seroepidemiological study of border disease was conducted in sheep and goats in various areas of Quebec. Sera of 10% of animals of selected flocks were collected and specific antibodies against bovine viral diarrhea- mucosal disease were tested by seroneutralization. Results show that 10.9% and 16% of sheep and goats respectively gave a positive reaction. The lower serological prevalence was found in sheep flocks of the Sherbrooke area (5.4%) while the highest percentage of positive sera was observed in the Quebec area (24.7%). The prevalence in goats varied according to areas (2.6 to 45.5%). No relation was observed in seropositive animals between age, sex, breed and the presence of abortions in the flocks. Our serological results indicate that border disease is probably present in Quebec sheep and goat flocks but that the clinical diagnosis of this disease is not well established.
