[Aggressive emperipolesis in chronic hepatitis (author's transl)].

The penetration and destruction of hepatocytes by lymphocytes (aggressive emperipolesis) has been described and examined quantitatively in biopsy specimens of chronic hepatitis. In 48 cases of chronic persistent hepatitis, the aggressive emperipolesis was rare and, obviously, not of great importance: sometimes there was no emperipolesis at all. In 36 cases of chronic aggressive hepatitis, however, emperioplesis was always evident, usually even to a very high extent.

Immunoglobulin abnormalities in multiple sclerosis. Relation to clinical parameters: disability, duration and age of onset.

Cerebrospinal fluid (CSF) and serum from 35 pairs of multiple sclerosis (MS) patients were analysed as regards mononuclear pleocytosis, concentrations of total protein, immunoglobulin G and A and beta-trace protein, and kappa:lambda ratios, as well as the serum/CSF ratios of IgG and albumin. The disability of the patients differed, whereas the age and the duration of the disease were similar in each pair. Similar analyses were also performed on CSF and serum from 72 patients, who were subdivided according to age at onset and severity of the disease. The highest mean values of the CSF-IgG and the lowest mean values of the serum/CSF IgG ratios were found in the more disabled patients. CSF immunoglobulin abnormalities were encountered more often and were more pronounced in the patients with the most malignant course of the disease, i.e., in those with severe disability after a short duration of the disease (less than 10 yr) and in severely disabled patients with an early age at onset of the disease(less than 25 yr). Contrarily, normal mean values of CSF-IgG concentrations and serum/CFS/IgG ratios were found in the groups of patients without disability after a duration of the disease of 10 years or more, and patients without disability and an early age at onset of the disease (less than 25 yr). The observations indicate that the immune response is most vigorous in disabled patients with a short duration or with an early age at onset of the disease. MS patients with a late age at onset (greater than 35 yr) showed a less pronouced immune response within the CNS, irrespective of the occurrence of disability. The most disabled patients also showed the most severe blood-brain barrier damage as manifested by high mean values of total protein in CSF and low serum/CSF albumin ratios. The patients with severe disability and a long duration of the disease (greater than 10 yr) had the highest content of beta-trace protein in the CSF, probably as a sign of destruction of brain matter.

[Glycine hydrazide as a bifunctional reagent for the synthesis of antigens with steroids as the immunodeterminant groups (author's transl)].

The suitability of glycine hydrazide as a link between steroids and carrier proteins in the synthesis of antigens was examined. Testosterone was used as hapten; bovine serum albumin as carrier protein. The reaction described here of testosterone with glycine hydrazide to form testosterone glycylhydrazone acetate took place under milk conditions and the yield was nearly quantitative. Rabbits immunized with the new antigen developed specific antibodies against testosterone.

[Lipomas of the neck (author's transl)].

Histological particularities, clinical details, diagnosis, malignant transformation and therapy of adipose tissue tumors of the neck are demonstrated. This will be completed by the discription of two cases, concerning this problem.

[Determination of the gestational age in late pregnancy by sonography (author's transl)].

In 219 normal pregnancies between 28 and 42 weeks the relationships of different ultrasound measurements of the fetal head and the fetal body to the gestational age were determined in order to improve the accurate diagnosis of fetal maturity during the last trimester of pregnancy. With the A and B ultrasound apparatus of Kretz, the biparietal diameter was determined by the A mode, the fronto occipital plane and the largest abdominal diamter were determined by the B scan. The following ultrasound measurements were taken: the two horizontal and transverse diameters, the circumferences and the planes. The abdominal measurements showed a higher rate of growth, compared to the cranial measurements a lesser rate of growth retardation and a higher variation of the normals. The correlation of the abdominal measurements with the gestational age was worse than that of the cranial measurements. Wth the exception of the biparietal diameter in the A mode, the mean diameters of the cranial and abdominal measurements gave better results than the single diameters. Circumference and surface measurements showed no advantages. The best results were obtained by combined fetal head and body measurements using the summation of the biparietal diameter in the A mode and the mean abdominal diameter in the B scan.

[Problems of premedication in pediatric gynecologic consultation hours].

It is reported about the behavior of 300 children, which were examined and treated gynecological after premedication, which was performed with Elroguil, Faustan, Lepinal, Myocuran, Sinophebin liqu. The sedative effect was not good in all cases. A premedication is not generally indicated, but in some of the cases the situation of examination is better.

[Oxygen pressure values in the ischemic muscle tissue of patients with occlusive arterial diseases (author's transl)].

The local oxygen pressure of muscle tissue was measured in 10 patients with occlusive atherosclerosis and in 9 healthy volunteers using a micro-Pt-electrode. In 3 cases, the muscular pO2 values of an ischemic leg were compared to the non-ischemic leg of the same patient. In both groups, there was a statistically significant difference in the oxygen pressure of the muscle tissue in normal legs of volunteers as compared to the ischemic legs. Measurements of the local oxygen pressure in the muscle enables us to have a quantitative measure for the ischemic situtation. It is a useful diagnostic method in patients suffering from ischemic claudication and seems to be an objective criterium of therapeutical methods.

[Galactography of the female breast (author's transl)].

Breast lesions were diagnosed in 1518 patients among 8432 who had had mammography [ 19%]. In 324 there was the characteristic appearance of "secretory disease" . Additional uni- or bilateral galactography was performed in 458 womenbecause of breast secretion. A normal milkduct system was found in 29.6% of them; ductalectasia with a lumen above 3 mm diameter was present in 14.6%. The most common cause ofsecretions was small cysts as part of a small-cystic mammary fibrosis or fibrocysticdisease of the breast [ 32.6%]. Intraductal proliferation was present in 21% of allgalactograms, occult carcinoma in 3 [ 0.7%]. It would seem that a secreting breast isnot as common an early symptom of mammary carcinoma as sometimes stated. The most commoncomplication of galactography was mastitis [ more or less severe], almost always re-sponding to oxyphenbutazone administration.

[Alpha-antitrypsin deficiency in infancy].

Clinical, histological (including electron-microscopic), immunohistochemical and genetic studies were performed on two infants with alpha1-antitrypsin deficiency. The clinical picture was one of neonatal biliary stasis. Liver biopsies revealed multiple cytoplasmic acidophilic bodies within many cells of the liver parenchyma which were strongly periodic acid-Schiff-positive, diastase-resistant and stained selectively with fluorescein-labelled rabbit antihuman alpha1-antitrypsin. Ultrastructurally, the bodies were situated within enlarged cisterns of the endoplasmatic reticulum. Both infants were of the protease inhibitor (Pi) phenotype ZZ, having inherited on PiZ gene from each parent. Results of Pi typing of both families were consistent with an autosomal co-dominant inheritance. Both infants are clinically well except for slight hepatomegaly at one year of age. But transaminase and gamma-glutamyl transpeptidase activities have remained elevated.

[Catheter embolisation of a bleeding renal hamartoma, with a contribution to its angiographic diagnosis].

Hamartomas of the kidney, often associated with tuberous sclerosis, have a typical angiographic appearance. Despite the similarity of the angiographic signs with malignant renal tumours, it is possible to differentiate these, as was shown in two patients which we have documented. If there is massive bleeding, fibrin embolisation carried out at the end of a diagnostic angiogram may lead to occlusion of the vessels and stop the bleeding. This was demonstrated in one of our patients.
