[Ultrasonic diagnosis of space-occupying lesions of the kidney (author's transl)].

In 140 patients who were suspected of having space-occupying lesions of the kidney the ultrasound findings were analysed. There were no false negative results. Out of 4 false positives two could be explained by nontumourous focal renal disease. In a further 10 cases a space-occupying lesion could not be excluded with certainty on the grounds of the ultrasound investigation alone. Out of a total of 96 space-occupying lesions (48 solid tumours, 48 cystic processes) 83 were correctly diagnosed sonographically.

[Plasmocytoma. A clinical and statistical survey (author's transl)].

Analysis of the case-records of 61 patients with plasmocytoma revealed an equal sex-distribution with a mean age of 58.6 years. The most common initial symptom of the disorder was ossary pain. The diagnosis was confirmed by bone marrow cytology, demonstration of paraproteins in serum and urine and the characteristic radiological changes. Renal insufficiency, immunodeficiency syndrome, fractures and neurological defects complicated the clinical course in many patients.

Possibilities and limitations of ultrasonic diagnosis of space occupying lesions in internal medicine.

Possibilities and limitations of ultrasonic tumour diagnosis in internal medicine (thyroid gland, liver, pancreas, kidney, malignant lymphomas) are discussed on the basis of a five years experience with about 4,000 examinations a year. A real time and gray-scale technique is used. The accuracy of the presented ultrasonic findings is proven by comparative studies. Besides the well known advantages of ultrasonography the independence from contrast medias must be stressed in comparison to diagnostic radiology. The main limitations of ultrasonography are the impossibility of diagnosing tumours smaller than 1.5-2 cm and the absence of an ultrasonic pattern typical of malignancy. To establish a morphobiological diagnosis, ultrasonically guided fine needle biopsy has proved to be a reliable method.

[Ultrasonography of pancreatic diseases (author's transl)].

The use of a modern ultrasonic equipment with real time and gray-scale technique enables a riskless and less time consuming examination of the pancreas. The accuracy of ultrasonography in the diagnosis of pancreatic diseases is demonstrated by a clinical study in comparison to the endoscopic retrograde pancreaticography. By ultrasonography it is not only possible, to detect an enlargement of the pancreas, but also to differentiate between the various diseases of the pancreas in about 75% of the cases. Follow up controls in acute pancreatitis and the simultaneous examination of the neighbouring organs in all cases of a pancreatic disease are an important aid for the decision of surgical treatment. The ultrasonic targeted fine needle aspiration biopsy seems to be a suitable method to establish a morphological diagnosis in certain pancreatic diseases too.

[Sonographic thyroid diagnosis].

Sonography is suitable for the examination of thyroid glands respectively goiters. This method particularly gains in the differentiation of cold nodules or cold regions. In more than 50% of patients with cold nodules sonography showed cystic lesions. In addition, ultrasonically guided percutaneous fine needle biopsy is employed for emptying cysts or gaining material for cytological examination.

[The diagnosis of gall-stones by ultrasonics (author's transl)].

Ultrasonic and radiological tests were compared in 267 patients suspected of having gall-stones. Radiological diagnosis was not possible in 20, because of lack of opacification, while in nine a gall-bladder was not demonstrable by ultrasound. In 226 the two tests coincided (positively or negatively), with 21 negative cholecystograms being interpreted as proof of stone. Seven of twelve discordant results were proven at operation: in three the ultrasonic, in four the radiological diagnosis was confirmed. Because of its accuracy, simplicity and absence of any risk ultrasonics is recommended as the first test to be performed if cholelithiasis is suspected.

[Fine needle aspiration biopsy of abdominal and retroperitoneal tumours under ultasonic guidance (author's transl)].

In 94 patients with tumours of the abdomen or the retroperitoneal space detected by ultrasonography fine needle aspiration puncture was performed under ultrasonic control. The aspirated material was stained with Pappenheim's solution for cytological examination. In 86 patients the diagnosis was definitely established. Of 49 patients with malignant growths (liver 35, kidney 9, colon 2, subcutaneous tissue 2, pancreas 1) tumour cells were gained in 40. In 4 other pathients the cytological findings were suspicious of malignancy. In 37 patients with benign processes (liver 35, subcutaneous tissue 2) no false-positive diagnosis was made. Complications induced by the puncture, especially hemorrhage were not encountered. The combined ultrasonic-cytological examination is simple, fast, almost riskless and well-tolerated. It allows to avoid other more expensive methods to obtain tumour material which usually carry some risk. It is especially valuable as a screening method for malignant growth in the abdominal cavity and the retroperitoneum.

[Efficacy of ultrasonography in pancreatic disorders compared to endoscopic retrograde pancreaticography (author's transl)].

To evaluate the diagnostic efficacy of ultrasonography in pancreatic disorders the results were compared with endoscopic retrograde pancreaticography in 140 II resection-retrograde cannulation of Vater's papilla was unsuccessful, examination of the pancreas was not possible in 20 patients with ultrasonography because of meteorism of severe adipositas. In 15 subjects the sonographic findings were equivocal. Out of 105 ultrasonic findings three were false-negative and 5 false-positive, whereas retrograde Wirsungography resulted in 9 false-negative and 2 false-positive interpretations. In 50 out of 67 cases of pancreatic disorders diagnosed by ultrasonography a further differentiation between malignant tumours and benign pseudotumours (pancreatitis or cyst) was possible. With respect to the diagnosis of pancreatic cysts ultrasonography was superior by 23 of 27 to retrograde pancreaticography (12 out of 27) since many cysts do not communicate with the ductal system. In comparison with the endoscopic method the risklessness and the low expenses have to be stressed. Both methods supplement each other, so that a high diagnostic accuracy is reached (more than 95 per cent).

[Results of therapy in acute leukemia in adults. Comparison of different combinations of cytotoxic drugs].

In 67 patients with acute leukemia 76 treatment-series were performed which were analyzed to evaluate whether chemotherapy has brought some progress during the past years. With 67 sufficient treatments a total of 26 remissions were achieved. VIDaP-scheme with 43 p.c. and cytosinarabinosid with 46 p.c. were significantly superior to the older scheme methotrexat-purinethol-prednisone with only 29 p.c. remissions. A remarkable deterioration of prognosis with increasing age rises the question whether treatment with cytotoxic drugs should be tried in patients more than 60 years old. Remission rate in patients below 20 was especially high with 71 p.c. so that the well-known good prognosis of juvenile leukemia can be extended with some limitations until age 20.

[Clusters of Hodgkin's disease. Case report of two brothers].

Case report of two brothers who suffered from lymphogranulomatosis successively. Familiary clusters are rare and are in favour of infectious and of hereditary factors in the pathogenesis of Hodgkin's disease. The own observation is discussed in reference to the world literature.

[Carbohydrate administration and electrolyte balance in patients under intensive care].

A twenty-four percent solution of carbohydrates 0.2 g/kg/h consisting of fructose, xylit and glucose in a 2:1:1 ratio and an aminoacid solution 0.03 g/kg/h were given to 13 patients of the internal medicine intensive care unit. There was an unimportant loss of carbohydrates, only, with but few exceptions, i.e. a loss of 11% in 3 patients. The results demonstrate that the carbohydrates of the combined solution become utilized independent of each other. The balance of water, sodium, potassium and magnesium were not only negative but also positive. The elimination of calcium was between 1-19 mval/24 h, of phosphate between 0.6-8.7 mval/24 h.

[Ultrasonic diagnosis of liver metastases (author's transl)].

166 patients suspected to harbor metastases in the liver were examined by ultrasonics. The results of this examination could be evaluated in 113 cases, yielding a correct diagnosis in 92%; in 53 cases with uncharacteristic changes of the liver the presence of metastases could be neither proven nor excluded, thus the overall yield of correct results was 72%. In 124 patients ultrasonic findings could be compared with scintigraphic findings (99m-Tc-sulfur colloid): A false diagnosis was made on the basis of ultrasonic findings in 5 cases, on the basis of scintigraphic findings in 18 cases; no conclusive diagnosis could be made by ultrasonic procedures in 34 cases, by isotope scanning in 48 cases. The ultrasonic diagnostics has the advantage of low cost and no risk and allows to differentiate between solid and cystic lesions.
