An evaluation of introductory courses in environmental health.

Introductory courses in environmental health are intended for doctors newly appointed to the emergency duty rotas of the Medical Officer for Environmental Health. Three courses have been held in Bristol between 1984 and 1987. They have been evaluated with a problem management questionnaire completed by 44 of the 47 participants before and after the teaching. Seventy-four per cent (23/31) of a comprehensive list of aspects that could be considered were mentioned by less than 30% of respondents at the beginning of the course and 65% (20/31) of these aspects were mentioned by at least 30% more participants at the end of the course than before it. There were no differences in these patterns between respondents who attended the different courses. The findings support the need and educational worth of such courses, and help to identify learning deficiencies. Problem management questionnaires are useful in their evaluation.

Interaction of glyceraldehyde-3-phosphate dehydrogenase with AMP as studied by means of a spin-labeled analog.

The binding of a spin-labeled AMP analog to tetrameric glyceraldehyde-3-phosphate dehydrogenase from rabbit muscle is described. The spin label, perdeuterated and 15N-substituted 4-amino-2,2,6,6-tetramethylpiperidine-1-oxyl, was attached to C-8 of AMP (C8-SL-AMP). Up to 8 equivalents of C8-SL-AMP bind per enzyme tetramer, i.e., 2 per monomer. Combining sites are the adenine subsite of the coenzyme-binding domain and the phosphate site. Glyceraldehyde 3-phosphate causes a conformational change in the enzyme that brings C8-SL-AMP molecules bound to adjacent R-axis-related subunits closer to one another by 0.2-0.3 nm and allows for spin-spin interaction between the nitroxide radicals. Similar, but less pronounced structural changes take place upon lowering the pH from 8 to 7. Addition of a single equivalent of NAD+ to a complex of the enzyme with 7.6 equivalents of C8-SL-AMP leads to the release of almost 4 C8-SL-AMP molecules. This supports our previous findings that binding of just one NAD+ molecule induces conformational changes in all four subunits.

Are malignant melanoma time trends explained by changes in histopathological criteria for classifying pigmented skin lesions?

The incidence of malignant melanoma in developed countries has been rising rapidly in recent years. Its causes and the reasons for this trend are uncertain. Changes in histopathological criteria for classifying pigmented skin lesions could explain the rising incidence rates. With support from the International Agency for Research on Cancer, this hypothesis is explored but not substantiated for time trends in the South Western Region, United Kingdom.

[Unstable stress profile--artefact or physiologic phenomenon?].

In 31 women with stable urethral closure pressure at rest the stress urinary profile (SUP) was found to be unstable. The reason was dislocation of the urethra during stress by between 1 mm and 8 mm. Of 14 women with stable SUPs, ten showed no dislocation whereas four had dislocation not exceeding 2 mm. A retrospective analysis revealed that dislocation had occurred in 40 of 114 patients (35.1%). SUPs with signs of dislocation should therefore be interpreted with care.

Can children's paintings of their doctors be interpreted?

There is increased emphasis nowadays on the role of the general practitioner in preventive care for children. Accordingly, an analysis of the child's perception of the doctor and of the factors which influence and mould that perception are important. In New Zealand children's paintings have been used for such analyses. The present study looked at children's paintings of their family doctors in a Devonshire village. The findings show that boys are more likely than girls to depict their doctors as a gruesome or threatening character, and suggest that medical contact has little, if any, influence on character portrayal of doctors. Other factors that might influence character portrayal and are now thought worthy of detailed study, include television viewing patterns, personality, age, home background and artistic ability.

Health risks of snorkel swimming in untreated water.

There are certain health risks associated with recreational exposure to bacteriologically polluted water. In this study, 27% (21/77) of swimmers who took part in a snorkel swimming event, experienced gastrointestinal symptoms within 48 hours of entering the untreated water. The incidence of these symptoms was statistically significantly greater than the incidence of such symptoms reported by two control populations (p less than 0.001), but not significantly different to 25% of people who had experienced gastrointestinal symptoms in a previous snorkel event in the same water environment. As the water quality came within the confidence intervals of the EEC standards for bathing water the adequacy of these standards needs further appraisal.
