The authors report a case of spleno-gonadal fusion in which the advantage of an angiographic study permitted, for the first time, an analysis of the vascularization of this malformation. The embryologic study explains the malformations associated with the continuous type of spleno-gonadal fusion.

A double blind study has been carried out on patients scheduled for appendicectomy. They were divided into two groups who were given diazepam as premedication either orally or intramuscularly. The clinical effects were measured with regard to anxiolysis, gastric contents volume and acidity. Orally premedicated patients were not significantly less anxious. No significant differences between both groups concerning gastric contents (volume and pH) were found. Therefore, oral premedication did not increase the risk of inhalation. Taking into account the previous findings in the literature, the pharmacological advantages of the oral route and the lack of complications, a wider use of oral premedication for elective operations in adults is recommended.
The binding of human fibrinogen to different morphological forms of Candida albicans (blastoconidia, germ tubes and hyphae) was investigated by immunofluorescence and electron microscopy. Fibrinogen appeared to bind more strongly to hyphae than to yeast forms, and was detected on the cell wall coat. These observations suggest a possible role for fibrinogen in fungal colonization processes in vivo.

The legitimacy of osteosynthesis for the treatment of spinal injuries in children is analyzed as a function of results in 10 cases, 7 affecting the cervical, 2 the thoracic and one the lumbar region, six of the children being under 10 years of age. Five of these children had serious neurologic disorders, including 4 with high tetraplegia. Technical difficulties of osteosynthesis were not made easier by the poorly adapted nature of the material for infantile statures. Indications for this surgery were not only those cases not improved by orthopedic therapy: instability of lesions, the child's condition and deterioration in the neurologic state can also require initial treatment by osteosynthesis. The principal complication is postoperative cervical kyphosis, but this can be prevented by early active reeducation, the stability of the fixation dispensing with the need for external contention.
Five patients were treated after a period varying between 6 weeks and 18 months following original injury. The five lesions were similar in that they could not be reduced by conventional orthopedic measures and that they were fixed during dynamic review examinations using a brilliance amplifier.

Intraoperative cholangiograms were studied in 250 patients. Analysis of the data obtained led to the establishment of a protocol for intraoperative cholangiography aimed at identification of anatomical anomalies and variations. Diagnosis of the latter must be achieved in order to avoid possible intraoperative complications. The common hepatic duct was formed by the junction of the right and left hepatic ducts in 52% of the cases studied. Absence of convergence of the posterior and anterior rami of the right hepatic duct was found in 30% of cases. Anatomical variations of the right sectorial duct system were seen in 12% of cases. Conversely, variations of the left sectorial duct system were rarely seen (2% of cases). Careful examination of the intraoperative cholangiograms led us to suspect certain anatomical variations in close to 1% of cases. These variations included abnormal hepatocystic duct, which if undiagnosed could lead to choleperitoneum or inadvertent ligation of the right hepatic duct. An abnormal hepatocystic duct terminating on the gall bladder was found in one patient. Study of the origin of the common bile duct allowed us to define the mode of termination of the cystic duct (on the right margin of the common hepatic duct in 80% of cases) and to identify a short choledochus in 2% of cases. Finally, variations of the duodenal termination of the common bile duct were studied and reflux into the pancreatic duct was seen in 27% of cases. However, the pathological significance of such reflux was rarely found.
The authors describe the anatomical basis of splenorenal anastomosis via the retroperitoneal approach. This technique can be used in cases where the transperitoneal approach is contraindicated.
By means of gel agar diffusion techniques (Ouchterlony and immunoelectrophoresis), the authors have demonstrated in hydatid cyst fluid and membrane of E. granulosus, the presence of, at least, one fraction showing a human albumin specificity. This is not a contamination from the host and must be synthesized by the parasite itself. The significance of such a fraction is discussed.

Intrathecal serotherapy appears to be effective in the treatment of tetanus, and it has been claimed that human immune globulins are innocuous. In the 4 cases reported here reversible paraplegia developed a few hours after intrathecal injection of 1500 to 2000 UI of human immune globulins prepared by the French Blood Transfusion Centre. We believe that paraplegia was related to the high doses administered. The fact that intrathecal serotherapy is normally used in severely ill patients whose impaired consciousness precludes motricity and sensitivity testing may explain why this complication has not previously been reported. The need for high doses of intrathecal immune globulins is discussed. It has recently been suggested that high doses are superior to low doses. We consider that this form of treatment is justified, even in the least severe forms of tetanus, but we feel that doses higher than 1000 IU are not without danger.

A patient with Paget's disease developed an osteogenic sarcoma of the cranium. This emphasizes the need to conduct full neuroradiological investigations, including a CT scan and even selective arteriography as well as standard radiographs, whenever the possible existence of such a lesion is suspected, to determine therapeutic strategy.
