The present paper focuses main and different aspects of pneumonia in the elderly, in relation to our recent experience in this field. Presentation, clinical features, cause and therapeutic aspects in geriatric age have important peculiarities (increased incidence of infection and non infection complications, and mortality). In the elderly patient pneumonia with slow resolution is a frequent occurrence. For a correct therapy is essential to distinguish between nosocomial and community acquired pneumonia. In our experience supportive measures seems important as much effective antibiotic therapy that can be carried out with different drugs: cephalosporins, macrolides, penicillines plus beta-lactamasi inhibitors; in case of nosocomial pneumonia and of 'difficult' germs, is more rationale to use antibiotic associations (also with aminoglycosides and/or chinolonics).
Ninety-six cases of fetal macrosomia have been tested to focus on the factors necessary to select women running the risk of delivering macrosomic fetuses. The results of these studies are the following ones: a) pregnant women with pathology are multiparas; b) they are usually characterized by peculiar weight increase in pregnancy; c) they have gone beyond pregnancy term; d) they have previously delivered macrosomic fetuses. The Authors focus on the necessity to anticipate diagnosis and to plan caesarean section in order to avoid the traumatic lesions this kind of babies are frequently affected by.
The authors describe a case of superior vena cava thrombosis with laterocervical lymphoadenopathy (probably of metastatic origin). Vascular tumors are a large neoplasm family with a wide clinical and histological spectrum. They may localize on the skin, soft tissues, liver, spleen and parotid gland. The case describes a 65 year-old male, affected by COLD and chronic hepatitis. He came to the hospital for strong gastric pain, which did not vary with meal assumption and objectivity of inflated superficial veins of arms, jugular veins, chest superficial veins and venous capillars and of the epigastric zone, accompanied with right laterocervical lymphoadenomegaly. Endoscopic ultrasonic and X-ray examinations showed a superior vena cava thrombosis determining superficial collateral, azygos and emiazygos vein inflation. The histologic examination of pathologic lymph nodes resulted as hemangioendotelioma/angiosarcoma, a rare case of lymphonodal vascular tumor with a very poor prognosis.
The aim of this study was to provide a histomorphological analysis of some secondary cartilages of mandible and temporal bone as observed in human fetuses 18-22 weeks old. The behavior of cartilage was studied in both these regions, which were decalcified, cut at 10 mu, stained with Mallory staining and examined by optical microscopy. In mandible symphysis menti and condylar cartilage were described. The symphysis appeared to be formed by a fibrous cartilagineous structure surrounded by membranous bone. This structure seems be round in the caudal sections and ovoidal in the rostral sections with the major axis perpendicular to the mean sagittal plane. Meanwhile the condyle is formed by secondary cartilage which may be appreciated in this development stage 5 zona. Secondary cartilage was observed also in the temporal bone nearby the primitive glenoid fossa. The development and the importance of these cartilagineous structures are discussed.
