The clinical syndrome resulting from deletion of 20p in combination with duplication of 10p, seen in two male members of the same family, is reported for the first time. The sister of one and the mother of the other patient are asymptomatic carriers of a balanced translocation. The two patients have multiple stigmata including dwarfism and pronounced psychomotoric retardation. The syndrome corresponds largely to an addition of the symptoms seen in cases with deletion of 20p and duplication of 10p reported hitherto.
We report on a female patient who received a polychemotherapy because of an extensive pemphigus vulgaris. Under this therapy, the general condition of the patient worsened. Noncharacteristic symptoms, such as dyspnoea and tachycardia occurred. In spite of comprehensive, also invasive diagnostics, pulmonary foci demonstrable by X-ray morphology and computer tomography could not be specified. In the course of treatment, a cerebral compression symptomatology developed. Post mortem, a pulmonary aspergillosis with abscess-formation in the brain was found. The antigen identification completed the diagnosis. The increasing significance of Aspergillus infections is pointed out. The results of clinical examination and the differential diagnosis as well as the possibility of the diagnosis of pulmonary aspergillosis are demonstrated.
An inexpensive and easy method of calibration of brachytherapy 137Cs tubes is described. Cesium brachytherapy sources are calibrated relative to an NIST-calibrated source. The detection system is a NaI detector and single channel analyzer with the window set on the photo peak.

Cellular distribution and localization of Ca2+ in bullfrog myocardial cells was investigated on the light and electron microscopical level using different preparation procedures. Fixation and dehydration in glutaraldehyde and ethanol solutions containing increased amounts of Ca2+ (5-25 mM) cause the formation of numerous electron-dense deposits at the external face of the sarcolemma. X-ray microanalysis demonstrates that the deposits are mainly composed of calcium. Similar results were obtained in cryopreparations by quick-freezing of fresh myocardial tissue. Granules with a diameter of 16.6 nm, which clearly exceed all other cell constituents in contrast, are found at the entire myocardial cell surface. Besides an increased amount of calcium the granules contain potassium and phosphorus. Fluorescence microscopy on single cells obtained from bullfrog atrium by treatment with digestive enzymes and stained with chlorotetracycline (CTC) as a fluorescent probe for membrane-bound Ca2+ clearly support the observations made on the fine structural level. A strong fluorescence signal depending on the CTC- and Ca2+-concentration of the incubation medium is observed at the sarcolemma and can be inhibited by addition of La3+. The results of the present investigation suggest that the surface coat of the sarcolemma of myocardial cells represents an important compartment for accumulation and release of external Ca2+. The external lamina which partly covers the surface of myocardial cells seems not to be involved in the Ca2+ exchange.
In the present investigation different finestructural and histochemical procedures were employed to demonstrate normal morphology and Ca2+-transport mechanism in bullfrog atrial myocardium. For normal morphology specimens were fixed in 1% OsO4, 2.5% glutaraldehyde or liquid propane at -185 degrees C before they were prepared for conventional embedding and freeze-etching, respectively. Special interest was focussed on the caveolae system, which is composed of single, spherical membrane invaginations (diameter, 85 nm), randomly distributed at the entire cell periphery. The caveolae enlarge the cell surface by 24.5% and occupy 8.5% of the cellular volume. The caveolae membrane contains a few intramembraneous particles with a diameter of 8.4 nm comparable to the size of ATPases as found in other cells. For histochemistry the specimens were first stabilized by treatment with 50% glycerol, 0.025% glutaraldehyde or 0.15% formaldehyde and then incubated in a medium containing 4 mM CaCl2, 4 mM EGTA, 5 mM MgCl2, 5 mM K2C2O4, 5 mM ATP and 20 mM histidine at pH 7.0. This incubation always succeeded in the formation of electron-dense deposits with elliptical shape, measuring 100-200 nm in length and 10-30 nm in diameter. According to X-ray spectra they deliver a characteristic calcium-peak and can be found within two different cellular compartments: in small invaginations of the sarcolemma, i.e. the caveolae-system, and in the intrafibrillar sarcoplasmic reticulum. The elliptical deposits can be clearly distinguished from round electron-dense granules measuring 16 nm in diameter, which are located within randomly distributed small vesicles and composed of potassium and phosphate. Contrary to the elliptical deposits the round granules are also present in controls and seem to be identical with the so-called atrial granules. In comparison to observations obtained with the same method in other muscular systems and derived from various control experiments the results of this study favour the existence of an ATP-dependent Ca2+-pumping mechanism in frog atrial muscle bound to both, the sarcoplasmic reticulum and the caveolae system.
OBJECTIVE: To verify the association between oxygen desaturation episodes and the dips in pH in infants with chronic obstructive respiratory symptoms. METHOD: Cross-sectional study with children 24 months old or younger hospitalized for investigation of chronic obstructive respiratory symptoms from 1997 to 1999. The patients underwent esophageal pH monitoring associated with transcutaneous oxygen saturation during the night. The patients were included in the study according to their need to be hospitalized and availability of equipment. The indices used to measure this association were reflux index, total number of refluxes, number of refluxes longer than 5 minutes, Euler index, ZMD index, 24-hour mean pH, and mean pH of desaturation. RESULTS: We studied 44 children. The mean age was 7.5 months, and 20% had desaturation below 93% during pH monitoring. We used the t test to compare the occurrence of desaturation with the pH monitoring parameters. We found higher significance with the reflux index (RI), number of episodes longer than 5 minutes, ZMD index, 24-hour mean pH, and mean pH of desaturation. The bivariate analysis, taking into account possible confounding factors and RI, showed PR equal to 6.61 (IC 95% 1.67 - 26.12) for an RI higher than 4%. CONCLUSION: Oxygen saturation monitoring associated with pH monitoring may be a useful tool to establish an association between GER and respiratory problems in patients with chronic or recurrent wheeze.
An outbreak of 25 cases of Andes virus-associated hantavirus pulmonary syndrome (HPS) was recognized in southern Chile from July 1997 through January 1998. In addition to the HPS patients, three persons with mild hantaviral disease and one person with asymptomatic acute infection were identified. Epidemiologic studies suggested person-to-person transmission in two of three family clusters. Ecologic studies showed very high densities of several species of sigmodontine rodents in the area.
Maintaining the professional knowledge and competency needed to practice in today's acute care settings is becoming increasingly difficult. This article introduces a continuing education program developed to assist staff nurses in meeting their learning needs. "Pathway to Excellence: Continuing Professional Education" is a peer education program or nursing education for nurses, by nurses, that can be implemented in all areas of clinical practice.

This paper presents results of a collaborative trial study (IUPAC project No. 650/93/97) involving 29 laboratories in 13 countries applying a method for detecting genetically modified organisms (GMOs) in food. The method is based on using the polymerase chain reaction to determine the 35S promotor and the NOS terminator for detection of GMOs. reference materials were produced that were derived from genetically modified soy beans and maize. Correct identification of samples containing 2% GMOs is achievable for both soy beans and maize. For samples containing 0.5% genetically modified soy beans, analysis of the 35S promotor resulted also in a 100% correct classification. However, 3 false-negative results (out of 105 samples analyzed) were reported for analysis of the NOS terminator, which is due to the lower sensitivity of this method. Because of the bigger genomic DNA of maize, the probability of encountering false-negative results for samples containing 0.5% GMOs is greater for maize than for soy beans. For blank samples (0% GMO), only 2 false-positive results for soy beans and one for maize were reported. These results appeared as very weak signals and were most probably due to contamination of laboratory equipment.
A definitive treatment is described for cervical dentinal sensitivity, featuring a semilunar coronally positioned flap to cover the denuded root surface. Many patients who complain of having sensitive teeth from exposed dentin secondary to gingival recession can be easily and conservatively treated with a semilunar flap procedure.
A polymerase chain reaction (PCR) was developed to differentiate the thickening agents locust bean gum (LBG) and the cheaper guar gum in finished food products. Universal primers for amplification of the intergenic spacer region between trnL 3' (UAA) exon and trnF (GAA) gene in the chloroplast (cp) genome and subsequent restriction analysis were applied to differentiate guar gum and LBG. The presence of <5% (w/w) guar gum powder added to LBG powder was detectable. Based on data obtained from sequencing this intergenic spacer region, a second PCR method for the specific detection of guar gum DNA was also developed. This assay detected guar gum powder in LBG in amounts as low as 1% (w/w). Both methods successfully detected guar gum and/or LBG in ice cream stabilizers and in foodstuffs, such as dairy products, ice cream, dry seasoning mixes, a finished roasting sauce, and a fruit jelly product, but not in products with highly degraded DNA, such as tomato ketchup and sterilized chocolate cream. Both methods detected guar gum and LBG in ice cream and fresh cheese at levels <0.1%.

Psychological differences were compared between 25 female patients with carcinoma of the breast (mamma) and 25 patients with mastopathia cystica fibrosa. Our methods consisted in psychoanalytical interviews and MMPI-tests. We investigated the early childhood-period, the object relationships, the structurs of the personality and the psychological stress before the beginning of the physical illness. Patients with carcinoma of the breast show more frequent broken home situations, and inclination to idealize the past, depressive traits and a tendency to repress inner conflicts. The sexual behavior shows statistically more frequent abortions in the cancer group. In the beginning of the cancer we found statistically more frequent the loss of a meaningful personal relationships (e.g. death of the mother). Several differences between the personal structure were also found in the MMPI-tests.
