[Psychodynamics of psychogenic somnolence (secondary narcolepsy). Case report].

The therapy of a patient suffering from secondary narcolepsy is described. The illness and the function of the symptom as a defense-mechanism are analysed. Sleep appears to be an ontogenetically archaic reaction to extreme--real and neurotic--dangers, which there is not possibility to escape from. The defense against extreme dangers is the withdrawal of all object-cathexis; following Freuds explanations this means: Sleep. In the patient the danger consisted of a regression to a narcisstic position: The symptom helped to avoid a separation of instincts in form of on the one hand narcisstic rage, on the other hand symbiosis. Taking into account the ideas of Spitz, the concept of fusion and separation of instincts is discussed.20

[Comparative, quantitative and morphological investigations (automatic image analysis--micromorphometric analysis) of myocardium of rat isoprenalin treatment (author's transl)].

The alterations of areas of myocardium of male rats (2, 4, and 6 months) are investigated after isoprenalin treatment (0.05 mg/kg body weight and day i.p. for 30 days). The investigations are carried out using automatic microscope picture analysis, morphometric point counting and resp. Both methods yield the same qualitative results: The effect of isoprenalin is dependent on age and yields for old rats (6 months) only very little morphometric alterations in the myocardium. The automatic microscope picture analysis is a very effective method of investigation (45 seconds/cross section).

[Real-time cross-sectional echocardiography using a mechanical sector scanner (author's transl)].

Ultrasonic one-dimensional echocardiography have proven extremely useful these last years in cardiac diagnosis. However, it allows only, structures lying along a narrow ultrasonic beam, to be visualized. In order to state more precisely the spatial relationship of the various cardiac structures, some two-dimensional systems, which displays the heart by constructing a plane image composed of many straight lines have been perfected. The authors, using such a system, specify its principal technical and clinical aspects and comment from iconographics documents and from their experience with more than 100 explorations, its advantages.

Development of Sertoli cell junctions in vitro--a freeze-fracture study.

Seminiferous tubules of 1-day-old rats were maintained in organ culture for up to 40 days. Five classes of intercellular junctions between Sertoli cells were observed by the freeze-fracture method as the tissue aged: (a) typical gap junctions; (b) focal tight junctions; (c) macular tight junctions; (d) meandering tight junctions; and (e) extensive tight junctions. The relative proportions of these types of Sertoli cell junctions were quantitated as the organ cultures progressed. The junctional structures observed and classified in organ culture were identical to those seen in vivo, but the timing of their appearance and/or disappearance, as well as their relative proportions, was different from that observed in the developing animal. Extensive tight junctions, with numerous parallel strands, were observed in the 40-day cultures; however, their oblique orientation with respect to the myoid layer was in contrast to the parallel orientation observed in vivo.

Structural and replicative forms of mitochondrial DNA from human leukocytes in relation to age.

The structure and replication of human leukocyte mitochondrial DNA (mtDNA) was investigated in healthy young adult males (23--37 years old), middle-aged males (42--52 years old) with secondary polycythemia, and elderly males (80--89 years old) who exhibited different degrees of age-related disease syndromes. The distribution of the various cell types within the white cell population was within normal limits in all samples. Total mtDNA was isolated in ethidium bromide--CsCl gradients and examined by electron microscopy after spreading by the aqueous and formamide techniques. The individual frequencies of catenated forms ranged from 2 to 6% but showed relatively little change (declining slightly) with age. The individual frequencies of circular dimers varied from 0 to 0.1% in the young adult and polycythemic groups and in 10 out of 12 elderly individuals. One elderly individual had a circular dimer frequency of 0.3% (including a circular molecular of tetramer size) and another had 4.5%. This finding suggest that agerelated cellular pathology may exist in the blood-forming system in some cases. The mode of replication of leukocyte mtDNA agrees well with that described for mouse L cells. There was no evidence of aberrant mtDNA replication as a result of aging.

Evaluation of three dry-sterilized hollow fiber artifical kidneys.

We investigated three new dry sterilized hollow fiber artificial kidneys (HFAK) (Cordis Dow CDAK 1.3, Travenol CF 1200, Extracorporeal Tri-Ex 1). Dry sterilization makes these dialyzers more economical by shortening set-up time. Dry sterilization also eliminates iatrogenic administration of residual sterilant. Water of imbibition can significantly increase the blood compartment volume of the dialyzer during dialysis. Consequently, a corrected blood volume for each dialyzer was established; these corrected volumes varied from 13--36% greater than the volume determined before dialysate flow. With low dose heparinization of these dialyzers there was between an 18 and 45% decrease in the post dialysis volume, presumably due to fiber clotting during dialysis. This volume added to the residual blood loss measured by a colorimetric technique accounted for a possible blood loss ranging between 26.9 and 53.9 ml per dialysis for these three dialyzers. Our results suggest that a relationship between dialyzer clotting and decreased dialyzer efficiency may exist. These three capillary flow dialyzers had a much lower platelet drop (0--9% pre to post) when compared to 30--40% drop of flat plate dialyzers. These dialyzers were found to be safe and easy to use but the high fiber clotting warrants further investigation in chronic dialysis patients on high dose heparin.

[Gastric xanthelasma (author's transl)].

A report is given about 42 cases with xanthelasma of the gastric mucosa diagnosed with 5 1/2 years. The condition was found (considering the total number of gastroscopies) in men 1.38 times as often as in women; the average age of all patients at the time of diagnosis was 62.5 years. Xanthelasma lesions are yellow or yellowish-white spots in the mucosa, consisting of foam-cells microscopically. The cytoplasma stains pale pink in a HE preparation; there is almost no staining with PAS, and sudan black staining is definitely positive in most cases. PAS and sudan black staining do allow differentiation of signet ring carcinoma cells. Gastric xanthelasma occurs quite often concurrently with chronic gastritis. It is due to local, not to general disturbances of lipid metabolism. Lipids probably are formed within the mucosa cells or may--in cases of intestinal metaplasia--originate from the gastric contents. Gastric xanthelasma does not constitute a nosologic entity, it is merely a consequence of other diseases of the gastric mucosa.

Physical and genetic studies with restriction endonucleases on the broad host-range plasmid RK2.

The cleavage map of the plasmid RK2 was determined for the five restriction endonucleases EcoRI, HindIII, BamH-I, SalI and HpaI. DNA has been inserted into several of these sites and cloned in Escherichia coli. Efforts to obtain derivatives of RK2 reduced in size by restriction endonuclease digestion of the plasmid were not successful and indicated that genes required for the maintenance of this plasmid in E. coli are not tightly clustered. An RK2 derivative possessing an internal molecular rearrangement was obtained by transformation with restriction endonuclease digests of the plasmid.
