The rare presentation of hyperthyroidism caused by an intrathoracic goiter with a normal-sized cervical thyroid gland is described. The toxic intrathoracic goiter demonstrated avid uptake of [131I] and [99mTc]pertechnetate, with comparatively faint isotopic accumulation seen in the cervical thyroid. A chest roentgenogram and radioisotope scan should be mandatory in cases of hyperthyroidism having no cervical thyroid enlargement to explore the possibility of a toxic intrathoracic goiter.

A computer-assisted radionuclide perfusion study was performed in 25 cases of solitary 'cold' thyroid nodules to assess their vascularity relative to that of normal adjoining thyroid tissue. Hypervascularity was seen in 3 malignant nodules; 1 follicular adenoma also showed increased vascularity. The 21 nodules demonstrating equal vascularity or avascularity with respect to normal thyroid tissue were benign in nature. Computer-generated time-activity curves were helpful in distinguishing hypervascular from equally vascular nodules. This technique, in conjunction with clinical evaluation, can be of value in deciding whether patients should undergo surgery or receive conservative management.
Liver glycogen was estimated after feeding heavy metals to rats. With the exception of copper (CU) and zinc (Zn) these metals i.e. lead (Pb), mercury (Hg), cadmium (Cd), chromium (Cr), manganese (Mn), molybdenum (Mo) and cobalt (Co) depleted liver glycogen. These results are discussed in relation to glycogenolysis, gluconeogenesis, hypo- and hyperglycemic conditions. The role of the responsible enzymes is also discussed.

Surface areas of the septum pellucidum, corpus callosum and the brain were measured from mid-saggital slices obtained with magnetic resonance imaging in 18 patients with schizophrenia and an equal number of normal volunteers. The patients showed larger septa and septo-brain ratios. These indices correlated with age in both patients and controls. In the patients, septal area also correlated with duration of illness. Changes in the septum pellucidum were unrelated to the intensity of positive and negative symptoms of schizophrenia. The patients had significantly longer corpora callosa, but the two groups did not differ on its size or the ratio between the latter and the mid-saggital slice area. Dimensions of the corpus callosum did not correlate with age, duration of the illness or positive and negative symptoms of schizophrenia. The anteroposterior elongation of the structure seemed to accompany enlargement of the septum pellucidum.

The alterations in the distribution and activity of certain key enzymes, viz. alkaline phosphatase, acid phosphatase, glucose-6-phosphatase, cholinesterase and lipase, have been determined in the liver of rats (Rattus rattus albino) after experimental poisoning with hexavalent chromium. The histochemical and biochemical observations presented herewith provide visual evidence of chromium-induced inhibition of all these enzymes except lipase, which was found to be stimulated insignificantly. The results have been interpreted in terms of changes in the micro-environment of the cell, formation of apo-enzymes, metal-protein complexes, oxidative phosphorylation and finally with liver function.

Two patients with lithium-responsive schizophrenia are described. Lithium withdrawal, done to evaluate the efficacy of lithium, triggered schizophrenic relapse, an infrequently reported phenomenon. Leonhard's comprehensive classification may help identify lithium-responsive forms of schizophrenia.

The necropsy and echocardiographic (echo) measurements of left ventricular (LV) wall thickness differ from each other. To investigate the reasons for this disparity, the diastolic and systolic LV thicknesses were correlated by echo and necropsy measurements in 41 patients. The diastolic LV wall thickness (12.7 +/- 2.8 mm) was significantly less than the systolic LV wall thickness by echo (16.7 +/- 3.1 mm; p less than 0.001) and the necropsy LV wall thickness (15.9 +/- 3.5 mm; p less than 0.001). The systolic measurements of the LV wall thickness by echo correlated well and did not differ from the necropsy measurements of the LV wall thickness (difference not significant). Thus, disparities exist between the echo diastolic LV wall thickness and the necropsy LV wall thickness, as the hearts are in the systolic phase of the cardiac cycle. The echo systolic LV wall thickness measurements do not differ from the necropsy LV wall thickness measurements, and the former are predictive of necropsy LV wall thickness.
Two paranoid schizophrenic young men murdered their mother and father while acutely delusional. To delineate the characteristics of homicidal, particularly parricidal patients, these two patients are compared with one parricidal and six homicidal patients. Preliminary warning signs, the contributory role of family life to the psychopathology, and the occurrence of parricide in schizophrenia are discussed.
Double vertebral artery arising from the first part of the right subclavian artery was found in one of the 80 cadavers. The course and characteristics of the artery are described highlighting the possible applied importance in certain clinical and surgical conditions.
