Student health during overseas electives.

Ill health is often encountered by travellers. As medical students are encouraged to spend their electives overseas, it was considered worthwhile to study the morbidity experience of University of Bristol medical students and to see whether any additional preventive measures could be identified for them. The findings suggest that the quality of advice could be further improved for medical students undertaking their electives abroad. Additional preventive measures were suggested by the students themselves. The suggestions may also be useful for other groups of travellers.

The synthesis of 15N- and deuterium-substituted, spin-labeled analogues of NAD+ and their use in EPR studies of dehydrogenases.

Two spin-labeled analogues of NAD+ were synthesized with an 15N and perdeuterated nitroxide radical, 4-amino-2,2,6,6-[2H17, 15N]tetramethylpiperidone-1-oxyl, which was attached to either the C-6 or C-8 position of the purine ring. The EPR spectra of these derivatives exhibit an approx. 6-fold increase in sensitivity compared with the corresponding 14N, protonated analogues due to a decrease in both the number of nuclear manifolds (from three to two) and the linewidth. The enhanced spectral resolution obtained with (2H17, 15N)spin-labeled-NAD+ analogues has facilitated simulation of the EPR lineshape of the nucleotide bound to lactate dehydrogenase (L-lactate:NAD+ oxidoreductase, EC 1.1.1.27). The spin-label moiety exhibits highly constrained motion indicative of a single environment. The motion of the spin label does not reflect the overall motion of the enzyme; rather, it is characteristic of some limited mobility relative to the lactate dehydrogenase. By contrast, the spin label on the membrane-bound enzyme, D-beta-hydroxybutyrate dehydrogenase (D-beta-hydroxybutyrate:NAD+ oxidoreductase, EC 1.1.1.30), is completely immobilized and exhibits two distinct spectral components for spin-labeled NAD+, which appear to differ in the polarity of the environment of the nitroxide.

Reliability of a test of cardiovascular fitness.

The reliability of a simple, cheap and portable test for estimating cardio-respiratory fitness (the Canadian Home Fitness Test) was assessed by the test/retest method. Substantial intra-subject variation was demonstrated which was between 49% and 85% of the observed group variation. At the 95% confidence level, this intra-subject variation represented an error of between 19.2% and 25.9% on the key measurement (mean post-exercise heart rate) depending on the degree of work done. The probable reason for this poor repeatability was observer error and this is discussed. It is concluded that this test is not suitable for either clinical or population research and alternative methods, such as historical review of physical activity, may be more useful.

Assessment of undergraduate general practice teaching.

Final-year medical students at the University of Bristol were asked to outline a plan of management for an elderly female patient who presented with several problems. These problems were presented in the form of a 'patient management questionnaire'. This questionnaire was completed before and again after a 9-week course that included a 3-week attachment to two general practitioners. The patterns of answers were compared to look for modifications in the plan of management that could be attributed to the teaching. The results indicate a broader plan of management after the course with more students considering the physical, social and family needs of the patient, rather than the clinical problems alone. Nevertheless, further experience of this method of assessment is needed before the technique can be considered a useful tool in the evaluation of general practice teaching.

Intramolecular hydride transfer of a combined coenzyme-substrate analog by D- and L-lactate dehydrogenases.

The synthesis of 3-[(3-carboxy-3-oxopropyl)aminocarbonyl]pyridine adenine dinucleotide, a new combined analog of NADH and pyruvate with pyruvate covalently attached to the amide nitrogen atom of the dihydronicotinamide ring via an additional methylene group, is described. In the presence of D-lactate dehydrogenase from Limulus polyphemus, from Lactobacillus leichmannii, and L-lactate dehydrogenase from pig skeletal muscle a redox reaction takes place between the pyruvate moiety and the dihydropyridine ring of the analog. This reaction is shown to be intramolecular by competition experiments with pyruvate. Degradation of the reaction products reveals that the carbon-2 atom of the formed lactate side chain exhibits D configuration in each of these cases studied.
