Traumatic intracerebellar hematomas.

Seven patients with "isolated" traumatic intracerebellar hematomas due to closed head injuries are presented. Computerized tomography parameters useful in the correct management of these hematomas include: location (hemisphere or midline), size, and appearance of the posterior fossa cisterns (intact or obliterated). Three patients with hemispheric hematomas were treated conservatively and recovered. One patient with a midline hematoma was managed conservatively and died. Three hematomas (one hemispheric and two midline) were operated on. The immediate results of surgery were good, but two patients died later due to medical complications. The results of this study support the concept that large hematomas (3 cm or greater) generally need surgery and that hemispheric hematomas of limited size (less than 3 cm) may be treated conservatively. The clinical course of prognosis of midline hematomas is grave, and evacuation of accessible hematomas should be considered early in the course of the disease.

Benign traumatic intracerebellar hematoma.

Prompt surgical intervention is thought to be necessary in patients with traumatic intracerebellar hematoma. The case reported here ran a benign course without operation. Pertinent serial computed tomographic scans are presented. It is concluded that not all traumatic hematomas of the cerebellum require operations.

Microsurgical removal of paraventricular cavernous angiomas. Report of two cases.

Two cases of paraventricular cavernous angiomas are presented. In one, the cavernous angioma was found in the right wall of the fourth ventricle, and in the other in the right thalamus encroaching upon the third ventricle. Both patients had onset of symptoms suggesting a tumor. Good results were obtained by the microsurgical approach to these malformations. The computerized tomography findings typical of cavernous angiomas are reviewed

Cerebral cavernous angiomas in children.

Five cases of cerebral cavernous angiomas in children are presented. Three children had seizures as the sole manifestation of the lesion, and two had intracerebral bleeding. These malformations are encountered more commonly in adults in the third to fifth decade, and are found most frequently in the white matter of the cerebral hemispheres. Intracranial bleeding, seizures, headaches, and slowly developing focal signs are the usual presenting complaints. In childhood, seizures are the initial symptom of the malformation in many cases. The value of computerized tomography (CT) in the detection of such malformations is stressed, and CT findings that are characteristic of cavernous angiomas are described.

Fracture of the distal radius with ulnar nerve palsy.

A patient with immediate palsy of the ulnar nerve secondary to a distal fracture of the radius and possibly a dislocation of the distal radio-ulnar joint (Galeazzi's fracture) is described. Surgery demonstrated a serious contusion of the nerve, scarring around the nerve, and thrombosis of the ulnar artery. Symptoms rapidly disappeared after neurolysis.

Spinal epidural hemangiomas.

A case of spinal epidural hemangioma is presented. The rarity of this lesion in comparison with the more frequent vertebral hemangiomas with secondary extension to the epidural space is emphasized. The special features of this case are noted: the acute clinical onset, the visualization by means of the spinal angiography, and the unusual cervical level.

The treatment of spasticity by means of dorsal longitudinal myelotomy and lozenge-shaped griseotomy.

This report describes the evolution of techniques of spinal surgery developed for the treatment of spasticity of varied etiology and further describes application of a T-shaped dorsal longitudinal myelotomy to five patients. Additionally, a losenge-shaped griseotomy was performed on three patients. Both techniques led to the elimination of hypertonicity and painful spasms, without producing sensory deficits. Although all procedures are performed at lumbar levels, some patients showed reduced spasticity in the neck or upper extremities as well.

An automated method for evaluation of the thyroid uptake of 99mTc-pertechnetate.

A method for evaluating the thyroid uptake function, based on the work of De Garreta, Williams, Glass, Atkins, Higgins et al., was developed using 99mTc-pertechnetate and a scintillation camera connected to a minicomputer system. The method has the advantages of a single visit, a low-dose examination, and the reliability of a full automated technique. The clinical results of the studies performed to date are in close correlation with those obtained from the radioactive iodine uptake test, and in good agreement with the findings of the above-mentioned authors.

Superior cerebellar artery aneurysms. Case report.

The authors report a case of left superior cerebellar artery aneurysm successfully operated. A review of the literature produced only 7 other reported cases. Symptoms and operative procedures are briefly discussed.
