[Myopathy with tubular aggregates. Clinical, biological and histological study of 2 cases].

Two male patients, aged 46 and 48, presented with muscle pain on exercise, generalized or limited to the upper extremities. Physical signs were limited to mild muscle wasting in one case. There was no myoglobinuria. Electromyography was normal in one case, and showed some polyphasic units in the other. An ischaemic lactate test was normal in the one case in which it was performed. Serum creatine kinase activity was normal or only mildly elevated. Histochemical and ultrastructural study of muscle showed tubular aggregates in type II fibers in both cases, and otherwise gave normal results. Symptoms were unchanged with a follow-up of 2 and 16 years, respectively. Tubular aggregates are found in a number of unrelated clinical and experimental conditions, which are reviewed. In these cases and in 3 similar patients, previously reported, tubular aggregates seem to be the hallmark of hitherto unrecognized metabolic disorders.

[Clinical analysis of 70 neuropathologic cases of multiple sclerosis].

A retrospective study of clinical files of 70 pathologically confirmed cases of Multiple Sclerosis (MS) (53 women and 17 men), selected from the records of the Laboratoire de Neuropathologie Charles Foix (Hôpital de la Salpêtrière) was performed. The following data were recorded and analysed by a computer program (HP 85): sex, age of onset of disease, clinical course (classified into Remittent, Remittent-Progressive, Progressive and Acute) and the date of each new neurological symptom or sign. The mean age of onset was 36.8 +/- 12. In women, the disease began earlier (34.6 +/- 12) and the duration was longer (17.4 +/- 12). In men the age onset was 40.6 +/- 11 and the duration was 12.5 +/- 6. In remittent courses, the mean age of onset was 30.8 +/- 13 and the duration was 21 +/- 10. In progressive courses, the age at onset was 45 +/- 10 and the duration was 2. In women, progressive courses began significantly later (42.3 +/- 9.2) and were shorter (15 +/- 8) than remittent courses which began at 26.8 +/- 8.2 and lasted 23 +/- 10. The histogram of the duration of clinical courses showed three groups: acute courses (8 cases less than 5 years long), intermediate courses (41 cases, between 5 and 20 years long), and long courses (21 cases longer than 20 years). Women were more often affected with acute (7/8 cases) or long courses (20/21 cases). The mean duration of the disease was the same when the symptoms and signs at onset were motor weakness, sensory disturbances, optic neuritis or diplopia.(ABSTRACT TRUNCATED AT 250 WORDS)

[Prolonged treatment of multiple sclerosis with average doses of azathioprine. An evaluation of 15 years' experience].

Since 1967, 211 probable or definite multiple sclerosis (MS) patients have received azathioprine (100 mg daily) as primary immunosuppressive treatment. Treatment results are evaluated in 145 patients who have received prolonged (one year or more) continuous therapy and whose disability had been evaluated prospectively. The main characteristics of these patients are the following: male 45, female 100; mean age of onset of MS: 27 years; mean duration of treatment and follow-up: 67 and 115 months, respectively. 48 patients were in the progressive phase of MS at the onset of treatment; 17 of these had a stable disability score during therapy; however, the number of stabilized patients decreased significantly with duration of treatment. Among 97 patients in the remittent phase of the disease, 22 had no further bout during treatment, 41 had bouts but had no increase in disability, and 34 were aggravated; in 14 of these, secondary progression occurred during therapy. Haematologic, cutaneous, digestive and infectious side-effects occurred but were always reversible. In the subgroup of 131 patients who had received no other immunosuppressive treatment than azathioprine and in whom information was obtained at the end of 1982, 10 cancers (all of them epitheliomas) were diagnosed. The frequency of malignancy in the patients followed for 5 years or more was 10 p. 100. Five patients died from cancer (4 of 6 deaths in the remittent group). These results are compared with those of other series. It is suggested that the use of azathioprine in MS should be restricted.

Birth order and risk of multiple sclerosis: are they associated and how?

By analyzing birth order distribution in 107 patients with multiple sclerosis. Conditionally to the number of siblings in the 107 families, patients' mean birth order is significantly greater than expected. The number of patients whose bith order is equal to one is significantly smaller than the corresponding expected value. The present finding is in contrast to recent results observed by Isager et al. (1980) and this potential association deserves to be further investigated.

[Treatment of multiple sclerosis with antilymphocyte serum: results of a pilot study on 50 patients followed-up over a 4-year period (author's transl)].

Intensive antilymphocyte serum treatment was given to 50 patients with severe, progressive multiple sclerosis for a period of 6 weeks. Conclusions were favourable as regards the possibility of using this treatment and its immunodepressive effect. Therapeutic efficacy is difficult to evaluate, but a favourable impression was obtained from the fact there was improvement and transitory stabilization in some of the patients. These results suggest that the study should be continued, using longer treatment, on more recent cases, and with a randomized control group.

Circulating immune complexes in multiple sclerosis. Relation to clinical and biological parameters.

Circulating immune complexes were looked for on 38 clinically definite MS (multiple sclerosis) patients compared to 35 other neurological patients and 26 healthy subjects. 29% of the MS sera and 8.6% of the other neurological sera were positive, whereas none of the control sera were positive. These differences are significant. Clinical status was analysed as regards the age at onset, the duration and course of the disease, the disability of the patients and the treatment they received. CSF (cerebrospinal fluid) parameters studied were pleocytosis, concentration of total protein, electrophoresis. The results suggest that IC (immune complex) are more frequent during the first 10 years of the MS disease, in patients with blood-brain barrier damage and in patients without oligoclonal IgG.

[Chloroquine neuromyopathy. One case in prophylactic maleriatherapy (author's transl)].

A case of neuromyopathy due to chronic chloroquine intoxication is reported. The neuromyopathy developped 9 months after malaria suppression therapy with chloroquine was started. The clinical picture was that of a peripherical neuropathy in the lower limbs and of a generalized myasthenic syndrome. Muscular biopsy showed typical pictures of "vacuolar myopathy". The metabolism of the drug was normal. The patient improved soon after chloroquine was discontinued.

Signs of interhemispheric disconnection in marchiafava-bignami disease.

We report three cases of Marchiafava-Bignami disease. The diagnosis was made on clinical grounds. Evidence of interhemispheric disconnection was found in all three cases. This included left hand anomia, left-sided apraxia, left-hand agraphia, and left-sided pseudoextinction to both visual and tactile stimuli. In two cases, there was a left-ear extinction on a dichotic listening test.
