[Treatment of advanced carcinoma of the prostate with Estracyt (author's transl)].

40 patients with prostatic carcinoma were treated with parenteral and/or oral Estracyt (estramustine phosphate) until 55 months. Metastases were present in 37 patients (stage D). 35 of the 40 patients developed metastases in spite of estrogen therapy and/or orchidectomy. Diminution of metastasic bone pain as well as improvement of hydroureteronephrosis was frequently observed. Paraplegia secondary to metastatic disease improved in 1 case for 6 months. Side effects were relatively rare and were mainly gastrointestinal. A possible hepatotoxic action of the compound has been pointed out previously. On the basis of our studies Estracyt is recommended in the treatment of primary estrogen resistent prostatic carcinoma and in metastatic carcinoma of the prostate not responding to conventional antiandrogenic therapy anymore.

[Treatment of urinary tract infections caused by unknown bacteria (author's transl)].

The treatment of acute febrile urinary tract infections in urological patients has to start in most cases without knowledge of bacteria and their sensitivity. The results of 2,000 antibiograms and clinical observations showed good efficiency in initial therapy if combined treatment with gentamycin and cephalothin was used. Using recommeneded doses in permanent control of serum creatinine and blood-urea nitrogen, no decrease of renal function was observed in 131 patients treated with this combined therapy.

Late results following cryosurgery of the prostate in 111 high-risk patients.

The so-called cryoprostatectomy was performed on 111 high-risk patients from November 1970 to December 1975 (60-month period). 86 patients (78.5%) had an adenoma and 25 patients (21.5%) a carcinoma of the prostate. During this period of time 1,085 more patients with adenoma or carcinoma were treated by open prostatectomy or TUR. Experience has shown that cryoprostatectomy should be performed only on high-risk patients whose physical condition excludes operation and the only alternative would be continuous urethral catheter drainage. Any further indication must be rejected.

[Determination of L-cystine aminopeptidase (serum oxytocinase, EC 3.4.1.2) during normal pregnancy].

It is given a method to the determination of cystine aminopeptidase activity in the serum in pregnancies. A time of few minutes is necessary for one determination. The determination is performed with a spectralphotometer or with the Reaction Rate Analyzer (LKB 8600) in using the chromogen substance 1-cystine-bis p-nitroanilide. This substance is solved in dimethylformamide. The normaly range of the 8th to the 41 th week of pregnancy is determinated in a number of 453. In twins the values were higher. The cystine aminopeptidase is nearly specifically in pregnancies and is produced in the plazenta. It is a statement of cellulary activity in the plazenta.

Effects of pressoreceptor stimulation on micturition relfex and urethral pressure profile in rabbits.

The influence of pressoreceptor stimulation on the micturition reflex and urethral pressure profile was studied. Stimulation of pressoreceptors of the aortic arch and carotid sinus causes an increase of parasympathetic tone and a decrease of sympathetic tone. Striated muscular tone was also decreased. Pressoreceptor stimulation was simulated by electrical stimulation of the depressor nerve in the rabbit. Our studies show that with pressoreceptor stimulation micturition occurred with 24 per cent less bladder filling and 32 per cent less intravesical pressure than before stimulation, and urethral pressure profile showed a 17 per cent decrease of the maximum and mean urethral pressure. The interpretation of the action of parasympathicomimetic and sympathicomimetic drugs on bladder and bladder neck must take into consideration the cardiovascular effects of these drugs and therefore the effect on the pressoreceptors.
