Treatment of bacterial cystitis with a single dose of trimethoprim, co-trimoxazole or amoxycillin compared with a course of trimethoprim.

A single dose of trimethoprim, co-trimoxazole or amoxycillin was compared with a five-day course of trimethoprim for the treatment of bacterial cystitis in general practice. The respective cure rates were 80%, 80%, 65% and 86%. These differences were not statistically significant. Side effects were minimal. Single dose therapy is recommended as the treatment of choice for bacterial cystitis in domiciliary practice.

Enoxacin for the treatment of urinary tract infection.

Enoxacin, a new orally absorbed broad spectrum anti-microbial agent, was used to treat 25 patients with moderate or severe urinary tract infections. Many were complicated and associated with renal insufficiency. Eighteen of the 25 were cured, five had a relapse and two became reinfected with a resistant Streptococcus faecalis. Fourteen of the patients reported side effects, including six with a neurological syndrome, one with widespread arthralgia and one with an Achilles tendonitis. Further studies are indicated with this potent drug to assess its role in the management of severe or complicated urinary infections and to assess the optimum dose and duration of therapy.

Vesicoureteric reflux: segregation analysis.

Complex segregation analysis was applied to data from 88 families containing at least one person with vesicoureteric reflux. Analysis showed that a single major locus was the most important causal factor in this condition, with the mutant allele being dominant to the normal allele and having a gene frequency of about 0.16%. Forty-five percent of gene carriers will have vesicoureteric reflux and/or reflux nephropathy as adults and 15% will develop renal failure, compared to 0.05% and 0.001%, respectively, for those not carrying the gene. This analysis confirms the importance of screening close relatives of persons with proven vesicoureteric reflux or reflux nephropathy.

Comparison of netilmicin with cefoperazone for the treatment of severe or complicated urinary tract infections.

A randomised trial was undertaken to compare the efficacy and safety of netilmicin and cefoperazone for the treatment of severe or complicated urinary tract infections. Thirty-two patients completed the study. Fifteen of 16 patients treated with netilmicin and nine of 16 treated with cefoperazone were cured. This difference was statistically significant (p = 0.037). No serious side effects or toxicity occurred with either drug, although five patients developed diarrhea after treatment with cefoperazone.

Urethral syndrome (abacterial cystitis)--search for a pathogen.

Thirty-one women with the recurrent urethral syndrome (abacterial cystitis) had extensive microbiological, cytological and histological investigations. A Streptococcus species was isolated from the bladder aspirate of one patient, a wall-deficient Streptococcus species from two others and both a Lactobacillus species and Ureaplasma urealyticum from another. A possible microbiological cause was therefore identified in the bladder in only 4 of the 31 patients. A Lactobacillus species was isolated from the bladder aspirate of one control subject. Lactobacilli were grown in the voided urine of the majority of patients and in seven of the eight control subjects. Chlamydia trachomatis and Herpes simplex were isolated from the cervix and the urethra of one patient. Urethral cytology was normal in all patients. Trigonitis was noted at cystoscopy in 26 of the 31 patients. Bladder biopsies showed squamous metaplasia in 15 and lymphocytic infiltration of the lamina propria in 29, giving support to an inflammatory aetiology of this enigmatic syndrome.

Spontaneous hemorrhage of the kidney.

A young hypertensive man with hypertensive nephrosclerosis presented with an acute abdominal emergency due to a spontaneous renal hemorrhage. Investigations demonstrated a large left perinephric hematoma. This was managed conservatively. With control of his hypertension, renal function has remained stable.

Calculation of effective renal plasma flow using 125I-orthoiodohippuran: comparison of four methods.

Fifty-three determinations of effective renal plasma flow (ERPF) were made on 21 patients with a wide range of renal function. Serial plasma samples for 6 hours after the injection of 125I-orthoiodohippurate were used to plot the plasma disappearance curve, and a standard ERPF was calculated using biexponential analysis of the curve. ERPF values calculated by 3 published approximation methods using limited plasma samples were compared to the standard values. All approximation methods overestimated the ERPF, the mean percentage error lying between + 18.3% +/- 20.4% and + 70.0% +/- 51.3%. With 2 of the 3 approximation methods the overestimation was particularly prominent in patients with moderate to severe renal insufficiency (ERPF less than 300 ml/min). For research purposes the more accurate and reliable biexponential method of analysis of ERPF is recommended.

Acute renal failure and polymyositis: case report.

Acute renal failure is a rare complication of acute polymyositis. We describe the first case noted in New Zealand and demonstrate the occasional serious nature of this disease and the role of corticosteroids in its management.

Comparison of single dose netilmicin with a five-day course of co-trimoxazole for uncomplicated urinary tract infections.

Women with uncomplicated urinary tract infections were randomly allocated to either a single 150 mg intramuscular dose of netilmicin or a standard five-day course of oral co-trimoxazole. Twenty-one of 22 were cured with netilmicin and all 20 with co-trimoxazole. No patient treated with netilmicin developed any side effects or obvious toxicity. Following co-trimoxazole one woman developed a severe skin rash and another nausea. Netilmicin is another drug which is highly effective when used in a single dose regimen for the treatment of uncomplicated urinary tract infections. There are many advantages of this approach to the management of a common clinical problem.
