Products for Alzheimer's self-stimulatory wanderers.

The objective of this study was to develop a variety of sensory stimulation products for the behavioral intervention of patients with Alzheimer's type dementia. Many caregivers have relied on physical and chemical restraints as the primary method of patient intervention due to the lack of appropriate dementia management products. This significantly lowers the sufferer's quality of care and life. As the age group most susceptible to Alzheimer's disease (65 and older) is the fastest growing segment of our society, an appropriate care solution must be sought. The specific aim of this study was to develop products that are sensory satisfying for the Alzheimer's patient that exhibits self-stimulatory wandering behavior. Sensory satisfying objects for product development would be determined through structured observations of self-stimulatory wanderers in an institutionalized setting. Variations of product design and mounting would be pursued in order to develop products that are not only safe and effective for patient use, but are easy for the caregiver to implement and maintain. Such products would have widespread commercial application in both the institutional and private care settings such as nursing homes, adult day care facilities, Alzheimer's care facilities, convalescent homes, mental health institutions, and assisted-living facilities.

Pentamidine. A risk-benefit analysis.

With the advent of the acquired immunodeficiency syndrome (AIDS), the therapeutic importance of pentamidine isethionate has greatly increased. This review summarises its pharmacology, its toxicity and clinical experience in the treatment of Pneumocystis carinii pneumonia (PCP). Data are conflicting as to whether pentamidine is more or less effective than cotrimoxazole (trimethoprim-sulfamethoxazole) for the treatment of PCP in individuals with AIDS, but due to its toxicity and expense, it is considered as second-line therapy by many authorities. Hypoglycaemia has been encountered in up to 27% of treatment courses with pentamidine, and nephrotoxicity in 25%. In an attempt to circumvent the toxicities associated with parenteral administration, aerosolised delivery has been evaluated for both therapy and prevention of PCP. Aerosolised pentamidine, on the basis of early clinical results, convenience and low toxicity, may become the drug of choice for prevention of PCP in individuals at high risk. However, its role in the treatment of PCP remains to be defined. Preliminary studies suggest that it is effective, but the data are insufficient to support its use outside of clinical trials.

Cryosurgery in the treatment of giant cell tumors of bone: a report of 52 consecutive cases.

Fifty-two cases of giant cell tumor of bone have been treated by cryosurgery--an extensive freezing of residual tumor after curettage. Cryosurgery is performed by direct pouring of liquid nitrogen into the tumor cavity through a funnel. The cavity is filled with methylmethacrylate and corticocancellous onlay grafts until peripheral bone regeneration occurs to provide bone stability and prevent postoperative pathologic fracture. Patients with lesions in a weight bearing bone are placed in a long leg ischial weight bearing brace until sufficient healing has taken place. Rebiopsy (a second stage diagnostic procedure) is performed 3-6 months after the original cryosurgery. By comparison of pathology, results and complications between our first series of 25 cases and the additional 27, we have observed only one frank malignant giant cell tumor (1.9% incidence). This is much lower than the previously reported 16% fully malignant complication rate, and may be the result of the rapid elimination of the giant cell tumor by cryosurgery.
